Tafinlar enthält den Wirkstoff Dabrafenib und hemmt das Wachstum von Krebszellen. Tafinlar wird zur Behandlung von schwarzem Hautkrebs (Melanom) eingesetzt, der inoperabel ist oder Ableger (Metastasen) gebildet hat und in welchem eine bestimmte genetische Veränderung (BRAF V600E Mutation) nachgewiesen wurde.
Tafinlar ist nur auf Verschreibung des Arztes oder der Ärztin zu verwenden.
Tafinlar darf nicht angewendet werden bei Überempfindlichkeit auf den Wirkstoff Dabrafenib oder einen der Hilfsstoffe.
Wenn Sie Probleme mit den Nieren haben oder gehabt haben.
Besprechen Sie bereits bestehende Leberprobleme mit Ihrem Arzt resp. Ihrer Ärztin. Er resp. sie wird deshalb Ihre Leberwerte vor und während der Behandlung regelmässig kontrollieren.
Informieren Sie Ihren Arzt resp. Ihre Ärztin, wenn Sie an Diabetes (Zuckerkrankheit) leiden. Ihr Arzt/Ihre Ärztin wird in diesem Fall die Blutzuckerwerte engmaschiger überwachen. Das gleiche gilt, wenn Sie an Herzrhythmusstörungen leiden. Ihr Arzt resp. Ihre Ärztin wird Ihre Herzfrequenz engmaschig überwachen.
Männer können während einer Behandlung mit Tafinlar eine reduzierte Spermienzahl haben. Diese kann möglicherweise nach dem Absetzen nicht zu normalen Werten zurückkehren. Für weitere Fragen diesbezüglich kontaktieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin.
Bei gleichzeitiger Anwendung von Tafinlar und anderen Arzneimitteln kann es zu einer Verstärkung oder Abschwächung der Wirkungen (erwünschte und unerwünschte) von Tafinlar oder zu einer Verstärkung der Wirkungen der anderen Arzneimittel kommen. Zu diesen Arzneimitteln gehören zum Beispiel:
Gewisse Antibiotika, Pilzmittel, Arzneimittel gegen HIV (AIDS) oder kortisonartige Arzneimittel, Arzneimittel gegen Epilepsie, Antidepressiva, gewisse blutverdünnende Mittel, hormonelle Schwangerschaftsverhütung (Pille, Spritzen, Pflaster), Arzneimittel zur Senkung von Blutfetten sowie Arzneimittel zur Reduktion der Magensäure. Informieren Sie Ihren Arzt resp. Ihre Ärztin, wenn Sie andere Arzneimittel anwenden. Er/Sie wird entscheiden, welche Sie zusammen mit Tafinlar verwenden dürfen und in welcher Dosierung Sie Tafinlar einnehmen müssen.
Tafinlar ist bisher bei Kindern und Jugendlichen unter 18 Jahren nicht geprüft worden.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie
Die Anwendung von Tafinlar wird während der Schwangerschaft nicht empfohlen. Informieren Sie vor Beginn der Behandlung Ihren Arzt oder Ihre Ärztin, wenn Sie schwanger sind oder es werden möchten, er bzw. sie wird Ihnen möglichweise vor Beginn der Behandlung mit Tafinlar die Durchführung eines Schwangerschaftstests empfehlen. Frauen im gebärfähigen Alter müssen während einer Behandlung mit Tafinlar und 28 Tage danach wirksame empfängnisverhütende Massnahmen treffen. Da die Wirksamkeit hormoneller Verhütungsmethoden (wie Pille, Injektionen oder Pflaster) beeinträchtigt sein kann, sollten zusätzliche Methoden (z.B. Kondome) angewendet werden. Tritt während der Behandlung mit Tafinlar eine Schwangerschaft ein, ist der Arzt bzw. die Ärztin sofort zu informieren.
Tafinlar wird während der Stillzeit nicht empfohlen. Es ist nicht bekannt, ob die Inhaltsstoffe von Tafinlar in die Muttermilch übergehen. Besprechen Sie mit Ihrem Arzt bzw. Ihrer Ärztin das weitere Vorgehen, falls Sie stillen oder stillen möchten.
Die übliche Dosis Tafinlar beträgt 150 mg (zwei 75 mg Kapseln), zweimal täglich. Die Kapseln sollten jeden Tag zur gleichen Tageszeit entweder mindestens eine Stunde vor oder mindestens zwei Stunden nach einer Mahlzeit eingenommen werden. Die Kapseln sollten mit etwas Wasser nacheinander unzerkaut geschluckt werden. In gewissen Fällen kann Ihr Arzt/Ihre Ärztin die Dosis reduzieren (z.B. bei starken Nebenwirkungen).
Falls Sie eine Dosis vergessen haben und mehr als 6 Stunden bis zur nächsten planmässigen Einnahme verbleiben, holen Sie die Einnahme nach. Falls bis zur nächsten planmässigen Dosis weniger als 6 Stunden verbleiben, überspringen Sie die vergessene Dosis und nehmen Sie die nächste Dosis zur gewohnten Zeit ein.
Nehmen Sie nie eine doppelte Dosis gleichzeitig ein.
Falls Sie zu viele Kapseln von Tafinlar eingenommen haben, kontaktieren Sie Ihren Arzt oder Apotheker, bzw. Ihre Ärztin oder Apothekerin.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Die Anwendung und Sicherheit von Tafinlar bei Kindern und Jugendlichen ist bisher nicht geprüft worden.
Achten Sie auf folgende Symptome:
Fieber: Tafinlar kann Fieber verursachen. Informieren Sie Ihren Arzt oder Apotheker, bzw. Ihre Ärztin oder Apothekerin unverzüglich bei anhaltend hoher Körpertemperatur während der Behandlung mit Tafinlar. Er bzw. sie wird anhand von Tests bestimmen, ob für das Fieber andere Ursachen vorliegen und das Problem behandeln. In einigen Fällen können Personen mit Fieber einen tiefen Blutdruck und Schwindel entwickeln. Falls das Fieber schwer ist, kann Ihr Arzt resp. Ihre Ärztin die Behandlung mit Tafinlar möglicherweise vorübergehend stoppen.
Hautveränderungen: Falls Sie Hautveränderungen unter der Behandlung mit Tafinlar feststellen, sollten Sie Ihren Arzt oder Apotheker resp. Ärztin oder Apothekerin so rasch wie möglich informieren. Bei bis zu einer von 10 Personen, die Tafinlar einnehmen, kann sich ein anderer Typ von Hautkrebs entwickeln. Normalerweise bleiben diese Läsionen lokal begrenzt, können chirurgisch entfernt und die Behandlung mit Tafinlar fortgesetzt werden.
Einige Personen, die Tafinlar einnehmen, können womöglich das Auftreten neuer Melanome beobachten. Normalerweise können diese Läsionen chirurgisch entfernt werden und die Behandlung kann fortgesetzt werden.
Ihr Arzt resp. Ihre Ärztin wird Ihre Haut vor der Behandlung mit Tafinlar untersuchen, während der Behandlung alle 2 Monate, sowie alle 2-3 Monate während 6 Monaten nach Absetzen der Behandlung.
Informieren Sie Ihren Arzt bzw. Ihre Ärztin so rasch wie möglich, falls Sie folgende Symptome beobachten:
Augen: Tafinlar kann ein Augenproblem (Uveitis) verursachen, welches unbehandelt das Sehvermögen schädigen kann. Uveitis kann sich rasch entwickeln und beinhaltet folgende Symptome:
Es ist wichtig, dass Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin unverzüglich kontaktieren, falls diese Symptome bei Ihnen auftreten, insbesondere falls Sie ein schmerzendes rotes Auge haben, das sich nicht rasch erholt. Womöglich ist das Beiziehen eines Augenspezialisten notwendig.
Beim Auftreten der folgenden Symptome ist unverzüglich der Arzt bzw. die Ärztin zu informieren: Starke Oberbauchschmerzen, Übelkeit oder Erbrechen, schnelles und tiefes Atmen, Unwohlsein, Appetit- und/ oder Gewichtsverlust. Dies sind möglicherweise frühe Warnzeichen für eine beginnende ? beim Fortschreiten unter Umständen lebensbedrohende ? Nebenwirkung wie Bauchspeicheldrüsenentzündung (Pankreatitis).
Folgende Nebenwirkungen können bei der Einnahme von Tafinlar auftreten:
Verdickung der Haut, Hautprobleme (Hautausschlag, warzenähnliche Wucherungen, die auch bösartig entarten, Rötung, Schwellungen an Handflächen, Finger und Fusssohlen), Kopfschmerzen, Durchfall, Übelkeit, Erbrechen, Appetitlosigkeit, Verstopfung, Schüttelfrost, Fieber, Husten, Schwächegefühl, Antriebslosigkeit, Gelenkschmerzen, Muskelschmerzen, Schmerzen an Händen und Füssen, ungewöhnlicher Haarausfall oder Ausdünnen der Haare.
Grippeähnliche Symptome, Entzündung des Nasenrachenraums, Verminderung des Blutphosphors, Erhöhung des Blutzuckers, Hautprobleme (Trockenheit, Rötung, braune oder gelbliche Verdickungen der Haut, Hautanhängsel, raue, schuppende Hautflecken, Juckreiz), Nierenversagen, verminderte Herzleistung, Herzrhythmusstörung.
Augenentzündung (Uveitis), Entzündung der Bauchspeicheldrüse (die mit starken Bauchschmerzen einhergehen kann; ihr Arzt wird Ihre Bauchspeicheldrüse bei Auftreten einer Entzündung engmaschig kontrollieren), Entzündung des Fettgewebes unter der Haut (kann mit knotenhaften schmerzenden Hautarealen einhergehen), allergische Reaktion, erneutes Melanom.
Entzündung der Niere.
Falls Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden.
Die Hartkapseln sollen nicht über 30 °C und ausser Reichweite von Kindern aufbewahrt werden. Bringen Sie nicht verbrauchte oder beschädigte Kapseln Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin zur fachgerechten Entsorgung zurück.
