Lasix, mit dem Wirkstoff Furosemid, ist ein harntreibendes Medikament, das Flüssigkeitsansammlungen (Ödeme) infolge Erkrankungen des Herzens, der Leber, der Nieren oder nach Verbrennungen aus den Geweben ausschwemmt. Die vermehrte Harnausscheidung beginnt etwa 1 Stunde nach Einnahme von Lasix und hält 6-8 Stunden an.
Lasix senkt auch den erhöhten, nicht aber den normalen Blutdruck, dies wenn nötig zusammen mit anderen blutdrucksenkenden Mitteln.
Lasix darf nur auf Verschreibung des Arztes bzw. der Ärztin eingenommen werden.
Die Behandlung mit Lasix bedarf der regelmässigen ärztlichen Kontrolle.
Da die Anwendung von Lasix zu einem Absinken des Kaliumspiegels im Blut führen kann, ist eine kaliumreiche Kost (mageres Fleisch, Kartoffeln, Bananen, Tomaten, Blumenkohl, Spinat, getrocknete Früchte usw.) empfehlenswert.
Gelegentlich kann eine medikamentöse Substitution von Kalium angezeigt sein.
Lasix darf nicht angewendet werden bei Nierenversagen mit fehlender Harnproduktion, bei Leberversagen mit Bewusstseinsstörungen, bei schwerem Kaliummangel (z.B. infolge längerdauerndem Erbrechen, schweren oder chronischen Durchfällen, Leberfunktionsstörungen, übermässigem Gebrauch von Abführmitteln), schwerem Natriummangel, reduziertem Blutvolumen oder Körperwasser, niedrigem Blutdruck, bei Überempfindlichkeit gegen Furosemid, Sulfonamide und in der Stillzeit.
Eine besonders sorgfältige Überwachung ist erforderlich bei:
Während einer langdauernden Therapie sollten bestimmte Blutlaborwerte regelmässig kontrolliert werden. Ein reduziertes Blutvolumen, ein Mangel an Körperwasser, bedeutende Elektrolytstörung und Störungen des Säure-Base-Haushalts müssen ausgeglichen werden. Besondere Überwachung ist bei Patienten mit hohem Risiko von Elektrolytstörungen notwendig sowie im Fall von zusätzlichem Flüssigkeitsverlust durch z.B. Schwitzen, Durchfall oder Erbrechen.
Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fähigkeit Werkzeuge oder Maschinen zu bedienen und die Fahrtüchtigkeit beeinträchtigen.
Kombination mit gewissen Antibiotika oder allgemein hörschädlichen Substanzen, welche das Risiko von Hör- und Nierenstörungen mit sich bringt.
In Einzelfällen kann es nach intravenöser Gabe von Lasix innerhalb von 24 Stunden nach Einnahme von Chloralhydrat zu Hitzegefühl, Schweissausbruch, Unruhe, Übelkeit, Blutdruckanstieg und Herzjagen kommen. Deshalb wird die gleichzeitige Anwendung von Furosemid und Chloralhydrat nicht empfohlen.
Wechselwirkungen können mit Genussmitteln wie Lakritze und mit Präparaten, die den Salzhaushalt beeinflussen, z.B. Abführmittel (besonders bei längerem Gebrauch), andere wasserausschwemmende Präparate, bestimmte Hormone, ein bestimmtes Mittel gegen Magengeschwüre (Carbenoxolon) eintreten und zu Kaliummangel führen.
Liegt Kalium- oder Magnesiummangel vor, können die Wirkungen und Nebenwirkungen von bestimmten Herzpräparaten (sog. Herzglykoside, z.B. Digoxin) verstärkt werden.
Die Wirkung anderer blutdrucksenkender Mittel kann durch Lasix verstärkt werden. Insbesondere in Kombination mit sog. ACE-Hemmern wurden starke Blutdruckabfälle bis hin zum Schock beobachtet. Die Gabe von ACE-Hemmern oder Angiotensin-II-Rezeptor-Antagonisten (auch eine Dosiserhöhung) kann bei mit Lasix vorbehandelten Patienten zu einer Verschlechterung der Nierenfunktion und in Einzelfällen zum akuten Nierenversagen führen. Die Lasix-Verabreichung soll vorübergehend unterbrochen oder in seiner Dosierung für 3 Tage reduziert werden, bevor ein ACE-Hemmer oder Angiotensin-II-Rezeptor-Antagonist gegeben oder deren Dosis gesteigert wird.
Lasix kann die unerwünschten Wirkungen nierenschädlicher Arzneimittel verstärken. Bei gleichzeitiger Behandlung mit Lasix und bestimmten Antibiotika (bestimmte Cephalosporine in hoher Dosierung, z.B. Cephaloridine, Cephalotin) kann die Nierenfunktion beeinträchtigt werden.
Bestimmte Schmerzmittel (z.B. Acetylsalicylsäure, Indometacin) können die Wirkung von Lasix abschwächen und bei bestehendem Mangel an Blutvolumen oder Körperwasser zu Nierenversagen führen. Die Toxizität hochdosierter Salicylate kann bei gleichzeitiger Anwendung von Lasix verstärkt werden (verstärkte Nebenwirkungen der Acetylsalicylsäure). Ein bestimmtes Gichtmittel (Probenecid), ein Antiepileptikum (Phenytoin) sowie Methotrexat können die Wirkung von Furosemid ebenfalls abschwächen Anderseits verstärkt Lasix die Wirkung bestimmter Präparate (z.B. Schmerzmittel, Theophyllin - ein bestimmtes Mittel gegen Asthma - und bestimmte Muskelrelaxanzien) oder bewirkt eine Abschwächung (z.B. von Mitteln gegen die Zuckerkrankheit oder von gefässverengenden Substanzen wie Epinephrin und Norepinephrin).
Da der Wirkstoff Furosemid den Blutharnsäurespiegel erhöhen kann und Ciclosporin (ein Transplantationsarzneimittel und ein das Immunsystem beeinflussendes Arzneimittel) die Ausscheidung von Harnsäure durch die Niere vermindert, ist das Risiko einer Gichtarthritis bei gleichzeitiger Behandlung von Furosemid und Ciclosporin A erhöht.
Bei gleichzeitiger Anwendung von Lasix und Cisplatin (ein Mittel zur Krebsbehandlung) ist mit der Möglichkeit eines Hörschadens zu rechnen.
Die gleichzeitige Gabe von Lasix und Lithium führt zu einer Verstärkung der herz- und nervenschädigenden Wirkung des Lithiums. Der Lithiumspiegel muss in Kombination mit Furosemid sorgfältig überwacht werden.
Die Resorption von Furosemid kann durch Sucralfat, ein Präparat gegen Magengeschwüre, beeinträchtigt werden. Die Einnahme muss 2 Stunden voneinander getrennt erfolgen.
Bei gleichzeitiger Anwendung von Risperidon Vorsicht walten lassen sowie Risiken und Nutzen einer Kombination oder gleichzeitigen Behandlung mit Furosemid oder anderen potenten Diuretika vor Therapieentscheidung sorgfältig abwägen.
Ihr Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin sind über Wechselwirkungen von Arzneimitteln informiert.

Aufgrund der bisherigen Erfahrungen ist bei bestimmungsgemässer Anwendung kein Risiko für das Kind bekannt. Systematische wissenschaftliche Untersuchungen wurden aber nie durchgeführt. Vorsichtshalber sollten Sie während der Schwangerschaft möglichst auf Arzneimittel verzichten oder den Arzt oder Apotheker bzw. die Ärztin oder Apothekerin um Rat fragen.
Eine Behandlung während der Schwangerschaft erfordert die Überwachung des Fötuswachtums.
Der Wirkstoff von Lasix, Furosemid, gelangt in die Muttermilch und hemmt die Milchproduktion. Lasix sollte während der Stillzeit nicht eingenommen oder angewendet werden. Ist eine Einnahme unumgänglich, wird empfohlen abzustillen.
Soweit vom Arzt bzw. von der Ärztin nichts anderes verordnet, gelten folgende Dosierungsrichtlinien:
Erwachsene erhalten im allgemeinen zu Beginn ½ bis 2 Tabletten. Als Erhaltungsdosis genügt meist ½ bis 1 Tablette.
Lasix nüchtern einnehmen. Die Tabletten sind unzerkaut mit etwas Flüssigkeit einzunehmen.
Säuglingen und Kindern wird Lasix vornehmlich oral verabreicht. Dosierungsempfehlung: 2 mg Furosemid/kg KG bis höchstens 40 mg/Tag.
Über die Dauer der Behandlung entscheidet der Arzt bzw. die Ärztin. Sie richtet sich nach Art und Schwere der Erkrankung.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin.
Folgende Nebenwirkungen können bei der Einnahme oder Anwendung von Lasix auftreten:
Als Folge der vermehrten Harn- und Elektrolytausscheidung werden häufig Störungen im Flüssigkeits- und Elektrolythaushalt beobachtet.
Harnausscheidung kann insbesondere bei älteren Patienten und Kindern zu reduziertem Blutvolumen und Körperwasser mit Kreislaufbeschwerden hervorrufen, die sich vor allem als Kopfschmerz, Schwindel, Schwäche, Schläfrigkeit, Sehstörungen, Mundtrockenheit, erniedrigtem Blutdruck und Kreislaufstörungen beim Aufstehen äussern. Zudem kann es infolge einer verminderten zirkulierenden Blutmenge zur Bluteindickung mit - besonders bei älteren Patienten - Thromboseneigung kommen.
Infolge erhöhter Natriumverluste über die Niere kann es - insbesondere bei eingeschränkter Zufuhr von Kochsalz - zu einem Natriummangel kommen. Dieser äussert sich häufig in folgenden Symptomen: Wadenkrämpfe, Appetitlosigkeit, Schwächegefühl, Schläfrigkeit, Teilnahmslosigkeit, Verwirrtheit und Erbrechen.
Kaliumverluste können sich vornehmlich nach kaliumarmer Kost sowie nach Erbrechen oder chronischem Durchfall einstellen. Ein Kaliummangel kann sich in Muskelschwäche sowie unvollständigen oder vollständigen Lähmungen, Erbrechen, Verstopfung und übermässiger Gasansammlung in Magen und Darm, übermässiger Harnausscheidung, gesteigertem Durstgefühl und übermässigem Trinken, Reizbildungs- und Reizleitungsstörungen des Herzens äussern. Schwere Kaliumverluste können zur Darmlähmung oder zu Bewusstseinsstörungen bis hin zum Koma führen.
Magnesiummangelzustände nach Magnesiumverlust mit Auftreten von Krampfzuständen oder Herzrhythmusstörungen wurden beobachtet.
Furosemid kann den Kalziumspiegel im Blut erniedrigen (in sehr seltenen Fällen sind Krampfzustände beobachtet worden).
Bei Frühgeborenen können sich kalziumhaltige Nierensteine bilden und Kalziumsalze im Nierengewebe einlagern.
Symptome einer Harnabflussbehinderung können durch harntreibende Medikamente manifestiert oder verschlechtert werden. Akute Harnretention mit Sekundärkomplikationen kann die Folge sein.
Magen-Darm-Störungen (Übelkeit, Erbrechen, Durchfall) kommen selten vor. In Einzelfällen können Gelbsucht durch Gallenstauung, akute Bauchspeicheldrüsenentzündung und erhöhte Leberwerte auftreten.
Gelegentlich können allergische Reaktionen (Haut- und Schleimhautreaktionen, Juckreiz, Nesselsucht, blasige Hauterscheinungen, Hautrötungen, Ekzeme, Haut- und Schleimhautblutungen) aber auch fieberhafte Zustände, Gefäss- oder Nierenentzündungen auftreten.
Sehr selten, fleckenhafte und pappelnbildende Hautausschläge und andere Ausschläge.
Blutbildveränderungen kommen selten vor. Dabei kann es unter Umständen zu verstärkter Infektions- und Blutungsneigung kommen. Blutbildveränderungen können lebensbedrohlich sein.
Selten kommt es zu Empfindungsstörungen oder Fieber.
Gelegentlich zu einer Lichtüberempfindlichkeit der Haut.
Es kann in seltenen Fällen zu meist reversiblen Hörstörungen und/oder zu Ohrgeräuschen kommen. Unter Furosemid wurden Fälle von Schwerhörigkeit beschrieben, die gelegentlich nicht reversibel waren.
Bei einer bestehenden Zuckerkrankheit kann sich die Stoffwechsellage verschlechtern; bei entsprechender Veranlagung kann eine Zuckerkrankheit in Erscheinung treten.
Bestimmte Blutlaborwerte können sich verändern.
Bei Gichtpatienten kann es zu einer Auslösung von Gichtanfällen kommen.
Die Einnahme von Lasix kann einen vorbestehenden Lupus verschlimmern oder diese Autoimmunerkrankung aktivieren.
Bei Auftreten von genannten Nebenwirkungen oder wenn Sie Nebenwirkungen beobachten, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Arzneimittel nicht in Reichweite von Kindern aufbewahren. Lasix ist bei Raumtemperatur (15-25 °C) und vor Licht geschützt aufzubewahren. Bei zu starker Lichteinwirkung kann es zu Verfärbungen kommen, wodurch die Wirksamkeit jedoch nicht beeinträchtigt wird.
Dieses Arzneimittel darf nicht nach dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden. Sollten Sie Arzneimittel besitzen, deren Haltbarkeit abgelaufen ist, so bringen Sie diese bitte in Ihre Apotheke zur Vernichtung zurück.
