Venlafaxin Spirig HC ist ein Antidepressivum, welches zu einer Gruppe von Arzneimitteln gehört, die als Serotonin-Noradrenalin-Wiederaufnahme-Hemmer (SNRI) bezeichnet wird. Venlafaxin Spirig HC enthält den Wirkstoff Venlafaxin, der die Funktion gewisser körpereigener Überträgerstoffe im Gehirn (v.a. Noradrenalin und Serotonin) beeinflusst und dadurch zu einer stimmungsaufhellenden Wirkung bei Depression führt.
Venlafaxin Spirig HC wird nur auf Verschreibung des Arztes oder der Ärztin bei Erwachsenen zur Behandlung von Verstimmungszuständen verschiedenen Schweregrades und verschiedener Ursache eingesetzt. Venlafaxin Spirig HC kann auch das Wiederauftreten depressiver Symptome sowie neuer Episoden verhindern. Die Behandlung mit Venlafaxin Spirig HC ist auch für ältere Patienten bzw. Patientinnen geeignet.
Die Verstimmungszustände äussern sich durch Niedergeschlagenheit, Traurigkeit, Freudlosigkeit, Erschöpfung, Wertlosigkeits- und Schuldgefühle, verlangsamtes Denken, Konzentrationsschwierigkeiten, Schlafstörungen, Krankheitsgefühl, Interesselosigkeit für alltägliche Tätigkeiten, Erregung und/oder Angst. Eine angemessene Behandlung der Depression ist wichtig, um Ihnen dabei zu helfen, sich davon zu erholen. Wenn sie nicht behandelt werden, kann Ihr Leiden anhalten, es kann ernster werden und schwieriger zu behandeln sein.
Die Wirkung von Venlafaxin Spirig HC tritt in der Regel zwischen 1 und 4 Wochen ein.
Bei bekannter Überempfindlichkeit gegen einen der in Venlafaxin Spirig HC enthaltenen Stoffe.
Venlafaxin Spirig HC soll weder zusammen, noch innerhalb der ersten zwei Wochen nach dem Absetzen von sogenannten MAO-Hemmern (werden u.a. zur Behandlung von Depressionen oder der Parkinson-Krankheit eingesetzt) eingenommen werden, da diese Kombination zu schweren Nebenwirkungen führen kann. Zwischen dem Absetzen von Venlafaxin Spirig HC und dem Beginn einer Therapie mit einem MAO-Hemmer sollten mindestens 7 Tage verstreichen. Ein Therapiewechsel darf nur unter sorgfältiger ärztlicher Kontrolle erfolgen.
Venlafaxin Spirig HC darf bei Kindern und Jugendlichen unter 18 Jahren nicht angewendet werden.
Eine lebensbedrohliche Situation, das sogenannte Serotonin-Syndrom (ernsthafte Veränderungen der Funktionen im Gehirn, in den Muskeln und im Verdauungstrakt durch hohe Serotoninwerte im Körper) kann unter der Behandlung mit Venlafaxin auftreten, insbesondere wenn es gleichzeitig mit anderen Arzneimitteln eingenommen wird, welche den Serotoninhaushalt beeinflussen. Das sind Arzneimittel zur Behandlung der Migräne, die sogenannten Triptane, Arzneimittel gegen Depressionen wie selektive Serotonin-Wiederaufnahme-Hemmer (SSRIs), Serotonin- und Noradrenalin-Wiederaufnahmehemmer (SNRIs) und trizyklische Antidepressiva, Lithium, die Schmerzmittel Tramadol, Fentanyl und Tapentadol, Dextromethorphan (Hustenstiller), Methadon, Johanniskraut-Präparate (Naturheilmittel gegen leichte Depressionen), MAO-Hemmer (inkl. dem Antibiotikum Linezolid und Methylenblau) und Tryptophan-Supplementa (bei Schlafbeschwerden und Depression). Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie andere Arzneimittel einnehmen oder beabsichtigen diese einzunehmen, inklusive pflanzliche Stoffe oder Nahrungsergänzungsmittel.
Das Serotonin-Syndrom kann sich durch eine Kombination einiger der folgenden Symptome bemerkbar machen: übermässige Reflexe, Zittern, Bewusstseinsverlust, Muskelkrämpfe, Unruhe, Angst, Wahnvorstellungen, Reizbarkeit, Delirium und Koma, schneller Puls, rasche Veränderungen des Blutdruckes, erhöhte Körpertemperatur, Übelkeit, Erbrechen und Durchfall. Benachrichtigen Sie Ihren Arzt bzw. Ihre Ärztin unverzüglich, falls solche Symptome auftreten.
Bei gleichzeitiger Anwendung folgender Arzneimittel mit Venlafaxin Spirig HC ist ebenfalls Vorsicht geboten und Sie müssen Ihren Arzt/Ihre Ärztin informieren, wenn Sie Arzneimittel einnehmen, die folgende Wirkstoffe enthalten: Ketoconazol (Pilzinfektionen), Haloperidol, Risperidon und Clozapin (sogenannte Neuroleptika bei psychiatrischen Erkrankungen), Metoprolol (Bluthochdruck und Herzbeschwerden), Cimetidin (Magenübersäuerung), Arzneimittel wie Beruhigungsmittel und Arzneimittel gegen Depressionen. Da es zu Wechselwirkungen kommen kann wird Ihr Arzt/Ihre Ärztin falls nötig die Dosis entsprechend anpassen.
Venlafaxin Spirig HC sollte nicht zusammen mit Appetitzüglern eingenommen werden. Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie Arzneimittel gegen Aids einnehmen.
Venlafaxin Spirig HC soll weder gleichzeitig noch innerhalb der ersten Monate nach dem Absetzen von Arzneimitteln gegen Herzrhythmusstörungen, die den Wirkstoff Amiodaron enthalten, eingenommen werden.
Wenn Sie depressiv sind, können Sie manchmal Gedanken haben, sich selbst zu verletzen oder Suizid zu begehen. Solche Gedanken können bei erstmaliger Anwendung von Antidepressiva verstärkt sein, denn diese Arzneimittel benötigen einige Zeit (2 Wochen oder mehr) bis sie wirken. Das Auftreten solcher Gedanken ist wahrscheinlicher, wenn Sie schon früher solche Gedanken hatten, oder wenn Sie ein junger Erwachsener sind. Daten aus klinischen Studien haben gezeigt, dass junge Erwachsene unter 25 Jahren ein erhöhtes Risiko für suizidale Verhaltensweisen haben können, wenn sie Antidepressiva einnehmen. Bei einer Verschlechterung der Symptome der Depression sollten Sie Ihren Arzt bzw. Ihre Ärztin sofort kontaktieren.
Sie müssen Ihren Arzt bzw. Ihre Ärztin informieren, wenn Sie früher unter aggressivem Verhalten gelitten haben, wenn Ihre Vorgeschichte manische Episoden (Gefühl übererregt oder euphorisch zu sein) bzw. eine bipolare Störung (abwechselnd depressiv und manisch) aufweist oder wenn jemand in Ihrer Familie daran gelitten hat.
Informieren Sie Ihren Arzt bzw. Ihre Ärztin, falls Sie an Krampfanfällen (Epilepsie) leiden. Sollten unter der Behandlung mit Venlafaxin Spirig HC Krampfanfälle auftreten, müssen Sie Ihren Arzt bzw. Ihre Ärztin sofort informieren und Venlafaxin Spirig HC absetzen.
Bei Patienten und Patientinnen, die an Nieren- und Leberfunktionsstörungen leiden, ist die Therapie vom Arzt bzw. der Ärztin zu überwachen und die Dosis gegebenenfalls entsprechend seinen/ihren Anweisungen zu reduzieren. Dialysepflichtige Patienten bzw. Patientinnen erhalten eine um 50% reduzierte Dosis.
Venlafaxin Spirig HC sollte mit Vorsicht angewendet werden bei Patienten mit Herzerkrankungen.
Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie bekanntermassen an einer Herzerkrankung oder unregelmässigem oder verlangsamtem Herzschlag (Herzrhythmusstörungen) leiden, kürzlich einen Herzinfarkt hatten oder eine Auffälligkeit im Elektrokardiogramm (EKG) haben, die «Verlängerung der QT-Zeit» genannt wird.
Es kann eine dosisabhängige Blutdruckerhöhung auftreten. Daher ist eine regelmässige Blutdruckkontrolle angezeigt. Informieren Sie Ihren Arzt bzw. Ihre Ärztin, falls Sie an Bluthochdruck leiden.
Über Mundtrockenheit wird bei mehr als 10% der mit Venlafaxin behandelten Patienten und Patientinnen berichtet. Dies kann das Risiko für Karies erhöhen. Daher sollten Sie besonders auf Ihre Zahnhygiene achten.
Bei entsprechend veranlagten Patienten bzw. Patientinnen können während der Behandlung mit Venlafaxin Spirig HC vermehrt Haut- oder Schleimhautblutungen auftreten. Informieren Sie Ihren Arzt bzw. Ihre Ärztin, falls
bei Ihnen die Neigung besteht, blaue Flecken zu entwickeln oder leicht zu bluten (frühere Blutgerinnungsstörungen), oder wenn Sie andere Arzneimittel einnehmen, die das Blutungsrisiko erhöhen können.
Informieren Sie Ihren Arzt bzw. Ihre Ärztin fallsSie an erhöhten Cholesterinwerten leiden.
Bei Patienten bzw. Patientinnen, die an einem Glaukom (erhöhter Augeninnendruck) leiden, ist Venlafaxin Spirig HC vorsichtig anzuwenden.
Ebenfalls mit Vorsicht soll Venlafaxin Spirig HC bei Patienten bzw. Patientinnen mit ungenügender Flüssigkeitszufuhr oder vermehrtem Wasserverlust (Urin, Schwitzen), einschliesslich älterer Patienten bzw. Patientinnen und Patienten bzw. Patientinnen unter Diuretikatherapie (Mittel zur Erhöhung der Wasserausscheidung) eingesetzt werden.
Informieren Sie Ihren Arzt bzw. Ihre Ärztin, falls Sie in der Vergangenheit niedrige Natrium-Blutwerte (Hyponatriämie) hatten.
Beim Auftreten einer allergischen Reaktion, wie z.B. Ausschlag oder Nesselfieber, informieren Sie bitte sofort Ihren Arzt bzw. Ihre Ärztin.
Venlafaxin Spirig HC Filmtabletten enthalten den Azofarbstoff Gelborange S (E110). Patientinnen und Patienten, die auf Azofarbstoffe, Acetylsalicylsäure sowie Rheuma- und Schmerzmittel (Prostaglandinhemmer) überempfindlich reagieren, sollen Venlafaxin Spirig HC Filmtabletten nicht anwenden.
Venlafaxin Spirig HC kann das Gefühl der Ruhelosigkeit oder einer Unfähigkeit, still zu sitzen oder zu stehen, hervorrufen. Sie sollten es Ihrem Arzt/Ihrer Ärztin mitteilen, wenn dies bei Ihnen zutrifft.
Sie sollten Alkohol meiden, während Sie Venlafaxin Spirig HC einnehmen.
Die Behandlung mit Venlafaxin Spirig HC soll wegen möglicher unerwünschter Wirkungen nicht abrupt abgesetzt werden. Es wurden folgende Absetzsymptome beobachtet: Angstgefühle oder Erregtheit, Kopfschmerzen, Grippe-ähnliche Symptome, Schlafstörungen (einschliesslich Schlaflosigkeit und intensiver Träume), Übelkeit, Erbrechen, Empfindungsstörungen (einschliesslich Parästhesien [Missempfinden, z.B. Kribbeln oder taubes, schmerzhaft brennendes Gefühl]), Schwindelgefühl und Zittern. Die Mehrzahl der Entzugssymptome sind nicht schwerwiegend und klingen spontan ab.
Venlafaxin Spirig HC kann gewisse Laboruntersuchungen zum Nachweis anderer Arzneimittel oder Drogen stören. Wenn Sie sich einem Labortest unterziehen müssen, informieren Sie Ihren Arzt oder Ihre Ärztin bzw. das Spital, dass Sie Venlafaxin Spirig HC einnehmen.
Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fahrtüchtigkeit und die Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigen! Individuelle Reaktionen zu Beginn der Behandlung und deren Bedeutung beim Bedienen von Maschinen und beim Führen von Fahrzeugen sollten von Ihrem Arzt bzw. von Ihrer Ärztin beurteilt werden.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden oder früher gelitten haben, Allergien haben oder andere Arzneimittel (auch selbstgekaufte) einnehmen bzw. äusserlich anwenden!
Teilen Sie Ihrem Arzt bzw. Ihrer Ärztin mit, wenn Sie schwanger sind oder eine Schwangerschaft planen. Venlafaxin Spirig HC darf während der Schwangerschaft ausschliesslich auf Verschreibung des Arztes oder der Ärztin verwendet werden.
Vergewissern Sie sich, dass Ihre Hebamme und/oder Ihr Arzt/Ihre Ärztin darüber informiert ist, dass Sie Venlafaxin Spirig HC einnehmen. Durch Anwendung ähnlicher Arzneimittel (SSRI) während der Schwangerschaft kann das Risiko einer schwerwiegenden Erkrankung beim Kind erhöht sein. Es handelt sich dabei um den andauernden Bluthochdruck im Lungenkreislauf (persistierende pulmonale Hypertonie) beim Neugeborenen. Dabei atmet das Kind schneller und hat ein bläuliches Aussehen. Diese Symptome beginnen gewöhnlich während der ersten 24 Stunden nach der Geburt. Wenn diese Symptome bei Ihrem Kind auftreten, sollten Sie sich sofort an Ihre Hebamme und/oder Ihren Arzt/Ihre Ärztin wenden.
Wenn Mütter in den späten Stadien der Schwangerschaft Venlafaxin Spirig HC einnehmen, können bei den Neugeborenen die folgenden Symptome sofort oder kurz nach der Geburt auftreten: Ess- und Schlafstörungen, Atmungsschwierigkeiten, Krampfanfälle, Schwierigkeiten bei der Regelung der Körpertemperatur, niedriger Blutzuckerspiegel, Zittern, angespannte oder übermässig entspannte Muskulatur, Erbrechen, Reizbarkeit und ständiges Weinen. Falls Ihr Neugeborenes irgendeines dieser Symptome aufweist, kontaktieren Sie Ihren Arzt oder Ihre Ärztin.
Venlafaxin Spirig HC geht in die Muttermilch über. Es besteht das Risiko einer Wirkung auf das Kind. Während der Behandlung mit Venlafaxin Spirig HC darf nicht gestillt werden. Ist die Einnahme des Präparates unabdingbar, soll abgestillt werden.
Die Dosierung und Behandlungsdauer wird von Ihrem Arzt bzw. Ihrer Ärztin individuell für Sie angepasst.
Die übliche empfohlene Tagesdosis zur Behandlung der Depression beträgt 75mg, d.h. 2x 1 Venlafaxin Spirig HC Filmtablette zu 37.5 mg täglich. Falls erforderlich, kann die Dosis vom Arzt bzw. von der Ärztin schrittweise erhöht werden, bis die gewünschte Wirkung eintritt, um dann wieder bis zur üblichen Dosierung reduziert zu werden. Aus diesem Grund kann Ihr Arzt bzw. Ihre Ärztin Ihnen die pro Tag verordnete Anzahl Filmabletten erhöhen. Die maximale Tagesdosis beträgt 375 mg.
Die Einnahme sollte vorzugsweise während einer Mahlzeit erfolgen.Wenn Sie eine grössere Menge Venlafaxin Spirig HC eingenommen haben als Sie sollten, konsultieren Sie umgehend Ihren Arzt/Ihre Ärztin. Die Symptome einer eventuellen Überdosierung können einen raschen Herzschlag, Änderungen des Wachheitsgrades (von Schläfrigkeit bis Koma reichend), verschwommenes Sehen, Krämpfe oder Anfall und Erbrechen einschliessen.
Wenn Sie die Einnahme einer Dosis von Venlafaxin Spirig HC vergessen haben, nehmen Sie diese ein, sobald Ihnen dies auffällt. Wenn aber bereits die Einnahme der nächsten Dosis fällig ist, überspringen Sie die versäumte Dosis und nehmen nur eine einzelne Dosis wie üblich ein.
Beenden Sie nicht abrupt die Behandlung oder reduzieren Sie nicht die Dosis, ohne Rücksprache mit Ihrem Arzt/Ihrer Ärztin gehalten zu haben, selbst wenn Sie sich besser fühlen. Eine Reduktion der Dosis oder mögliche Beendigung der Behandlung sollte stufenweise und langsam und in Absprache mit Ihrem Arzt bzw. Ihrer Ärztin erfolgen. Es ist bekannt, dass Nebenwirkungen auftreten, wenn die Einnahme von Venlafaxin Spirig HC beendet wird, vor allem wenn Venlafaxin Spirig HC plötzlich abgesetzt oder die Dosis zu schnell reduziert wird. Es können folgende Beschwerden auftreten: Angstgefühle oder Erregtheit, Kopfschmerzen, Grippe-ähnliche Symptome, Schlafstörungen (einschliesslich Schlaflosigkeit und intensiver Träume), Übelkeit, Erbrechen, Empfindungsstörungen (einschliesslich Parästhesien [Missempfindungen, z.B. Kribbeln oder taubes, schmerzhaft brennendes Gefühl]), Schwindelgefühl und Zittern.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Unerwünschte Wirkungen treten eher zu Beginn der Behandlung oder bei Einnahme hoher Dosen auf. Im Allgemeinen nehmen die Symptome im weiteren Verlauf der Behandlung an Intensität und Häufigkeit wieder ab.
Bei Auftreten der folgenden Nebenwirkungen brechen Sie die Behandlung unverzüglich ab und kontaktieren Sie Ihren Arzt/Ihre Ärztin bzw. suchen Sie die Notfallstation des nächstgelegenen Spitals auf: Engegefühl in der Brust; pfeifendes Atmen, Schwierigkeiten beim Schlucken oder Atmen, Anschwellen des Gesichts, des Rachens, der Hände oder Füsse; Nervosität oder Angst; Schwindelgefühl; pochende Empfindung; plötzliches Erröten der Hände und/oder Wärmegefühl; starker Ausschlag; Jucken oder Nesselsucht.
Wenn Sie eines der folgenden Anzeichen bei sich bemerken, benötigen Sie möglicherweise dringend ärztliche Hilfe:
Herzbeschwerden (beschleunigte oder unregelmässige Herzfrequenz, erhöhter Blutdruck).
Beschwerden der Augen (verschwommenes Sehen, geweitete Pupillen).
Beschwerden des Nervensystems (Schwindelgefühl, Kribbeln, Bewegungsstörungen, Krämpfe oder Anfall).
Psychiatrische Beschwerden wie Hyperaktivität und Euphorie.
Folgende Nebenwirkungen können bei der Einnahme von Venlafaxin Spirig HC auftreten:
Zentrales Nervensystem: Schläfrigkeit, Schlaflosigkeit, Mundtrockenheit, Schwindelgefühl, Angstgefühl, Nervosität, Kopfschmerzen, Zittern, ungewöhnliche Träume, Parästhesien (Missempfinden, z.B. Kribbeln oder taubes, schmerzhaft brennendes Gefühl), Benommenheit, Verwirrtheit, Erregung, Gefühl, übererregt oder euphorisch zu sein, Gefühl der Ruhelosigkeit oder Unfähigkeit, still zu sitzen oder stehen, Gedanken, sich das Leben zu nehmen oder sich selbst zu verletzen, unwillkürliche Muskelkontraktionen oder unkontrollierte Muskelbewegungen, erhöhte Muskelspannung, Muskelzuckungen (Myoklonie), Muskelschwäche und -schmerzen, Krampfanfälle, Teilnahmslosigkeit, Wahrnehmungsstörungen.
Es kann ein sogenanntes malignes neuroleptisches Syndrom verursacht werden, welches sich vor allem durch Fieber, unregelmässigen Puls, Bewusstseinsstörungen und Muskelsteifheit äussert. Eine weitere Nebenwirkung (sogenanntes Serotoninsyndrom), welche besonders bei Kombination mit gewissen anderen zentralwirksamen Arzneimitteln auftreten kann, äussert sich durch Bewusstseinstrübung, Muskelstarre, Muskelzittern, Zuckungen und Fieber. In beiden Fällen sollten Sie das Arzneimittel nicht weiter einnehmen und sofort Ihren Arzt oder Ihre Ärztin informieren.
Magen-Darm-System: Übelkeit (sehr häufig), verminderter Appetit, Erbrechen, Verstopfung, Durchfall, nächtliches Zähneknirschen, starke Bauch- oder Rückenschmerzen (was auf ernste Darm-, Leber- oder Bauchspeicheldrüsenbeschwerden hinweisen könnte).
Herz-Kreislauf: Erhöhter Blutdruck, Herzklopfen, Herzrasen, Blutdruckabfall bei Lagewechsel, Hitzewallungen, niedriger Blutdruck, kurzdauernder Bewusstseinsverlust (Synkope), Herzrhythmusstörungen.
Stoffwechsel: Gewichtsveränderungen, erhöhte Cholesterin- und Prolaktinwerte, Leberfunktionsstörungen, Gelbsucht, Bauchspeicheldrüsenentzündung, Leberentzündung. Es sind Störungen des Flüssigkeits- und Salzhaushaltes z.B. mit Verwirrtheit, Krampfanfällen, Ödemen (Schwellungen infolge Ansammlung wässriger Flüssigkeit in den Gewebespalten, z.B. der Haut und Schleimhäute) beobachtet worden.
Sinnesorgane: Sehstörungen inkl. Störung der Sehschärfeanpassung (Akkommodationsstörung), Pupillenerweiterung (Mydriasis), Engwinkelglaukom, Störungen der Geschmacksempfindung, Ohrgeräusche.
Andere: verlängerte Blutungsdauer, allgemeine starke Blutungen, Haut- und Schleimhautblutungen, Blutungen im Magen-Darmtrakt, schwarzer, Teer-artiger Stuhl, Störung der Blutbildung mit erhöhtem Risiko für Infektionen, Schwächegefühl, Überempfindlichkeitsreaktionen, Gähnen, Schwitzen (einschliesslich Nachtschweiss), Hautausschläge, Nesselsucht, Jucken, Haarausfall, Blutbildveränderungen, Ejakulationsstörungen, Orgasmusstörungen, fehlender Orgasmus, Libidoabnahme, Erektionsstörung, Störungen der Regelblutung, Probleme beim Wasserlösen, Harninkontinenz, Schüttelfrost, Überempfindlichkeit gegenüber Licht. Husten, Atemnot, keuchendes Atmen, Kurzatmigkeit und erhöhte Temperatur, ähnlich wie bei einer Lungenentzündung (pulmonale Eosinophilie).
Es wurde ein erhöhtes Risiko von Knochenbrüchen bei Patienten und Patientinnen im Alter über 50 Jahren, welche diese Art von Arzneimittel einnehmen, beobachtet.
Venlafaxin Spirig HC kann Überempfindlichkeitsreaktionen der Haut- und Atmungsorgane auslösen, insbesondere bei Patientinnen oder Patienten mit Asthma, Nesselfieber (chronischer Urtikaria) oder mit Überempfindlichkeit auf Acetylsalicylsäure und andere Rheuma- und Schmerzmittel.
