    +-Was ist Rasilez HCT und wann wird es angewendet?

    Ihr Arzneimittel heisst Rasilez HCT und ist als Filmtablette erhältlich. Jede Filmtablette enthält zwei aktive Wirkstoffe: Aliskiren (150 mg oder 300 mg) und Hydrochlorothiazid (12.5 mg oder 25 mg). Beide Substanzen senken den Blutdruck.

    Aliskiren gehört zu einer neuen Klasse von Arzneimitteln, die Reninhemmer genannt werden und helfen, erhöhten Blutdruck zu senken. Reninhemmer führen dazu, dass der Körper weniger Angiotensin II herstellen kann. Angiotensin II führt dazu, dass sich die Blutgefässe verengen und sich dadurch der Blutdruck erhöht. Durch eine Verminderung der Menge an Angotensin II können sich die Blutgefässe erweitern, was zu einer Senkung des Blutdrucks führt.

    Hydrochlorothiazid gehört zu einer Gruppe von Arzneimitteln, die Thiazid-Diuretika genannt wird. Hydrochlorothiazid erhöht das Harnvolumen, was ebenfalls den Blutdruck senkt.

    Rasilez HCT wird zur Behandlung von hohem Blutdruck verwendet.

    Rasilez HCT darf nur auf Verschreibung des Arztes bzw. der Ärztin eingenommen werden.

    +-Was sollte dazu beachtet werden?

    Befolgen Sie die Anweisungen Ihres Arztes bzw. Ihrer Ärztin genau, auch wenn sie möglicherweise von den allgemeinen Informationen in dieser Packungsbeilage abweichen.

    Möglicherweise testet Ihr Arzt bzw. Ihre Ärztin vor der Behandlung und in regelmässigen Abständen während der Behandlung Ihr Blut, um die Werte für Kalium, Magnesium, Calcium, Natrium, Zucker, Cholesterin, Harnsäure, die Anzahl roter und weisser Blutkörperchen und die Plättchenanzahl zu bestimmen. Möglicherweise überwacht Ihr Arzt bzw. Ihre Ärztin auch Ihre Nierenfunktion.

    +-Wann darf Rasilez HCT nicht angewendet werden?
    • Wenn Sie allergisch (überempfindlich) sind gegen die in Rasilez HCT enthaltenen Wirkstoffe Aliskiren und Hydrochlorothiazid, gegen Sulfonamide oder gegen einen der sonstigen Bestandteile von Rasilez HCT.
    • Wenn Sie durch die Einnahme von Aliskiren ein Angioödem erlitten haben oder an einem hereditären oder idiopathischen Angioödem leiden.
    • Wenn Sie an stark verminderter Harnmenge leiden (Anurie).
    • Wenn Sie schwanger sind, planen schwanger zu werden oder stillen.
    • Falls Sie an Diabetes mellitus (Typ 1 oder Typ 2) oder an eingeschränkter Nierenfunktion leiden und Angiotensin II Rezeptor Blocker (ARB) oder Angiotensin Converting Enzym (ACE)-Hemmer einnehmen.
    • Wenn der Patient unter 2 Jahren alt ist.

    Wenn einer dieser Fälle auf Sie zutrifft, informieren Sie Ihren Arzt bzw. Ihre Ärztin, und nehmen Sie Rasilez HCT nicht ein. Ihr Arzt bzw. Ihre Ärztin wird entscheiden, ob dieses Arzneimittel für Sie geeignet ist.

    +-Wann ist bei der Einnahme von Rasilez HCT Vorsicht geboten?

    Aliskiren kann die Nierenfunktion verschlechtern und die Kaliumwerte in Ihrem Blut erhöhen. Dieser Effekt kann durch die gleichzeitige Einnahme gewisser Medikamente (ACE-Hemmern, Angiotensin II Rezeptor-Blockern, gewisse Schmerzmittel und Entzündungshemmer (NSAID einschliesslich COX2 Hemmer)) verstärkt werden. Besonders betroffen sind Patienten mit bereits eingeschränkter Nierenfunktion, Diabetes, vermindertem Blutvolumen, eingeschränkter Herzfunktion oder Lebererkrankungen. Ihre Ärztin oder Ihr Arzt sollte die Elektrolyt-Werte (vor allem Kalium) in Ihrem Blut sowie Ihre Nierenfunktion und Ihren Blutdruck kontrollieren.

    Weiter ist Vorsicht geboten:

    • Falls Sie einen ACE-Hemmer oder Angiotensin II Rezeptor Blocker (ARB) einnehmen.
    • Falls Sie ein weiteres Diuretikum (ein Arzneimittel, das die von Ihnen produzierte Harnmenge erhöht) wie z.B. Furosemid einnehmen.
    • Falls Sie Ciclosporin (ein Arzneimittel zur Verhinderung der Organabstossung nach Transplantationen oder bei anderen Erkrankungen wie rheumatoider Arthritis oder atopischer Dermatitis) oder Itraconazol (ein Arzneimittel zur Behandlung von Pilzinfektionen) einnehmen.
    • Falls Sie an Fieber, Hautauschlag im Gesicht oder an Gelenkschmerzen leiden, könnte dies ein Hinweis auf Lupus erythematodes, eine Immunerkrankung, sein (oder ein Hinweis darauf, dass Sie früher einmal daran litten).
    • Falls Sie an Diabetes leiden (hoher Blutzucker).
    • Falls Ihnen mitgeteilt wurde, dass Sie hohe Cholesterin- oder Triglyzeridwerte im Blut haben.
    • Falls Ihnen gesagt wurde, dass Sie niedrige Kaliumwerte (mit oder ohne Symptome wie Muskelschwäche, Muskelkrämpfe, abnormer Herzrhythmus) oder Magnesiumwerte im Blut haben.
    • Falls Ihnen gesagt wurde, dass Sie niedrige Natriumwerte im Blut haben (mit oder ohne Symptome wie Müdigkeit, Verwirrtheit, Muskelzucken, Krämpfe).
    • Falls Ihnen mitgeteilt wurde, dass Sie eine erhöhte Menge Calcium im Blut haben (mit oder ohne Symptome wie Übelkeit, Erbrechen, Verstopfung, Magenschmerzen, häufiges Wasserlösen, Durst, Muskelschwäche und Zucken).
    • Falls Ihnen gesagt wurde, dass Sie erhöhte Harnsäurewerte im Blut haben, und/oder an Gicht leiden.
    • Falls Ihre Nierenfunktion schwer eingeschränkt ist, oder Sie unter einer Verengung oder einem Verschluss einer Arterie leiden, welche die Niere mit Blut versorgt.
    • Falls Sie an einer Lebererkrankung leiden.
    • Falls Sie an Allergien oder Asthma leiden.
    • Falls bei Ihnen eine plötzliche Abnahme des Sehvermögens oder Augenschmerzen auftreten, könnten dies Symptome eines erhöhten Druckes in Ihren Augen sein. Diese können innerhalb von Stunden bis Wochen nach Behandlungsbeginn auftreten. In diesem Fall kontaktieren Sie bitte unverzüglich Ihren Arzt.
    • Falls Sie an schwerer Laktose-Unverträglichkeit leiden (Rasilez HCT enthält Laktose (Milchzucker)).

    Wenn einer dieser Fälle auf Sie zutrifft, informieren Sie Ihren Arzt bzw. Ihre Ärztin bevor Sie Rasilez HCT einnehmen. Falls bei Ihnen nach der Einnahme von Rasilez HCT die untenstehenden Symptome auftreten, müssen Sie die Behandlung mit Rasilez HCT abbrechen und unverzüglich Ihren Arzt bzw. Ihre Ärztin benachrichtigen: Reaktion mit Atem- oder Schluckbeschwerden, Brustenge, Nesselfieber, Hautausschlag, Schwellung, Juckreiz, Schwindel, Erbrechen, Bauchschmerzen (Anzeichen einer schweren allergischen Reaktion) oder Reaktionen wie Schwellung im Gesicht, der Arme und Beine, Augen, Lippen, Hals oder Zunge (Anzeichen eines Angioödem). Sie müssen in diesen Fällen dringend überwacht und eventuell entsprechend behandelt werden.

    Falls Sie nach Beginn der Behandlung mit Rasilez HCT an Anzeichen von zu tiefem Blutdruck wie Schwäche oder Benommenheit leiden, fragen Sie Ihren Arzt bzw. Ihre Ärztin um Rat.

    Bei gleichzeitiger Anwendung von Rasilez HCT mit anderen Arzneimitteln kann es zu Wechselwirkungen kommen.

    Dazu gehören:

    • Arzneimittel, um Infektionen zu behandeln wie z.B. Ketoconazol, Itraconazol, Amphotericin oder Penicillin G, oder Arzneimittel gegen Herzrhythmusstörung,
    • ACE-Hemmer oder Angiotensin II Rezeptor Blocker,
    • Andere blutdrucksenkende Arzneimittel (z.B. Amlodipin),
    • Kalium-sparende Arzneimittel, Kaliumersatzpräparate oder kaliumhaltige Salzersatzpräparate,
    • Lithium, Antidepressiva, Antipsychotika, Arzneimittel um einige psychiatrische Erkrankungen zu behandeln,
    • Carbamazepin, ein Arzneimittel, um Epilepsie zu behandeln,
    • Gewisse schmerzstillende oder entzündungshemmende Arzneimittel wie nicht-steroidale Entzündungshemmer (NSAIDs), oder Cox-2 selektive Hemmer und Substanzen, die auf den Salz- und Wasserhaushalt wirken, können zu einer Verschlechterung der Nierenfunktion führen. Die blutdrucksenkende Wirkung von Substanzen, die auf den Salz- und Wasserhaushalt wirken, einschliesslich Aliskiren, kann durch NSAIDs abgeschwächt werden,
    • kortisonähnliche Arzneimittel,
    • Digoxin oder andere Digitalisglykoside (ein Herzmedikament),
    • Verapamil (ein Arzneimittel zur Behandlung von Bluthochdruck und Herzkrankheiten),
    • Curare-Derivate (muskelentspannende Arzneimittel, die bei Operationen eingesetzt werden),
    • Allopurinol (Antigichtmittel),
    • Amantadin (Antiparkinson-Therapie, auch zur Behandlung von Influenza eingesetzt),
    • zytotoxische Arzneimittel (Krebstherapie),
    • Anticholinergika (Arzneimittel, die bei vielen Erkrankungen eingesetzt werden wie z.B. Krämpfen im Magen-Darm-Trakt und der Harnblase, Asthma, Reisekrankheit, Muskelkrämpfe, Parkinson-Erkrankung, und zur Unterstützung in der Anästhesie),
    • Diazoxid (wird eingesetzt, um Bluthochdruck zu behandeln),
    • Ciclosporin (ein Arzneimittel, das nach Transplantationen benutzt wird, um eine Organabstossung zu vermeiden, oder bei anderen Erkrankungen wie rheumatoider Arthritis oder atopischer Dermatitis),
    • Insulin oder orale Antidiabetika (z.B. Metformin),
    • Cholestyramin und Colestipol: dies sind Austauscherharze, die hauptsächlich zur Behandlung von hohen Lipidwerten im Blut eingesetzt werden,
    • Atorvastatin (Cholesterinsenkend)
    • Vitamin D und Calciumsalze,
    • Barbiturate, Narkotika (Schlaf auslösende Arzneimittel) und Alkohol,
    • Drucksteigernde Amine wie Noradrenalin (Arzneimittel zur Steigerung des Blutdrucks).

    In diesen Fällen kann es notwendig sein, die Dosierung zu ändern, spezielle Vorsichtsmassnahmen zu treffen oder in einigen Fällen sogar die Einnahme von einem der Arzneimittel zu stoppen.

    Fahrtüchtigkeit und Bedienen von Maschinen

    Arzneimittel, welche zur Behandlung von Bluthochdruck eingesetzt werden, können Schwindel und Müdigkeit hervorrufen und die Konzentrationsfähigkeit beeinflussen. Bevor Sie also ein Fahrzeug oder eine Maschine benutzen, oder falls Sie eine andere Tätigkeit ausüben, welche Konzentration erfordert, vergewissern Sie sich, wie Sie auf die Wirkung von Rasilez HCT reagieren.

    Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie

    • an anderen Krankheiten leiden,
    • Allergien haben oder
    • andere Arzneimittel (auch selbstgekaufte!) einnehmen, kürzlich eingenommen haben oder äusserlich anwenden.
    +-Darf Rasilez HCT während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

    Schwangerschaft

    Sie dürfen Rasilez HCT nicht einnehmen, falls Sie schwanger sind oder planen schwanger zu werden. Falls Sie denken, Sie seien schwanger, oder falls Sie eine Schwangerschaft planen, ist es wichtig, dass Sie sich sofort an Ihren Arzt bzw. Ihre Ärztin wenden.

    Stillzeit

    Sie dürfen nicht stillen, wenn Sie Rasilez HCT einnehmen, da es Ihrem Kind schaden könnte. Sagen Sie es Ihrem Arzt bzw. Ihrer Ärztin, wenn Sie stillen.

    +-Wie verwenden Sie Rasilez HCT?

    Befolgen Sie die Anweisungen Ihres Arztes bzw. Ihrer Ärztin sorgfältig. Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

    Patienten bzw. Patientinnen mit zu hohem Blutdruck bemerken Ihre Erkrankung häufig nicht. Die meisten fühlen sich ziemlich wohl. Deshalb ist es sehr wichtig, dass Sie dieses Arzneimittel genau so einnehmen, wie es Ihnen Ihr Arzt bzw. Ihre Ärztin verordnet. So erhalten Sie das beste Ergebnis und vermindern das Risiko von Nebenwirkungen. Halten Sie Ihre Kontrolltermine bei Ihrem Arzt bzw. Ihrer Ärztin ein, auch wenn Sie sich wohl fühlen.

    Dosierung

    Ihr Arzt wird Ihnen genau sagen, wie viele Filmtabletten Rasilez HCT Sie einnehmen sollen.

    Die übliche Dosis beträgt täglich eine Filmtablette Rasilez HCT 150/12.5 mg oder Rasilez HCT 300/12.5 mg oder Rasilez HCT 150/25 mg oder Rasilez HCT 300/25 mg.

    Je nachdem, wie Sie auf die Behandlung ansprechen, wird Ihr Arzt bzw. Ihre Ärztin vielleicht eine höhere oder niedrigere Dosis vorschlagen.

    Ältere Patientinnen und Patienten erhalten die gleiche Dosis Rasilez HCT wie jüngere.

    Rasilez HCT ist nur für die Anwendung bei Erwachsenen bestimmt. Es darf bei Patienten unter 2 Jahren nicht angewendet werden und sollte bei Patienten zwischen 2 und 6 Jahren nicht angewendet werden. Rasilez HCT wird für die Anwendung bei Patienten zwischen 6 und 18 Jahren nicht empfohlen.

    Falls Sie an leichter bis mittelschwerer Leber- oder Niereninsuffizienz leiden, muss die Anfangsdosierung nicht angepasst werden. Bei schwerer Leberinsuffizienz dürfen Sie Rasilez HCT nur mit besonderer Vorsicht anwenden. Bei schwerer Niereninsuffizienz wird die Anwendung von Rasilez HCT nicht empfohlen.

    Anwendung

    Nehmen Sie Rasilez HCT jeden Tag zur gleichen Zeit ein. Dies hilft Ihnen, sich an Ihre Arzneimitteleinnahme zu erinnern. Schlucken Sie die Rasilez HCT Filmtabletten ganz mit einer kleinen Menge Wasser. Kauen Sie die Filmtabletten nicht und zerbröseln Sie sie nicht. Sie können Rasilez HCT Filmtabletten mit oder ohne Nahrung einnehmen.

    Dauer der Anwendung

    Befolgen Sie die Anweisungen Ihres Arztes bzw. Ihrer Ärztin genau. Er bzw. sie wird Ihnen sagen, wie lange Sie Rasilez HCT einnehmen sollen.

    Falls Sie die Einnahme von Rasilez HCT vergessen haben

    Falls Sie vergessen haben, eine Dosis einzunehmen, nehmen Sie sie, sobald Sie sich daran erinnern, wenn es bis zur nächsten Einnahme noch mehr als 12 Stunden dauert. Falls bis zur nächsten Dosis weniger als 12 Stunden verbleiben, nehmen Sie einfach die nächste Filmtablette zur gewohnten Zeit. Nehmen Sie nicht die doppelte Dosis, um die vergessene Dosis nachzuholen.

    Falls Sie mehr Rasilez HCT eingenommen haben, als Sie sollten

    Falls Sie versehentlich zu viele Filmtabletten eingenommen haben bzw. falls Sie ein Schwächegefühl und/oder Benommenheit spüren, sprechen Sie sofort mit Ihrem Arzt bzw. Ihrer Ärztin. Es könnte sein, dass Sie medizinische Betreuung benötigen.

    Wenn Sie die Einnahme von Rasilez HCT abbrechen

    Wenn Sie die Behandlung mit Rasilez HCT abbrechen, kann sich Ihre Krankheit verschlimmern. Hören Sie nicht auf, Ihr Arzneimittel einzunehmen, ausser Ihr Arzt bzw. Ihre Ärztin hat dies ausdrücklich angeordnet.

    +-Welche Nebenwirkungen kann Rasilez HCT haben?

    Die unten aufgeführten Häufigkeiten der Nebenwirkungen sind gemäss folgendem Grundsatz definiert:

    Häufig: Diese Nebenwirkungen können 1 bis 10 von 100 Patienten betreffen.

    Gelegentlich: Diese Nebenwirkungen können 1 bis 10 von 1?000 Patienten betreffen.

    Selten: Diese Nebenwirkungen können 1 bis 10 von 10?000 Patienten betreffen.

    Sehr selten: Diese Nebenwirkungen können weniger als 1 von 10?000 Patienten betreffen.

    Wie alle Arzneimittel kann Rasilez HCT Nebenwirkungen haben, die aber nicht bei jedem Patienten bzw. jeder Patientin auftreten müssen.

    Häufig: Durchfall.

    Andere mögliche Nebenwirkungen, die durch Aliskiren oder Hydrochlorothiazid allein bedingt sein können:

    Aliskiren

    Häufig

    • Erhöhte Kaliumwerte im Blut (Hyperkalämie).
    • Schwindel/Benommenheit.

    Gelegentlich:

    • Hautausschlag.
    • schwere Hautveränderungen einschliesslich schmerzhafter Blasenbildung an den Schleimhäuten und schweren Störungen des Allgemeinbefindens (Steven-Johnson-Syndrome) sowie blasige Ablösungen der Epidermis der Haut (toxische epidermale Nekrolyse).
    • niedriger Blutdruck.
    • reduzierte Urinausscheidung (Nierenfunktionsstörung).

    Selten:

    • Atem- oder Schluckbeschwerden, Schwellung im Gesicht, der Arme und Beine, Augen, Lippen, Hals und/oder Zunge.
    • stark reduzierte Urinausscheidung (Nierenversagen).

    Bei diesen Anzeichen sollten Sie die Behandlung abbrechen und umgehend Ihren Arzt oder Ihre Ärztin informieren.

    Andere Nebenwirkungen (Häufigkeit unbekannt):

    Brustenge, Nesselfieber, Schwellungen, Juckreiz, Erbrechen und Bauchschmerzen (Zeichen einer schweren allergischen Reaktion, anaphylaktische Reaktion).

    Übelkeit, Appetitlosigkeit, dunkler Urin, gelbliche Haut und Augen (Anzeichen einer Leberfunktionsstörung).

    Schwellung von Händen, Fussgelenken oder Füssen, Hautrötung.

    Hydrochlorothiazid

    Sehr häufig: Verminderte Kaliumwerte im Blut oder Erhöhung der Blutlipide.

    Häufig: Juckender Ausschlag, verringerter Appetit, Übelkeit und Erbrechen, Schwächegefühl nach dem Aufstehen (was durch Alkohol, Narkosemittel oder Beruhigungsmittel verschlimmert werden kann), Erektionsstörungen, verminderte Natrium- oder Magnesiummenge im Blut, oder erhöhte Harnsäurespiegel im Blut.

    Selten: erhöhte Sonnenempfindlichkeit, Bauchbeschwerden, Verstopfung, Kopfschmerzen, Schlafstörungen, Depression, Kribbeln oder Taubheitsgefühl, Sehstörungen, erhöhte Kalziumwerte im Blut, erhöhter Blutzuckerspiegel, Ausscheidung von Zucker im Urin und Verschlechterung der diabetischen Stoffwechsellage, Gelbe Augen oder Haut (Gelbsucht), unregelmässiger Herzschlag (Arrhythmie), kleine Blutungen in der Haut oder Unterhaut.

    Selten: violett-rote Flecken, Fieber, Juckreiz (Anzeichen, die auf eine Entzündung der Blutgefässe deuten, auch Vaskulitis genannt), rote Haut, Blasenbildung an den Lippen, Augen oder am Mund, Schälen der Haut, Fieber (toxische epidermale Nekrolyse, Erythema multiforme), Gelenkschmerzen, Muskelbeschwerden, Fieber (Lupus erythematodes), schwere Schmerzen im Oberbauch (Bauchspeicheldrüsenentzündung); Husten, pfeifende Atmung, Atemnot (Atemwegserkrankungen einschliesslich Pneumonitis und Lungenödem), Muskelkrämpfe, Schwäche, Störung des Säure-Base Haushalts.

    Abnahme des Sehvermögens oder Schmerzen in den Augen aufgrund eines erhöhten Druckes (mögliche Anzeichen eines akuten Engwinkelglaukoms).

    Veränderungen des Blutbildes können sich wie folgt äussern:

    • Spontane Blutungen und/oder Blutergüsse dicht unter der Hautoberfläche, die ein Zeichen von niedriger Plättchenzahl sind (Thrombozytopenie).
    • Fieber, Halsschmerzen oder Bildung von Geschwüren im Mund aufgrund von Infektionen, häufigere Infekte (Leukopenie, Agranulozytose).
    • Schwäche, Blutergüsse und häufige Infekte (Panzytopenie, aplastische Anämie).
    • Blasse Hautfarbe, Müdigkeit, Atemnot, dunkle Harnfarbe (hämolytische Anämie).

    Informieren Sie umgehend Ihren Arzt bzw. Ihre Ärztin, falls Sie eines dieser Anzeichen spüren.

    Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

    +-Was ist ferner zu beachten?

    Arzneimittel für Kinder unerreichbar aufbewahren.

    Nicht über 30 °C, in der Originalverpackung und vor Feuchtigkeit geschützt lagern.
