Tramadol-Mepha ist ein Schmerzmittel, das zur Behandlung von mässigen bis starken Schmerzen eingesetzt wird. Es kann auf Verschreibung des Arztes oder der Ärztin sowohl bei plötzlich einsetzenden Schmerzzuständen (z.B. Wundschmerzen, Knochenbrüchen) als auch bei anhaltenden Schmerzzuständen (z.B. erhebliche Nervenschmerzen, Tumorschmerzen) verwendet werden.
Tramadol-Mepha ist ein stark wirksames Schmerzmittel, das bei längerem Gebrauch zu Gewöhnung und Abhängigkeit führen kann. Befolgen Sie bitte genau die von Ihrem Arzt bzw. von Ihrer Ärztin vorgeschriebene Dosierung und Behandlungsdauer und geben Sie das Arzneimittel nicht an andere Personen weiter.
Sprechen Sie in diesen Fällen vor Beginn der Einnahme mit Ihrem Arzt bzw. mit Ihrer Ärztin.
Patienten, die auf stark wirksame Schmerzmittel vom Typ der sogenannten Opioide empfindlich reagieren (z.B. mit epileptischen Anfällen), sollen Tramadol-Mepha nur mit Vorsicht anwenden.
Es sind epileptische Anfälle bei Patienten oder Patientinnen beschrieben worden, die Tramadol in der empfohlenen Dosis eingenommen haben. Das Risiko kann sich erhöhen, wenn die empfohlene maximale Tagesdosis von 400 mg Tramadol überschritten wird.
Bitte beachten Sie, dass Tramadol-Mepha Kapseln zu einer seelischen und körperlichen Abhängigkeit führen können. Bei längerem Gebrauch kann die Wirkung von Tramadol-Mepha Kapseln nachlassen, sodass höhere Arzneimengen eingenommen werden müssen (Toleranz-Entwicklung). Bei Patienten oder Patientinnen, die zu Missbrauch oder Abhängigkeit von Arzneimitteln neigen, ist daher eine Behandlung mit Tramadol-Mepha Kapseln nur für kurze Dauer und unter strengster ärztlicher Kontrolle durchzuführen.
Bitte teilen Sie Ihrem Arzt bzw. Ihrer Ärztin auch mit, wenn eines dieser Probleme während der Einnahme von Tramadol-Mepha Kapseln auftritt oder wenn diese früher einmal bei Ihnen zutrafen.
Tramadol-Mepha Kapseln dürfen nicht zusammen mit MAO-Hemmstoffen (bestimmten Arzneimitteln zur Behandlung von Depressionen und der Parkinson-Erkrankung) eingenommen werden (siehe Rubrik «Wann dürfen Tramadol-Mepha Kapseln nicht eingenommen werden?»).
Die schmerzlindernde Wirkung von Tramadol-Mepha Kapseln kann vermindert und die Wirkungsdauer verkürzt werden, wenn Sie Arzneimittel einnehmen, die einen der folgenden Wirkstoffe enthalten:
Ihr Arzt bzw. Ihre Ärztin wird Ihnen sagen, ob und gegebenenfalls in welcher Dosierung Sie Tramadol-Mepha Kapseln in diesem Fall einnehmen dürfen.
Das Risiko von Nebenwirkungen erhöht sich
Trinken Sie während der Behandlung mit Tramadol-Mepha Kapseln keinen Alkohol, da dessen Wirkung verstärkt und die Wirkung des Arzneimittels in unvorhersehbarer Weise beeinflusst werden kann.
Nahrungsmittel beeinflussen die Wirkung von Tramadol-Mepha Kapseln nicht.
Wenn Sie die Behandlung mit Tramadol-Mepha unterbrechen oder vorzeitig beenden, führt dies wahrscheinlich zu einem erneuten Auftreten der Schmerzen. Möchten Sie die Behandlung wegen unangenehmer Begleiterscheinungen abbrechen, wenden Sie sich bitte an Ihren Arzt oder an Ihre Ärztin.
Im Allgemeinen wird ein Abbruch der Behandlung mit Tramadol-Mepha keine Nachwirkungen haben.
Bei einigen wenigen Patienten oder Patientinnen, die Tramadol-Mepha über einen sehr langen Zeitraum eingenommen haben, kann es jedoch zu Nachwirkungen kommen. Sie könnten sich unruhig, ängstlich, nervös oder zittrig fühlen. Sie könnten hyperaktiv sein, Schlafstörungen oder Magen-Darm-Beschwerden haben. Sehr wenige Personen könnten Panikanfälle, Halluzinationen, Fehlempfindungen wie Kribbeln, Prickeln und Taubheitsgefühl oder Ohrgeräusche (Tinnitus) bekommen. Weitere ungewöhnliche Symptome des zentralen Nervensystems, wie z.B. Verwirrung, Wahn, veränderte Wahrnehmung der eigenen Person (Depersonalisation) und der Realität (Derealisation) sowie Verfolgungswahn (Paranoia), sind sehr selten beobachtet worden. Wenn eine dieser Nebenwirkungen nach Abbruch der Behandlung mit Tramadol-Mepha Kapseln bei Ihnen auftritt, wenden Sie sich bitte an Ihren Arzt bzw. an Ihre Ärztin.
Tramadol-Mepha Kapseln können unter anderem zu Schwindel, Benommenheit und Sehstörungen (verschwommene Sicht) führen und damit die Reaktionsfähigkeit, die Fähigkeit Werkzeuge oder Maschinen zu bedienen und die Fahrtüchtigkeit beeinträchtigen. Dies gilt insbesondere für die gemeinsame Anwendung mit Substanzen, welche die Psyche beeinflussen.
Informieren Sie Ihren Arzt oder Apotheker, bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden (Externa)!
Über die Unbedenklichkeit von Tramadol in der Schwangerschaft liegen keine ausreichenden Beweise vor. Daher sollten Sie Tramadol-Mepha Kapseln nicht einnehmen, wenn Sie schwanger sind oder es werden möchten, es sei denn auf ausdrückliche Anweisung Ihres Arztes bzw. Ihrer Ärztin.
Die wiederholte Einnahme von Tramadol-Mepha Kapseln in der Schwangerschaft kann zur Gewöhnung des ungeborenen Kindes an Tramadol und infolgedessen nach der Geburt zu Entzugserscheinungen beim Neugeborenen führen.
Tramadol wird in sehr geringen Mengen in die Muttermilch ausgeschieden. Tramadol-Mepha sollte deshalb von stillenden Frauen nicht eingenommen werden. Bei einmaliger Gabe von Tramadol-Mepha ist eine Unterbrechung des Stillens in der Regel nicht erforderlich.
Bitte fragen Sie Ihren Arzt bzw. Ihre Ärztin um Rat.
Nehmen Sie Tramadol-Mepha Kapseln immer genau nach Anweisung des Arztes bzw. der Ärztin ein. Wenn Sie unsicher sind, fragen Sie Ihren Arzt oder Ihre Ärztin, bzw. Ihren Apotheker oder Ihre Apothekerin.
Die Dosierung sollte der Stärke Ihrer Schmerzen und Ihrer individuellen Empfindlichkeit angepasst werden. Grundsätzlich sollte die geringste schmerzlindernd wirkende Dosis gewählt werden.
Nehmen Sie nicht mehr als 8 Kapseln Tramadol-Mepha (entsprechend 400 mg Tramadolhydrochlorid) täglich ein, es sei denn, Ihr Arzt bzw. Ihre Ärztin hat dies ausdrücklich verordnet.
Soweit vom Arzt bzw. von der Ärztin nicht anders verordnet, sollen Tramadol-Mepha Kapseln ? unabhängig von den Mahlzeiten ? wie folgt angewendet werden:
1-2 Kapseln alle 4 bis 6 Stunden mit etwas Flüssigkeit einnehmen.
Die Wirkung hält je nach Schmerzen 4 bis 8 Stunden an.
Tramadol-Mepha Kapseln sind nicht für die Anwendung bei Kindern unter 12 Jahren bestimmt.
Bei älteren Patienten oder Patientinnen (über 75 Jahre) kann es zu einer Verzögerung der Ausscheidung von Tramadol kommen.
Falls dies bei Ihnen zutrifft, kann Ihr Arzt bzw. Ihre Ärztin Ihnen eine Verlängerung der Abstände zwischen den Einnahmen empfehlen.
Wenn Sie eine Leber- und/oder Nierenfunktionsschwäche haben, kann Ihr Arzt bzw. Ihre Ärztin Ihnen eine Verlängerung der Abstände zwischen den Einnahmen empfehlen.
Tramadol-Mepha sollte nicht länger als notwendig eingenommen werden. Wenn eine länger dauernde Schmerzbehandlung erforderlich erscheint, wird Ihr Arzt bzw. Ihre Ärztin gegebenenfalls in kurzen Abständen überprüfen (etwa durch das Einlegen von Anwendungspausen), ob und inwieweit die Einnahme von Tramadol-Mepha Kapseln weiterhin erforderlich ist.
Wenn Sie versehentlich eine zusätzliche Dosis von Tramadol-Mepha Kapseln einnehmen, hat dies im Regelfall keine negativen Auswirkungen. Die nächste Dosis Tramadol-Mepha Kapseln sollten Sie wie verschrieben einnehmen.
Nach Einnahme erheblich zu hoher Dosen kann es zu engen Pupillen, Erbrechen, Blutdruckabfall, beschleunigtem Herzschlag, Kreislaufkollaps, Bewusstseinsstörungen bis hin zum Koma (tiefe Bewusstlosigkeit), epileptiformen Krampfanfällen und Verminderung der Atmung bis hin zum Atemstillstand kommen. Rufen Sie bei Auftreten dieser Zeichen unverzüglich einen Arzt bzw. eine Ärztin zu Hilfe!
Wenn Sie die Einnahme von Tramadol-Mepha Kapseln vergessen haben, können Ihre Schmerzen erneut auftreten. Nehmen Sie nicht die doppelte Dosis ein, um die vergessene Dosis nachzuholen, sondern führen Sie die Einnahme wie vorher fort.
Ändern Sie nicht von sich aus die vorgeschriebene Dosierung oder die Therapiedauer. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker, bzw. Ihrer Ärztin oder Apothekerin.
Sehr häufig: Übelkeit und Schwindel.
Häufig: Benommenheit, Kopfschmerzen, Erbrechen, Verstopfung, Mundtrockenheit und Schwitzen sowie Erschöpfung.
Gelegentlich: Beeinflussung der Kreislaufregulation (Herzklopfen, Herzjagen, Schwächeanfälle, bis hin zum Kreislaufzusammenbruch mit Ohnmacht (Kollaps)). Diese unerwünschten Wirkungen können insbesondere bei aufrechter Körperhaltung und bei Patienten oder Patientinnen auftreten, die körperlich belastet sind. Ferner kann es gelegentlich zu Brechreiz, Magenbeschwerden (z.B. Magendruck, Völlegefühl, Durchfall), und Hauterscheinungen (z.B. Juckreiz, Hautausschlag, rasch auftretende Hautrötung) kommen.
Selten: Verlangsamung der Herzfrequenz (Bradykardie), Blutdruckanstieg, Muskelschwäche, unwillkürliche Muskelzuckungen, Koordinationsstörungen, Appetitveränderungen, Störungen beim Wasserlassen, Fehlempfindungen auf der Haut (wie z.B. Kribbeln, Prickeln, Taubheitsgefühl) Zittern, Sprachstörungen, Pupillenerweiterung, Pupillenverengung und verschwommenes Sehen.
Bei Überschreitung der empfohlenen Dosierung und bei gleichzeitiger Anwendung von anderen Arzneimitteln, die eine dämpfende Wirkung auf bestimmte Gehirnfunktionen haben (z.B. Beruhigungs- und Schlafmittel, Psychopharmaka), kann eine Verminderung der Atmung auftreten. Krampfanfälle traten überwiegend nach Anwendung hoher Tramadol-Dosierungen auf oder nach gleichzeitiger Anwendung von Arzneimitteln, welche selbst krampfauslösend wirken können (z.B. Antidepressiva) oder die Krampfschwelle erniedrigen.
Ebenfalls selten sind: psychische Nebenwirkungen wie Halluzinationen, Verwirrtheit, Delirium, Ängstlichkeit, Schlafstörungen und Albträume.
Psychische Beschwerden können hinsichtlich Stärke und Art individuell (je nach Persönlichkeit und Behandlungsdauer) unterschiedlich sein. Darunter sind Stimmungsveränderungen (meist gehobene Stimmung, gelegentlich gereizte Stimmung), Veränderungen der Aktivität (meist Dämpfung, gelegentlich Steigerung) und Veränderungen des Entscheidungsverhaltens oder Wahrnehmungsstörungen. Eine Abhängigkeit kann sich einstellen. Nach Absetzen der Medikation, können Entzugsreaktionen auftreten, vgl. auch Kapitel «Wann ist bei der Einnahme von Tramadol-Mepha Kapseln Vorsicht geboten».
Über eine Verschlimmerung von Asthma wurde berichtet.
Allergische Reaktionen, z.B. Hautauschläge, Atemnot, pfeifende Atemgeräusche, Haut- und Schleimhautschwellungen (angioneurotisches Ödem) und Schockreaktionen sind in sehr seltenen Fällen aufgetreten. Sie sollten unverzüglich einen Arzt bzw. eine Ärztin konsultieren, wenn Sie Symptome wie Schwellung von Gesicht, Zunge und/oder Rachen und/oder Schwierigkeiten beim Schlucken oder Hautausschlag mit gleichzeitigen Atembeschwerden haben.
Sehr selten wurden Erhöhungen der im Labor bestimmten Leberenzymwerte beobachtet, die auf Leberprobleme hinweisen können.
Unbekannte Häufigkeit: Senkung des Blutzuckerspiegels.
Wenn eine Nebenwirkung sich verschlimmert, bzw. Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Tramadol-Mepha Kapseln sind bei Raumtemperatur (15-25 °C) und ausserhalb der Reichweite von Kindern aufzubewahren.
Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden.
Sollten Sie Packungen besitzen, deren Haltbarkeit abgelaufen ist, so bringen Sie diese bitte in Ihre Apotheke zur Vernichtung zurück.
