Phenytoin-Gerot ist ein Arzneimittel zur Behandlung der Epilepsie. Seine Wirkung beruht wahrscheinlich vorwiegend auf einer Hemmung der Ausbreitung von Krampfpotentialen in der Hirnrinde. Phenytoin-Gerot vermindert Anzahl und Schwere der Anfälle, ohne müde zu machen. Es führt bei vielen Patienten und Patientinnen zur Anfallsfreiheit bzw. zu einer weitgehenden Kontrolle der Anfälle. Ihr Arzt oder Ihre Ärztin verschreibt Ihnen Phenytoin-Gerot zur Behandlung von:
Grossen epileptischen Anfällen (Grand mal), Schlafepilepsie, diffuser Epilepsie, Herdanfällen (fokale Anfälle), besonders Jackson-Anfällen, Schläfenlappenepilepsie (psychomotorische Epilepsie) sowie von kombinierten epileptischen Anfallsformen mit Beteiligung grosser epileptischer Anfälle.
Phenytoin-Gerot darf nur auf Verschreibung des Arztes oder der Ärztin angewendet werden.
Sie sollten Phenytoin-Gerot nicht einnehmen, wenn bei Ihnen Überempfindlichkeit gegen den Wirkstoff Phenytoin bzw. andere Substanzen dieser Wirkstoffklasse (Hydantoine) oder auf einen der Hilfsstoffe (siehe Kapitel «Was ist in Phenytoin-Gerot enthalten?») besteht oder bestimmte Reizleitungsstörungen am Herzen festgestellt wurden. Dies gilt auch, wenn bei Ihnen ein Herzinfarkt weniger als 3 Monate zurückliegt.
Sie sollen Phenytoin-Gerot nur mit Vorsicht anwenden, wenn Sie unter einer Störung der Lungenfunktion bzw. unter bestimmten Herz-Kreislauf-Störungen, wie z.B. sehr niedrigem Blutdruck, Herzmuskelschwäche oder stark verlangsamtem Herzschlag leiden.
Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fähigkeit Werkzeuge oder Maschinen zu bedienen und die Fahrtüchtigkeit beeinträchtigen! Dies trifft insbesondere zu Behandlungsbeginn oder bei höherer Dosierung zu.
Alkoholgenuss muss unterbleiben, da dies zu unerwünschten Wechselwirkungen führen kann. Die Wirkung zahlreicher Arzneimittel kann durch die gleichzeitige Einnahme von Phenytoin-Gerot verstärkt oder abgeschwächt werden. Ebenso beeinflussen eine Reihe von Präparaten die Wirkung von Phenytoin-Gerot. Dies gilt vor allem für Arzneimittel zur Blutverdünnung, Beruhigungsmittel, verschiedene Antibiotika, Antidiabetika, Kortisonpräparate, bestimmte Schmerzmittel und Arzneimittel gegen Magengeschwüre. Besonders zu beachten ist die Wechselwirkung zwischen Phenytoin-Gerot und empfängnisverhütenden Arzneimitteln. Durch eine gleichzeitige Einnahme von Phenytoin-Gerot kann der Effekt der empfängnisverhütenden Mittel beeinträchtigt werden, und es kann dadurch zu einer ungewollten Schwangerschaft kommen. Unzeitgemässe Blutungen können ein Hinweis auf eine solche Wechselwirkung sein. Bitte informieren Sie in diesem Fall Ihren Arzt oder Ihre Ärztin. Falls Sie unter Behandlung mit Arzneimitteln zur Blutverdünnung stehen, wird Ihr Arzt oder Ihre Ärztin regelmässige Kontrolluntersuchungen durchführen. Wie bei anderen Arzneimitteln, die zur Langzeitanwendung vorgesehen sind, wird Ihr Arzt oder Ihre Ärztin u.a. auch regelmässig Blutbild und Leberfunktion kontrollieren.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Ihre Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden.
Ihr Arzt oder Ihre Ärztin wird im Falle einer Schwangerschaft sorgfältig Nutzen und Risiko einer Phenytoin-Gerot-Behandlung gegeneinander abwägen. Keinesfalls dürfen Sie das Präparat ohne ausdrückliche Verschreibung des Arztes oder der Ärztin einnehmen oder ohne Befragen des Arztes oder der Ärztin einfach absetzen. Falls Sie Phenytoin-Gerot während der Schwangerschaft einnehmen müssen, wird Ihr Arzt oder Ihre Ärztin die Dosierung besonders zwischen dem 20. und 40. Schwangerschaftstag so gering wie möglich halten. Eine Kombination mit weiteren Arzneimitteln müssen Sie während dieser Zeit vermeiden. Der Wirkstoff von Phenytoin-Gerot tritt in geringer Menge in die Muttermilch über. Der Arzt bzw. die Ärztin entscheidet, ob ein Abstillen erforderlich ist.
Ihr Arzt oder Ihre Ärztin legt das für Sie geeignete Einnahmeschema je nach Ausprägung des Krankheitsbildes individuell fest. Im allgemeinen beginnt man bei Erwachsenen, die noch nicht mit anderen Arzneimitteln gegen Epilepsie vorbehandelt wurden, mit einer Dosis von 3-mal täglich 1 Tablette Phenytoin-Gerot; diese Tagesdosis kann in wöchentlichen Abständen um 100 mg gesteigert werden. Meist sind Tagesdosen von 3?4 Tabletten ausreichend. Eine Gesamtdosis von 600 mg pro Tag sollte nicht überschritten werden.
Phenytion-Gerot ist ungeeignet für Kinder unter 6 Jahren. Bei Kindern über 6 Jahren wird empfohlen, die Therapie mit einer täglichen Dosis von 2?5 mg/kg KG in 2 oder 3 gleichmässig verteilten Dosen zu beginnen. Die Tagesdosis kann alle drei Tage um 1 mg/kg KG erhöht werden. Eine schnelle Aufsättigung kann mit 5?8 mg/kg KG erfolgen. Erhaltungsdosen liegen um 4?8 mg/kg KG pro Tag. Die maximale Dosis beträgt 300 mg täglich.
Teilen Sie Ihrem Arzt oder Ihrer Ärztin mit, wenn Sie bereits mit anderen Arzneimitteln gegen Epilepsie behandelt werden. Diese Präparate dürfen nicht plötzlich, sondern nur stufenweise abgesetzt und langsam durch Phenytoin-Gerot ersetzt werden.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin.
Folgende Nebenwirkungen können bei der Einnahme oder Anwendung von Phenytoin-Gerot auftreten: Häufig: Doppeltsehen, Augenzittern, Störung der Bewegungskoordination, zunehmende Erregbarkeit, Zittern, Sprachstörungen, Konzentrationsschwierigkeiten, Abgeschlagenheit. Bei länger dauernder Überdosierung können auftreten: starrer Blick, Appetit- und Gewichtsverlust, Erbrechen, Ermüdungserscheinungen, Wahrnehmungsstörungen und Bewusstlosigkeit. Im Allgemeinen verschwinden derartige Reaktionen, die vor allem bei Behandlungsbeginn auftreten, nach entsprechender Anpassung der Dosis unter ärztlicher Kontrolle. Selten können ? besonders bei intravenöser Verabreichung ? Herz-Kreislaufstörungen, wie z.B. Blutdruckabfall, Verschlechterung einer bestehenden Herzmuskelschwäche, Störungen der Reizleitung am Herzen sowie Herzrhythmusstörungen auftreten. Die Verschlechterung einer bestehenden Störung der Lungenfunktion wurde ebenfalls in seltenen Fällen gesehen. Als weitere Nebenwirkungen wurden übermässige Zahnfleischentwicklung, allergische Hauterscheinungen bzw. Blutbildveränderungen beobachtet. Bei länger dauernder Anwendung kann es zu Leberfunktionsstörungen oder u.a. zu verstärkter Körperbehaarung bei Frauen (Hirsutismus) kommen. Ausserdem kann eine nichtentzündliche Nervenerkrankung auftreten, die sich z.B. in Sensibilitätsstörungen äussern kann.
Es gibt Berichte von Knochenerkrankungen, wie Ausdünnung des Knochens (Osteopenie und Osteoporose) und Knochenbrüche (Frakturen). Fragen Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin bei Langzeitbehandlung mit Antiepileptika, einer Vorgeschichte mit Osteoporose oder wenn Sie Steroide einnehmen, um Rat.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Bewahren Sie Phenytoin-Gerot am besten in der Originalverpackung bei Raumtemperatur (15?25 °C) an einem für Kinder unerreichbaren Ort auf.
Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «Exp» bezeichneten Datum verwendet werden.
