Rifampicin Labatec ist ein Medikament, das nur auf Verordnung des Arztes oder Ärztin zur Kombinationstherapie von Tuberkulose und Lepra angewendet wird.
Zur Behandlung von Tuberkulose und Lepra muss Rifampicin Labatec immer kombiniert mit anderen Tuberkulose- oder Lepramittelnangewendet werden, um die Entstehung von resistenten Bakterien zu vermeiden. Zur Behandlung von Lepra ist eine Kombination aus drei Arzneimittel (Multi Drug Therapy (MDT)) notwendig, um das Aufkommen resistenter Stämme zu verhindern. Zu beachten ist, dass die Kalender- Blisterpackungen zur MDT von der WHO gratis erhältlich sind.
Rifampicin Labatec wird auch allein verabreicht zur prophylaktischen Behandlung von gesunden Trägern von Meningitisbakterien (z.B. Personen, die in Kontakt mit Kranken waren), um eine Ansteckung anderer Personen zu vermeiden.
Rifampicin Labatec steht dem Arzt resp. der Ärztin auch als parenteral anwendbare Form zur Verfügung.
Damit die Behandlung wirksam ist, müssen Sie unbedingt die von Ihrem Arzt oder Ihrer Ärztin verschriebene Dosis genau einhalten und dürfen die Einnahme von Rifampicin Labatec nicht unterbrechen.
Dieses Medikament wurde Ihnen von Ihrem Arzt oder Ihrer Ärztin zur Behandlung Ihrer gegenwärtigen Erkrankung verschrieben.
Rifampicin Labatec wirkt nicht gegen alle Mikroorganismen, die Infektionskrankheiten verursachen. Die Anwendung eines falsch gewählten oder nicht richtig dosierten Medikamentes kann Komplikationen verursachen. Sie sollten es daher nie von sich aus zur Behandlung anderer Krankheiten oder anderer Personen verwenden. Desgleichen sollten Sie Rifampicin Labatec bei späteren Reinfektionen nicht ohne erneute ärztliche Konsultation anwenden.
Rifampicin hat eine intensive braunrötliche Farbe, welche manchmal die Haut, die Tränen, den Speichel, den Schweiss und den Stuhl rötlich verfärbt. Zudem können weiche Kontaktlinsen eine bleibende rötliche Farbe annehmen.
Bei einer bekannten Überempfindlichkeit gegen den Wirkstoff, Rifampicin, gegen Antibiotika der gleichen Familie und/oder gegen einen der in der Zusammensetzung enthaltenen Hilfsstoffe. Eine Überempfindlichkeit kann sich zum Beispiel durch Asthma, Atembeschwerden, Kreislaufstörungen, Anschwellungen der Haut (zum Beispiel Nesselsucht) und der Schleimhäute oder Hautausschläge.
Im Fall von schweren Lebererkrankungen, zum Beispiel akuter Gelbsucht.
Wenn Sie bereits einmal eine medikamentös bedingte Leberentzündung durchgemacht haben.
Wenn Sie an einer Nervenentzündung der Gliedmassen leiden.
Wenn die Stoffwechselkrankheit «Porphyrie» vorliegt.
Wenn Sie ein gleichzeitiger Behandlung mit Voriconazol, Proteaseinhibitoren oder Halothan bekommen.
Rifampicin Labatec darf nie zusammen mit der Wirkstoffkombination Saquinavir/Ritonavir eingenommen werden, da es zu schweren Leberzellschädigungen kommen kann.
Bei Frühgeborenen und Neugeborenen darf Rifampicin Labatec nur in extremen Notfällen verwendet, da bei diesen Kindern das Leberenzymstem noch nicht vollständig entwickelt sind.
Infolge individueller Reaktionen kann die Behandlung das Fahrverhalten oder das Bedienen von Maschinen beeinflussen. Dieses Risiko wird im Fall gleichzeitigen Alkoholkonsums noch erhöht.
Bei einem Auftreten von akuten, schweren Überempfindlichkeitsreaktionen ist Rifampicin Labatec sofort abzusetzen. Patientinnen und Patienten, bei welchen solche Erscheinungen auftreten, dürfen nie wieder mit Rifampicin Labatec behandelt werden.
Der in Rifampicin Labatec enthaltene Wirkstoff kann Leberfunktionsstörungen, Leberentzündungen (Hepatitis) und schwere Leberschäden, in Einzelfällen mit tödlichem Verlauf verursachen.
Ihr Arzt oder Ihre Ärztin wird deshalb Ihre Leberfunktion sorgfältig überwachen, insbesondere bei Vorliegen folgender Risikofaktoren für Leberzellschäden:
Frühe Warnzeichen einer Leberentzündung sind Müdigkeit, Schwächegefühl, Unwohlsein, Appetitlosigkeit, Übelkeit oder Erbrechen. Weitere Zeichen sind Gelbfärbung der Augen (Bindehaut) oder der Haut oder entfärbter Stuhl. Suchen Sie bei Auftreten solcher Symptome umgehend Ihren Arzt oder Ihre Ärztin auf. Vor und während der Behandlung sollten die Nierenfunktion, die Leberwerte und das Blutbild überwacht werden.
Patientinnen oder Patienten mit schweren Nierenfunktionsstörungen dürfen Rifampicin Labatec nur einnehmen, wenn sie unter strikter medizinischer Überwachung stehen. Ihr Arzt bzw. Ihre Ärztin wird zu Beginn und danach in regelmässigen Abständen neben der Leberfunktion auch das Blutbild und die Nierenfunktion überprüfen.
Sehr viele Arzneimittel (Ihre Ärztin oder Ihr Arzt bzw. Ihre Apothekerin oder Ihr Apotheker weiss welche) können weniger wirksam sein oder es können vermehrt Nebenwirkungen auftreten, wenn sie gleichzeitig mit Rifampicin Labatec verabreicht werden (z.B. bestimmte Mittel gegen Epilepsie, bestimmte Mittel gegen Zuckerkrankheit, gewisse Herzmittel, Mittel zur Blutverdünnung, Pilzmittel, Kortikosteroide, magensäurehemmende Mittel, Antidepressiva, Neuroleptika, zentral wirksame Schmerzmittel, Methadon, Mittel gegen Schilddrüsenerkrankungen, Mittel zur Behandlung zu hoher Blutfettwerte, Mittel gegen Bluthochdruck, Asthmamittel, Mittel gegen AIDS, Mittel gegen Viren, bestimmte die Immunabwehr herabsetzende Mittel, bestimmte Beruhigungsmittel und Antibiotika und Mittel zur Behandlung von Pilzinfektionen). Dies gilt insbesondere auch für die «Pille»; es sollten deshalb andere nicht-hormonelle Verhütungsmethoden angewandt werden.
Patientinnen und Patienten mit Zuckerkrankheit müssen vorsichtig sein, da die Kontrolle des Diabetes während der Behandlung mit Rifampicin Labatec erschwert ist.
Bei Epileptikern können Anfälle möglicherweise vermehrt auftreten.
Arzneimittel gegen Magenübersäuerung können die Wirkung von Rifampicin Labatec abschwächen. Deshalb sollte Rifampicin Labatec mindestens 1 Stunde vor einem Präparat gegen Magenübersäuerung eingenommen werden.
Die Therapie darf nicht ohne Absprache mit der Ärztin oder dem Arzt unterbrochen werden, um Abwehrreaktionen zu vermeiden (Hautreaktionen, Grippesymptome).
Informieren Sie Ihren Arzt, Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden!
Informieren Sie Ihren Arzt oder Ihrer Ärztin vor der Behandlung mit Rifampicin Labatec, ob Sie schwanger sind, eine Schwangerschaft planen oder stillen. Er oder sie wird entscheiden, ob der therapeutische Nutzen von Rifampicin Labatec ein eventuelles Risiko für das Kind rechtfertigt. Wenn Sie während der Behandlung schwanger werden, sollten Sie dies dem Arzt oder der Ärztin so früh wie möglich mitteilen.
Für die Behandlung der Tuberkulose bei Erwachsene und Jugendliche über 12 Jahre, ist einmal täglich eine Dosis von 450-600 mg Rifampicin Labatec einzunehmen, je nach Gewicht.
Bei anderen Infektionen beträgt die tägliche Dosis 600-1200 mg und wird gewöhnlich auf 2 oder 3 Gaben verteilt, mindestens ½ Stunde vor der Mahlzeit eingenommen.
Erwachsene Träger des Erregers von Hirnhautentzündung erhalten 2 Tage lang 600 mg zweimal pro Tag, mit 12 Stunden Intervall.
Dosierung: Der Arzt oder die Ärztin wird eine dem Körpergewicht des Kindes entsprechende Dosis verschreiben. Die Dauer der Einnahme ist von der Art der Krankheit abhängig.
Bei Lepra wird Rifampicin Labatec in der Regel nach Empfehlung der Weltgesundheitsorganisation wie folgt verordnet (in Kombination mit anderen Arzneimitteln):
Rifampicin Labatec 600 mg 1 Mal pro Monat.
Rifampicin Labatec 450 mg 1 Mal pro Monat.
Der Arzt oder die Ärztin wird entscheiden, welches die optimale Dosierung ist.
Um eine komplette Heilung von der Lepra zu erreichen, müssen Sie Rifampicin Labatec über den gesamten Behandlungszeitraum einnehmen, auch wenn Sie nach einigen Monaten eine Besserung feststellen. Die Behandlung mit Rifampicin Labatec kann bis zu 12 Monate dauern. Wenn Sie mit der Einnahme von Rifampicin Labatec vorzeitig aufhören, können die Symptome wiederkommen.
Um eine optimale Wirkung zu erreichen, muss das Medikament 1/2 Stunde vor den Mahlzeiten eingenommen werden (im Allgemeinen vor dem Frühstück und vor dem Abendessen).
Sie dürfen die Behandlung niemals ohne die ausdrückliche Anweisung des Arztes oder der Ärztin absetzen. Die Symptome der Krankheit verschwinden oft vor der vollständigen Heilung der Infektion. Setzen Sie die Behandlung gemäss den Anweisungen Ihres Arztes oder Ihrer Ärztin fort, auch wenn Sie keine Symptome der Krankheit mehr haben. Wenn die Behandlungsdauer unzureichend ist oder das Medikament frühzeitig abgebrochen wird, kann es zu einem erneuten Ausbruch der Krankheit kommen.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Folgende Nebenwirkungen können bei der Einnahme von Rifampicin Labatec auftreten:
Fieber, anaphylaktische Reaktionen (Überempfindlichkeitsreaktionen mit Kreislaufversagen), gelegentlich Hautausschlag, schwere Hautreaktionen.
Konsultieren Sie sofort Ihren Arzt bzw. Ihre Ärztin beim Auftreten von Schwellungen der Haut (z.B. Nesselfieber) und Schleimhäuten, grossflächigem Hautausschlag, Hautrötungen, Atemproblemen oder Kreislaufbeschwerden.
Gesichtsrötung und Juckreiz mit oder ohne Hautausschlag, Augenrötungen und erhöhte Lichtempfindlichkeit.
Nesselsucht, Hautausschlag, Juckreiz, sehr selten ernsthafte Überempfindlichkeitsreaktionen mit schweren Hautreaktionen, punktförmige Hautblutungen.
Diese können sich durch Mundtrockenheit, Appetitlosigkeit, Übelkeit, Erbrechen, Gasansammlung im Darm, Magenschmerzen, Oberbauchbeschwerden oder Durchfall äussern. Bei Auftreten von schweren, anhaltenden Durchfällen während oder nach der Behandlung sollten Sie unverzüglich Ihre Ärztin oder Ihren Arzt konsultieren.
Arzneimittel, welche die Darmperistaltik (Darmbewegung) hemmen, dürfen nicht eingenommen werden.
Selten Bauchspeicheldrüsenentzündung.
Leberfunktionsstörungen, Leberentzündungen, Gelbsucht und Leberschäden, in Einzelfällen mit tödlichem Verlauf (siehe «Wann ist bei der Einnahme von Rifampicin Labatec Vorsicht geboten?»). Zeichen einer Leberentzündung können Müdigkeit, Schwäche, Appetitlosigkeit, Unwohlsein, Übelkeit und Erbrechen sein. Kontaktieren Sie bei Auftreten dieser Symptome sofort Ihren Arzt oder Ihre Ärztin.
Ein Mangel an Blutplättchen (mit oder ohne punktförmige Hautblutungen), an weissen Blutkörperchen sowie eine Blutarmut können auftreten. Eine Zunahme bestimmter weisser Blutkörperchen wurde ebenfalls beobachtet. Konsultieren Sie sofort Ihren Arzt oder Ihre Ärztin, wenn punktförmige Hautblutungen auftreten.
Häufig Müdigkeit, Schläfrigkeit, Kopfschmerzen, Benommenheit, Schwindel, Kribbelgefühl, Verlust der Sehnenreflexe, selten Koordinationsstörungen der Bewegungen, Verwirrtheit, Muskelschwäche, Krämpfe, epileptische Anfälle, Gedächtnisstörungen und Bewusstseinsveränderungen, Halluzinationen, Orientierungsstörungen, Taubheitsgefühl, Kribbeln, Brennen oder Schwächegefühl an Händen und Füssen.
Bindehautreizungen, Augenrötungen, Sehstörungen.
Grippesymptome.
Kurzatmigkeit, Keuchen.
Niederer Blutdruck, Ödeme, Anaphylaxie (Überempfindlichkeitsreaktionen mit Kreislaufversagen).
Beschwerden beim Wasserlösen, akutes Nierenversagen.
Muskelschwäche, Gelenkschmerzen, rheumatische Beschwerden.
Während einer längeren antituberkulösen Behandlung mit Rifampicin Labatec können gelegentlich Störungen des Menstruationszyklus auftreten.
Rötliche Verfärbung von Körperflüssigkeiten (Urin, Speichel, Tränen, Stuhl, Schweiss).
Wenn Sie eine der oben genannten Nebenwirkungen oder weitere Nebenwirkungen bemerken, die hier nicht beschrieben sind, bei denen Sie einen Zusammenhang mit der Anwendung von Rifampicin Labatec vermuten, sollten Sie unverzüglich Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Sie sollten den Konsum alkoholischer Getränke während der Behandlung vermeiden.
Rifampicin kann manchmal eine orangerote bis braunrote Verfärbung des Urins, des Stuhls, des Schweisses, des Speichels und der Tränen hervorrufen. Träger von weichen Kontaktlinsen sollten die Linsen während der Dauer der Behandlung nicht tragen.
Wie jedes Medikament ist Rifampicin Labatec ausserhalb der Reichweite von Kindern aufzubewahren. Das Medikament ist in der Originalpackung und vor Feuchtigkeit und Wärme geschützt, bei einer Temperatur zwischen 15-30 °C für die Kapseln und zwischen 15-25 °C für die Tablette, aufzubewahren.
Das Arzneimittel darf nur bis zu dem auf der Packung mit «EXP» bezeichneten Datum verwendet werden.
Wenn die Behandlung abgeschlossen ist, sollten Sie die nicht gebrauchten kapseln an die Verkaufsstelle zurückbringen (Arzt oder Apotheker), damit diese fachgerecht entsorgt werden können.
