Der Wirkstoff in Palexia retard ist das zentral wirksame Schmerzmittel (Analgetikum) Tapentadol. Seinen schmerzlindernden Effekt erzielt es durch seine Wirkung an spezifischen Nervenzellen des Rückenmarks und des Gehirns.

Palexia retard wird zur Behandlung von starken chronischen Schmerzen angewendet.

+-Was sollte dazu beachtet werden?

Palexia retard ist ein stark wirksames Schmerzmittel, das bei längerem Gebrauch zu Gewöhnung und Abhängigkeit führen kann. Befolgen Sie daher bitte genau die von Ihrem Arzt bzw. von Ihrer Ärztin vorgeschriebene Dosierung und Behandlungsdauer und geben Sie das Arzneimittel nicht an andere Personen weiter.

+-Wann darf Palexia retard nicht angewendet werden?

bei einer bekannten Überempfindlichkeit gegenüber dem Wirkstoff Tapentadol oder einem der Hilfsstoffe
wenn Sie an Asthma leiden oder Ihre Atmung bedrohlich langsam oder flach ist
wenn Sie an einer Darmlähmung leiden
wenn Sie eine akute Vergiftung mit Alkohol, Schlaftabletten, Schmerzmitteln oder anderen Medikamenten, die auf die Stimmung und Gefühle wirken (Psychotropika), haben (siehe Kapitel «Wann ist bei der Einnahme von Palexia retard Vorsicht geboten»)
wenn Sie gleichzeitig bestimmte Medikamente zur Behandlung einer Depression (MAO-Hemmer) einnehmen beziehungsweise in den vergangenen 14 Tagen vor Beginn der Behandlung mit Palexia retard eingenommen haben (siehe Kapitel «Wann ist bei der Einnahme von Palexia retard Vorsicht geboten»)
Wenn Sie an Epilepsie leiden und diese durch Behandlung nicht ausreichend kontrolliert werden kann.

+-Wann ist bei der Einnahme von Palexia retard Vorsicht geboten?

wenn Ihre Atmung langsam oder flach ist (Atemdepression)
wenn Sie unter erhöhtem Hirndruck oder Bewusstseinsstörungen bis hin zum Koma leiden
nach einer Kopfverletzung oder einem Hirntumor
wenn Sie an Epilepsie leiden beziehungsweise wenn bei Ihnen ein erhöhtes Risiko besteht, einen epileptischen Anfall zu erleiden
wenn Sie ein Leber- oder Nierenleiden haben (siehe Kapitel «Wie ist Palexia retard einzunehmen?»)
wenn Sie an einer Bauchspeicheldrüsen- oder Gallenerkrankung leiden, einschliesslich Bauchspeicheldrüsenentzündung.

Sprechen Sie in diesen Fällen vor Beginn der Einnahme mit Ihrem Arzt oder Ihrer Ärztin.

Palexia retard kann körperlich und psychisch abhängig machen. Sollten Sie zu Arzneimittelmissbrauch oder Medikamentenabhängigkeit neigen oder medikamentenabhängig sein, sollten Sie die Tabletten nur kurzzeitig und unter strengster ärztlicher Kontrolle einnehmen.

Bei Einnahme von Palexia retard mit anderen Arzneimitteln

Palexia retard soll nicht gleichzeitig mit Arzneimitteln, die zur Behandlung von Depressionen verwendet werden (MAO-Hemmer), eingenommen werden. Informieren Sie Ihren Arzt oder Ihre Ärztin, wenn Sie MAO-Hemmer einnehmen beziehungsweise innerhalb der letzten 14 Tage eingenommen haben.

Ein Serotonin-Syndrom ist ein seltener, lebensbedrohlicher Zustand, der bei einigen Patienten berichtet wurde, die Tapentadol in Kombination mit sogenannten serotoninergen Arzneimitteln (z.B. bestimmte Arzneimittel zur Behandlung von Depressionen) eingenommen haben. Anzeichen eines Serotonin-Syndroms können z.B. Verwirrtheit, Unruhe, Fieber, Schwitzen, unkoordinierte Bewegungen von Gliedmassen oder Augen, unkontrollierbare Muskelzuckungen, kurze ruckartige Zuckungen einzelner Muskeln oder Durchfall sein. Ihr Arzt kann Sie diesbezüglich beraten.

Das Risiko von Nebenwirkungen erhöht sich, wenn Sie Palexia retard gleichzeitig mit einem der folgenden Arzneimittel einnehmen:

andere Schmerzmittel wie z.B. Morphin und Kodein (auch in Hustenmitteln enthalten)
Anästhetika, Phenotiazine, Beruhigungsmittel, Schlaftabletten oder andere Arzneimittel, die eine dämpfende Wirkung auf das Zentralnervensystem haben (einschliesslich Alkohol).

Ihre Atmung kann bedrohlich langsam oder flach werden (Atemdepression) und der Blutdruck fällt möglicherweise ab. Es kann sein, dass Ihr Bewusstsein vermindert ist, Sie sich schläfrig fühlen oder Sie sich einer Ohnmacht nahe fühlen. Falls dies der Fall ist, wenden Sie sich an Ihren Arzt oder Ihre Ärztin.

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte) einnehmen oder äusserlich anwenden!

Bei Einnahme von Palexia retard zusammen mit Nahrungsmitteln und Getränken

Trinken Sie während der Behandlung mit diesen Tabletten keinen Alkohol, da einige Nebenwirkungen, wie z.B. Schläfrigkeit, verstärkt auftreten können. Nahrungsmittel haben keinen Einfluss auf die Wirkung dieser Tabletten.

Palexia retard kann die Reaktionsfähigkeit, die Fahrtüchtigkeit und die Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigen, da es Schläfrigkeit, Schwindel und verschwommenes Sehen verursachen kann. Dies kann insbesondere dann der Fall sein, wenn Sie gerade mit der Einnahme von Palexia retard begonnen haben, wenn Ihr Arzt oder Ihre Ärztin die Dosierung ändert, Sie Alkohol trinken oder Beruhigungsmittel einnehmen. Fragen Sie bitte Ihren Arzt oder Ihre Ärztin, ob Sie Auto fahren beziehungsweise Maschinen bedienen dürfen.

+-Darf Palexia retard während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Fragen Sie vor der Einnahme von allen Arzneimitteln Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin um Rat.

Sie dürfen diese Tabletten nicht einnehmen

wenn Sie schwanger sind, ausser auf Anweisung des Arztes
während der Geburt, da es bei dem Neugeborenen zu einer bedrohlich verlangsamten oder flachen Atmung kommen kann (Atemdepression)
während der Stillzeit, da der Wirkstoff in die Muttermilch übergehen kann.

+-Wie verwenden Sie Palexia retard?

Nehmen Sie Palexia retard immer genau nach Anweisung des Arztes oder der Ärztin ein. Bitte fragen Sie bei Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin nach, wenn Sie sich nicht ganz sicher sind.

Ihr Arzt bzw. ihre Ärztin wird die Dosierung der Schmerzintensität und Ihrem persönlichen Schmerzempfinden entsprechend festlegen. Im Allgemeinen sollte die niedrigste schmerzlindernde Dosis eingenommen werden.

Erwachsene

Im Allgemeinen wird 1 Tablette im Abstand von 12 Stunden eingenommen.

Ihr Arzt oder Ihre Ärztin wird Ihnen gegebenenfalls eine geeignetere Dosis verschreiben und/oder andere Einnahmeintervalle vorschlagen. Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

Kinder

Palexia retard ist für Kinder und Jugendliche unter 18 Jahren nicht geeignet.

Ältere Patienten und Patientinnen

Bei älteren Patienten und Patientinnen (über 65 Jahre) ist üblicherweise keine Dosisanpassung erforderlich. Möglicherweise wird bei einigen Personen dieser Altersgruppe der Wirkstoff Tapentadol verzögert ausgeschieden. Sollte dies bei Ihnen zutreffen, kann Ihr Arzt oder Ihre Ärztin Ihnen gegebenenfalls ein anderes Einnahmeschema empfehlen.

Leber- und Nierenerkrankung (Insuffizienz)

Patienten und Patientinnen mit schwerer Leberstörung sollten diese Tabletten nicht einnehmen. Sollten Sie mittelmässige Beschwerden haben, kann Ihr Arzt bzw. Ihre Ärztin Ihnen ein anderes Einnahmeschema empfehlen.

Patienten und Patientinnen mit schweren Nierenstörungen sollten diese Tabletten nicht einnehmen.

Art der Anwendung

Palexia retard ist oral einzunehmen.

Die Tabletten müssen stets unzerkaut und ungeteilt mit ausreichend Flüssigkeit (vorzugsweise mit einem Glas Trinkwasser) eingenommen werden. Die Tabletten können unabhängig von den Mahlzeiten eingenommen werden.

Dauer der Anwendung

Sie sollten die Tabletten nicht länger einnehmen als von Ihrem Arzt oder Ihre Ärztin verschrieben.

Wenn Sie eine grössere Menge von Palexia retard eingenommen haben, als Sie sollten

Nach Einnahme von sehr hohen Dosen kann Folgendes auftreten:

Pupillenverengung, Erbrechen, Blutdruckabfall, erhöhter Herzschlag, Kreislauf-Kollaps, Bewusstseinsstörungen bis hin zum Koma (tiefe Bewusstlosigkeit), epileptische Krampfanfälle, bedrohlich verlangsamte oder flache Atmung oder Atemstillstand.

Bei Auftreten dieser Krankheitszeichen sollte unverzüglich ein Arzt oder eine Ärztin gerufen werden!

Wenn Sie die Einnahme von Palexia retard vergessen haben

Nehmen Sie nicht die doppelte Dosis ein, wenn Sie die vorherige Einnahme vergessen haben, sondern führen Sie die Einnahme wie vorher fort. Wenn Sie die Einnahme der Tabletten vergessen haben, können Ihre Schmerzen erneut auftreten.

Wenn Sie die Einnahme von Palexia retard abbrechen

Wenn Sie die Behandlung vorzeitig beenden, kann dies zu einem erneuten Auftreten der Schmerzen führen. Möchten Sie die Behandlung wegen unangenehmer Begleiterscheinungen abbrechen, wenden Sie sich bitte an Ihren Arzt oder Ihre Ärztin.

Im Allgemeinen wird ein Abbruch der Behandlung keine Nachwirkungen haben. Gelegentlich kann es jedoch bei Personen, die die Tabletten einige Zeit eingenommen haben, zu Unwohlsein führen, wenn sie die Einnahme abrupt beenden.

Es können folgende Symptome auftreten:

Unruhe, tränende Augen, laufende Nase, Gähnen, Schwitzen, Frösteln, Muskelschmerzen und Pupillenerweiterung.
Reizbarkeit, Angstzustände, Rückenschmerzen, Gelenkschmerzen, Schwäche, Bauchkrämpfe, Schlafstörungen, Übelkeit, Appetitlosigkeit, Erbrechen, Durchfall, Blutdruckanstieg, erhöhter Herzschlag sowie erhöhte Atemfrequenz.

Wenn eine dieser Nebenwirkungen nach Abbruch der Behandlung bei Ihnen auftritt, wenden Sie sich bitte an Ihren Arzt oder Ihre Ärztin.

+-Welche Nebenwirkungen kann Palexia retard haben?

Wie alle Arzneimittel kann Palexia retard Nebenwirkungen verursachen, die aber nicht bei jedem auftreten müssen. Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind oder wenn eine der aufgeführten Nebenwirkungen Sie erheblich beeinträchtigt, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

Sehr häufig:

Übelkeit, Verstopfung, Schwindel, Schläfrigkeit, Kopfschmerzen.

Häufig:

verminderter Appetit, Angstzustände, depressive Verstimmung, Schlafstörungen, Nervosität, Ruhelosigkeit, Aufmerksamkeitsstörung, Zittern, Muskelzucken, Erröten, Kurzatmigkeit, Erbrechen, Durchfall, Verdauungsstörungen, Juckreiz, verstärktes Schwitzen, Hautausschlag, Schwächegefühl, Müdigkeit, Empfinden von Schwankungen der Körpertemperatur, trockene Schleimhäute, Flüssigkeitsansammlung im Gewebe (Ödem).

Gelegentlich:

allergische Reaktionen, Gewichtsverlust, Desorientierung, Verwirrtheit, Erregbarkeit (Ruhelosigkeit), Wahrnehmungsstörungen, Albträume, euphorische Stimmung, Bewusstseinsstörungen, Beeinträchtigung des Erinnerungsvermögens, mentale Beeinträchtigung, Ohnmacht, Sedierung, Schwierigkeiten bei der Kontrolle der Bewegungsabläufe, Sprachstörungen, Taubheitsgefühl, abnormale Hautempfindungen (z. B. Kribbeln, Prickeln), Sehstörungen, beschleunigter Herzschlag, verlangsamter Herzschlag, Herzklopfen, Blutdruckabfall, Bauchbeschwerden, Nesselsucht, Harnverzögerung, häufiges Wasserlassen, Störungen der sexuellen Funktionen, Medikamentenentzugssyndrom (siehe Kapitel «Wenn Sie die Einnahme von Palexia retard abbrechen»), Störungen des Befindens, Reizbarkeit.

Selten:

Abhängigkeit gegenüber dem Wirkstoff, Haut- und Schleimhautschwellungen (angioneurotisches Ödem) und Schockreaktionen, abnormales Denken, epileptischer Anfall, sich einer Ohnmacht nahe fühlen, abnormale Koordination, bedrohlich langsame und flache Atmung (Atemdepression), gestörte Magenentleerung, sich wie betrunken fühlen, sich entspannt fühlen.

+-Was ist ferner zu beachten?

Dieses Arzneimittel enthält Lactose. Bitte nehmen Sie Palexia retard daher erst nach Rücksprache mit Ihrem Arzt oder Ihrer Ärztin ein, wenn Ihnen bekannt ist, dass Sie unter einer Unverträglichkeit gegenüber bestimmten Zuckern leiden.

Palexia retard nicht über 30 °C lagern und ausserhalb der Reichweite von Kindern aufbewahren.

Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden. Sollten Sie Packungen besitzen, deren Haltbarkeit abgelaufen ist, so bringen Sie diese bitte in Ihre Apotheke zur Vernichtung zurück.
