    Die Apomorphin HCl Amino Ampulle enthält eine Injektionslösung mit dem Wirkstoff Apomorphinhydrochloridhemihydrat. Die Lösung wird unter die Haut (subkutan) injiziert.

    Apomorphinhydrochloridhemihydrat gehört zu einer Gruppe von Arzneimitteln, die als Dopamin-Agonisten bezeichnet werden. Es wird zur Behandlung der Parkinson-Krankheit bei Patientinnen und Patienten angewendet, bei welchen behindernde motorische Komplikationen trotz individuell eingestellter Behandlung mit Levodopa (und einem peripheren Decarboxylase-Hemmer) und/oder anderen Dopamin-Agonisten weiter bestehen. Apomorphin HCl Amino hilft, die Phasen der Unbeweglichkeit (ON-OFF-Phänomene) zu verkürzen.

    Dieses Arzneimittel darf nur auf Verschreibung des Arztes oder der Ärztin angewendet werden.

    +-Wann darf Apomorphin HCl Amino nicht angewendet werden?

    Apomorphin HCl Amino darf nicht angewendet werden:

    • wenn Sie unter 18 Jahre alt sind,
    • wenn Sie an Atemschwierigkeiten leiden,
    • wenn Sie an Demenz oder Alzheimer leiden,
    • wenn Sie an psychischen Erkrankungen mit Symptomen wie Halluzinationen (Sinnestäuschungen), Wahnvorstellungen, Verwirrtheitszuständen, Realitätsverlust leiden,
    • wenn Sie an einer Lebererkrankung leiden,
    • wenn Sie unter der Anwendung von Levodopa unter schweren Dyskinesien (Störungen des Bewegungsablaufs) oder schweren Dystonien (Bewegungskrämpfen) leiden,
    • wenn Sie überempfindlich gegen Apomorphin oder einen der Hilfsstoffe von Apomorphin HCl Amino sind.
    +-Wann ist bei der Anwendung von Apomorphin HCl Amino Vorsicht geboten?

    Bitte informieren Sie Ihren Arzt bzw. Ihre Ärztin vor der Anwendung von Apomorphin HCl Amino, wenn

    • Sie an einer Nierenerkrankung leiden,
    • Sie an einer Lungenerkrankung leiden,
    • Sie eine Herzerkrankung haben,
    • Sie niedrigen Blutdruck haben oder sich beim Stehen der Ohnmacht nahe und schwindlig fühlen,
    • Sie blutdrucksenkende Arzneimittel einnehmen,
    • Sie zu Übelkeit oder Erbrechen neigen,
    • Ihre Parkinson-Krankheit Ihnen psychische Störungen wie Halluzinationen und Verwirrtheit verursacht,
    • Sie älter oder geschwächt sind.
    • Sie oder Familienangehörige bekanntlich ein auffälliges Elektrokardiogramm (EKG) aufweisen, welches als «Long-QT-Syndrom» bezeichnet wird.

    Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie oder Ihre Familie bzw. Betreuungspersonen bemerken, dass Sie einen Drang oder ein Verlangen entwickeln, sich in einer für Sie ungewöhnlichen Weise zu verhalten, und Sie dem Impuls, dem Trieb oder der Versuchung nicht widerstehen können, bestimmte Dinge zu tun, die Ihnen selbst oder anderen schaden können. Dies nennt man Impulskontrollstörungen und dazu gehören Verhaltensweisen wie zum Beispiel Spielsucht, Kaufsucht, übermässiges Essen oder Geldausgeben, ein abnorm starker Sexualtrieb oder eine Zunahme sexueller Gedanken oder Gefühle.

    Ihr Arzt bzw. Ihre Ärztin muss möglicherweise Ihre Dosis anpassen oder das Arzneimittel absetzen.

    Apomorphin HCl Amino kann die Reaktionsfähigkeit, die Fahrtüchtigkeit und die Fähigkeit Werkzeuge oder Maschinen zu bedienen, beeinträchtigen! Einige Patientinnen und Patienten können sich zu Beginn der Therapie schläfrig oder schwindlig fühlen. Ferner können plötzliches Einschlafen ohne jegliche Warnsymptome von Müdigkeit oder starke Schläfrigkeit tagsüber auftreten. Sie dürfen deshalb während der Behandlung mit Apomorphin HCl Amino kein Motorfahrzeug führen oder andere Aktivitäten ausüben (z.B Maschinen bedienen), bei denen eine verminderte Aufmerksamkeit Sie selbst oder andere gefährdet, falls Sie an plötzlichem Einschlafen oder an starker Müdigkeit tagsüber leiden und bis ausreichend Erfahrungen über die Beeinträchtigungen vorliegen.

    Apomorphin HCl Amino enthält Natriummetabisulfit (E223), welches in seltenen Fällen schwere allergische Reaktionen (Überempfindlichkeitsreaktionen) mit Symptomen wie Ausschlag oder Juckreiz, Atemschwierigkeiten, Schwellung der Augenlider, des Gesichts oder der Lippen, Schwellung oder Rötung des Zunge auslösen kann.

    Informieren Sie Arzt bzw. Ihre Ärztin, vor der Anwendung von Apomorphin HCl, wenn Sie Arzneimittel anwenden, die bekanntlich Ihren Herzschlag beeinflussen. Dies schliesst Arzneimittel gegen Herzrhythmusstörungen (wie Chinidin und Amiodaron), gegen Depression (einschliesslich trizyklische Antidepressiva wie Amitriptylin und Imipramin) und gegen bakterielle Infektionen (Makrolid-Antibiotika wie Erythromycin, Azithromycin und Clarithromycin) und Domperidon mit ein.

    Wenn Sie gleichzeitig noch andere Arzneimittel (z.B. Arzneimittel wie Clozapin zur Behandlung einiger psychischer Störungen, blutdrucksenkende Arzneimittel, andere Arzneimittel gegen die Parkinsonsche Erkrankung) einnehmen, so informieren Sie unbedingt Ihren Arzt bzw. Ihre Ärztin, da Apomorphin HCl die Wirkung dieser Arzneimittel verändern kann.

    Wenn Sie sowohl Levodopa (ein anderes Arzneimittel gegen Parkinson?sche Erkrankung) als auch Apomorphin HCl Amino anwenden, sollte Ihr Arzt bzw. Ihre Ärztin Ihr Blut regelmässig überprüfen.

    Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie

    • an anderen Krankheiten leiden,
    • Allergien haben oder
    • andere Arzneimittel (auch selbst gekaufte!) einnehmen oder äusserlich anwenden!
    +-Darf Apomorphin HCl Amino während einer Schwangerschaft oder in der Stillzeit angewendet werden?

    Apomorphin HCl Amino darf während der Schwangerschaft nur angewendet werden, wenn Ihnen der Arzt oder die Ärztin Apomorphin HCl Amino ausdrücklich verordnet hat. Fragen Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin vor der Anwendung von Apomorphin HCl Amino um Rat, wenn Sie schwanger sind, wenn Sie denken, dass Sie schwanger sind oder wenn Sie eine Schwangerschaft planen.

    Es ist nicht bekannt, ob Apomorphin in die Muttermilch übergeht. Daher sollte das Stillen während der Therapie mit Apomorphin HCl Amino vermieden werden. Sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin, wenn Sie stillen oder vorhaben zu stillen.

    +-Wie verwenden Sie Apomorphin HCl Amino?

    Halten Sie sich genau an die verordnete Dosis und an die nachstehend beschriebene Injektionstechnik. Ihr Arzt oder Ihre Ärztin wird Sie bezüglich Vorbereiten der Spritzen und Injektionstechnik genau informieren.

    Zur Verhinderung von Übelkeit und Erbrechen sollten Sie mindestens 2 Tage vor Behandlungsbeginn mit Apomorphin HCl Amino Domperidon behandelt werden.

    Einzelinjektion

    Um mögliche Nebenwirkungen zu vermeiden, legt Ihr Arzt bzw. Ihre Ärztin die für Sie geeignete und optimale Dosierung von Apomorphin HCl Amino fest.

    Die übliche Dosis beträgt zwischen 3 mg und 30 mg täglich mit 1 bis 10, Injektionen pro Tag. Spritzen Sie nie mehr als 10 ml auf einmal.

    Eine Tagesgesamtdosis von 100 mg darf nicht überschritten werden.

    Dauerinfusion

    Wenn sich Ihre Symptome mit Einzelinjektionen nicht ausreichend beherrschen lassen oder wenn Sie mehr als 10 Injektionen täglich benötigen, ist bei Ihnen eventuell eine Dauerinfusion mit Apomorphin erforderlich. Ihr Arzt bzw. Ihre Ärztin entscheidet, ob Sie diese Form der Behandlung benötigen und welche Dosis für Sie am besten ist.

    Die übliche Dosis beträgt zwischen 1 mg bis 4 mg (0,1-0,4 ml) pro Stunde. Die Infusion wird in der Regel nur durchgeführt, wenn Sie wach sind, vor dem Schlafengehen wird die Infusion normalerweise abgestellt. Die Infusionsstelle ist alle 12 Stunden zu wechseln.

    Vorbereitung und Durchführung der Injektion

    Zur Durchführung der Injektion benötigen Sie eine sterile Spritze (z.B. 1 ml?2 ml) und eine sterile, subkutane Nadel (z.B. 25G, ø 0,5 mm, Länge 14?16 mm vom Typ Insulinnadel).

    Die Ampulle zum Schutz vor Schnittverletzungen mit einem Tuch oder Ähnlichem umwickeln. Suchen Sie den farbigen Punkt am Ampullenhals, er ist die Sollbruchstelle der Ampulle. Öffnen Sie die Ampulle, indem Sie den Kopf der Ampulle durch Druck nach hinten oberhalb der Sollbruchstelle abbrechen.

    Entnehmen Sie die erforderliche Menge an Apomorphin HCl Amino Injektionslösung mit der Spritze. Vor der Injektion müssen eventuelle Luftblasen aus der Spritze entfernt werden, indem Sie die Spritze aufrecht halten und den vorderen Teil (nicht die Nadel) mit dem Fingernagel beklopfen. Drücken Sie dann langsam auf den Spritzenkolben um alle Luftblasen zu entfernen. Hören Sie auf zu drücken, sobald keine Luftblasen mehr in der Spritze sind.

    Apomorphin HCl Amino wird unter die Haut (subkutan) in den Oberschenkel oder die Bauchdecke gespritzt, so wie Ihr Arzt bzw. Ihre Ärztin oder das Pflegepersonal es Ihnen gezeigt hat. Apomorphin HCl Amino darf auf keinen Fall in die Vene gespritzt werden.

    Desinfizieren Sie die entsprechende Hautstelle, nehmen Sie eine Hautfalte zwischen Daumen und Zeigefinger und stechen Sie in einem leicht schrägen Winkel in die Hautfalte.

    Die Spritze darf nur einmal verwendet werden. Die benutzten Spritzen, Nadeln und Ampullen müssen in einem dazu geeigneten Behälter entsorgt werden.

    Die Lösung muss nach dem Öffnen der Ampulle sofort verwendet werden. Nicht verwendete Lösung muss verworfen werden.

    Dieses Arzneimittel darf bei Kindern und Jugendlichen unter 18 Jahren nicht angewendet werden.

    Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin.

    Wenn Sie eine grössere Menge von Apomorphin HCl Amino angewendet haben, als Sie sollten

    • Informieren Sie sofort Ihren Arzt bzw. Ihre Ärztin oder die Notaufnahme des nächsten Krankenhauses.
    • Es können verlangsamter Puls, übermässige Übelkeit, extreme Schläfrigkeit und Atembeschwerden auftreten. Besonders beim Aufstehen können Sie sich ausserdem aufgrund von niedrigem Blutdruck der Ohnmacht nahe oder schwindlig fühlen. Möglicherweise hilft es Ihnen, sich hinzulegen und die Füsse hochzulegen.

    Wenn Sie die Anwendung von Apomorphin HCl Amino vergessen haben

    Spritzen Sie die nächste Dosis, wenn Sie sie benötigen. Nehmen Sie nicht die doppelte Dosis, wenn Sie die vorherige Anwendung vergessen haben.

    Wenn Sie die Anwendung von Apomorphin HCl Amino abbrechen

    Brechen Sie die Behandlung mit Apomorphin HCl Amino  nicht ab, ohne vorher Ihren Arzt bzw. Ihre Ärztin zufragen.

    Wenn Sie weitere Fragen zur Anwendung des Arzneimittels haben, fragen Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin oder Ihr Pflegepersonal.

    +-Welche Nebenwirkungen kann Apomorphin HCl Amino haben?

    Wenn Sie eine allergische Reaktion haben, beenden Sie die Anwendung von Apomorphin HCl Amino und kontaktieren Sie sofort Ihren Arzt bzw. Ihre Ärztin oder die Notaufnahme des nächsten Krankenhauses.

    Anzeichen einer allergischen Reaktion können sein:

    • Hautausschlag
    • Atembeschwerden
    • Schwellung der Augenlider, des Gesichtes, der Lippen, des Hals-Rachenbereichs oder der Zunge

    An der Injektionsstelle kann sehr häufig nach der Injektion eine Verhärtung oder Knotenbildung auftreten. Diese können bei längerfristiger Anwendung bestehen bleiben und mit Rötung und Empfindlichkeit verbunden sein. Es sollte Sorge getragen werden, dass sich Bereiche mit Wundstellen nicht infizieren. Durch Abwechseln der Injektionsstellen, können diese Erscheinungen manchmal reduziert werden.

    Weiter kann es in häufigen Fällen zu Übelkeit und Erbrechen, Schläfrigkeit oder Müdigkeit kommen. Gelegentlich wurden Verwirrtheitszustände oder Trugbilder (Halluzinationen), Blutdruckabfall bei Lagewechsel mit Benommenheit, Hautausschläge, Atembeschwerden und unwillkürliche Bewegungen in «On»-Phasen beobachtet. Bei Patienten, die mit Levodopa und Apomorphin behandelt werden, kann es zu Blutarmut (Anämie) kommen.

    Es gibt Patientinnen und Patienten, die unter der Anwendung von Apomorphin und/oder anderen Arzneimitteln zur Behandlung der Parkinson-Krankheit für sie ungewöhnliche Verhaltensweisen zeigen wie Spielsucht, Kaufsucht, übermässiges Essen oder Geldausgeben, ein abnorm starker Sexualtrieb oder eine Zunahme sexueller Gedanken oder Gefühle. Falls Sie bei sich selber oder Ihre Angehörigen oder Betreuungspersonen bei Ihnen die Entwicklung von für Sie untypischen, ungewöhnlichen Verhaltensweisen feststellen, besprechen Sie dies mit Ihre Arzt oder Ihrer Ärztin.

    Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden.
