    +-Was ist Neupro und wann wird es angewendet?

    Neupro enthält den Wirkstoff Rotigotin, welcher zur Arzneimittelgruppe der sogenannten Dopaminagonisten gehört, die im Gehirn Dopaminrezeptoren stimulieren.

    Neupro wird angewendet

    • zur symptomatischen Behandlung des Restless-Legs-Syndroms (RLS), einer Erkrankung, die durch einen unwiderstehlichen Drang, die Beine zu bewegen, gekennzeichnet ist.
    • entweder alleine zur symptomatischen Behandlung der Parkinson-Erkrankung im frühen Stadium oder in Kombination mit Levodopa im fortgeschrittenen Stadium der Erkrankung, wenn starke Schwankungen der Beweglichkeit auftreten.
    +-Wann darf Neupro nicht angewendet werden?

    Wenn Sie überempfindlich (allergisch) gegen den Wirkstoff Rotigotin oder einen der sonstigen Bestandteile von Neupro sind.

    Neupro enthält Natriummetabisulfit (Ph.Eur.) (E 223), eine Substanz, die selten Überempfindlichkeitsreaktionen und Bronchialkrämpfe (Bronchospasmen, d.h. Verkrampfungen der Atemwegsmuskulatur) hervorrufen kann.

    Wenn Sie an einer schweren Lebererkrankung leiden.

    Wenn bei Ihnen eine Kernspintomografie (Verfahren zur Darstellung der inneren Organe und Körpergewebe) oder eine Kardioversion (Behandlung von Herzrhythmusstörungen) durchgeführt werden muss, müssen Sie vor diesen Untersuchungen das Neupro-Pflaster entfernen (sonst kann es zu Verbrennungen aufgrund des im Pflaster enthaltenen Aluminiums kommen). Nach erfolgter Untersuchung können Sie ein neues Pflaster aufkleben.

    +-Wann ist bei der Anwendung von Neupro Vorsicht geboten?
    • Dieses Arzneimittel kann Ihren Blutdruck beeinflussen. Daher sollte dieser regelmässig kontrolliert werden, insbesondere zu Beginn der Behandlung.
    • Es wird empfohlen, während der Anwendung von Neupro regelmässig Augenuntersuchungen durchführen zu lassen. Sollten Sie dennoch zwischen zwei Untersuchungen Schwierigkeiten beim Sehen bemerken, sollten Sie sich unverzüglich an Ihren Arzt bzw. Ihre Ärztin wenden.
    • Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fahrtüchtigkeit und die Fähigkeit Werkzeuge oder Maschinen zu bedienen beeinträchtigen.

    Wenn es bei Ihnen zu Schläfrigkeit oder einer plötzlichen Schlafattacke (Episoden plötzlichen Einschlafens) kommt, wenden Sie sich bitte an Ihren Arzt bzw. Ihre Ärztin.

    Diese Schlafattacken können manchmal ohne Vorwarnung oder vorangehende Schläfrigkeit auftreten.

    Falls Sie an plötzlichem Einschlafen oder starker Schläfrigkeit tagsüber leiden, dürfen Sie deshalb während der Anwendung von Neupro kein Fahrzeug führen und keine Aktivitäten ausüben (z.B. Werkzeuge oder Maschinen bedienen), bei denen eine verminderte Aufmerksamkeit Sie selbst oder andere gefährdet. Sie sollen so lange auf das Führen eines Fahrzeuges sowie auf obengenannte Aktivitäten verzichten, bis Sie durch eine ausreichende Erfahrung mit der Anwendung des Arzneimittels das eventuelle Auftreten dieser Beeinträchtigungen hinreichend sicher ausschliessen können.

    • Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie oder Ihre Familie/Ihre Betreuungspersonen bemerken, dass Sie ein für Sie unübliches drang- oder triebhaftes Verhalten entwickeln oder wenn Sie dem Impuls, Trieb oder der Versuchung nicht widerstehen können, bestimmte Dinge zu tun, die Ihnen oder anderen schaden können. Diese werden Impulskontrollstörungen genannt und können mit Verhalten wie sinnlosen Wiederholungshandlungen, Spielsucht, übermässigem Essen oder Geld ausgeben, anormal starkem Sexualtrieb oder einer Zunahme von sexuellen Gedanken und Gefühlen einhergehen.
      Möglicherweise muss Ihr Arzt bzw. Ihre Ärztin, Ihre Behandlung anpassen oder beenden.
    • Neupro kann anomales Denken und Verhalten verursachen. Dieses anomale Denken und Verhalten kann sich in einzelnen oder mehreren verschiedenen Erscheinungsformen wie anomale Gedanken zur Realität, Halluzinationen (Sehen oder Hören von Dingen, die nicht wirklich sind) Verwirrtheit, Orientierungsstörung, aggressivem Verhalten, Ruhelosigkeit und Wahnvorstellung zeigen. Bitte wenden Sie sich an Ihren Arzt bzw. Ihre Ärztin, wenn Sie derartige Wirkungen bemerken.
    • Wenn Sie Neupro zur Behandlung Ihres Restless-Legs-Syndroms anwenden, kann es unter Umständen sein, dass die Symptome früher als gewöhnlich am Abend (oder am Nachmittag), in stärkerer Ausprägung und/oder an anderen Gliedmassen auftreten.
    • Wie jedes Pflaster bzw. jeder Verband kann Neupro Hautreaktionen wie Rötung und Juckreiz verursachen. Diese sind in der Regel leicht bis mittelschwer ausgeprägt und betreffen nur den Hautbereich, auf dem das Pflaster aufgeklebt war. Nach Entfernen des Pflasters klingen die Reaktionen normalerweise innerhalb weniger Stunden ab. Wenn bei Ihnen die Hautreaktion länger als einige Tage anhält, es sich um eine schwere Reaktion handelt oder sie sich über den durch das Pflaster bedeckten Hautbereich ausbreitet, wenden Sie sich bitte an Ihren Arzt bzw. Ihre Ärztin. Vermeiden Sie, die Stellen, die eine durch Neupro bedingte Hautreaktion zeigen, Sonnen- und Solariumlicht auszusetzen. Um Hautreaktionen zu vermeiden, sollten Sie das Pflaster jeden Tag auf eine andere Stelle kleben und dieselbe Hautstelle erst nach 14 Tagen wieder verwenden.
    • Neupro sollte nicht bei Kindern und Jugendlichen angewendet werden.

    Bei Anwendung von Neupro mit anderen Arzneimitteln

    Während Sie Neupro anwenden sollten Sie folgende Arzneimittel nicht einnehmen, da diese möglicherweise die Wirkung von Neupro verringern: Antipsychotika (zur Behandlung bestimmter psychischer Erkrankungen) oder Metoclopramid (zur Behandlung von Übelkeit und Erbrechen).

    Wenn Sie gleichzeitig mit Neupro und Levodopa behandelt werden, können manche Nebenwirkungen schwerer ausgeprägt sein, wie zum Beispiel das Sehen oder Hören von Dingen, die nicht wirklich sind (Halluzinationen), unwillkürliche Bewegungen in Zusammenhang mit der Parkinson-Erkrankung (Dyskinesie) und Anschwellen der Beine und Füsse.

    Bitte fragen Sie Ihren Arzt bzw. Ihre Ärztin, ob Sie während der Anwendung von Neupro Alkohol trinken oder Beruhigungsmittel (z.B. Benzodiazepine, Arzneimittel zur Behandlung von psychischen Erkrankungen und Depressionen) einnehmen können.

    Die möglichen Wechselwirkungen von Rotigotin mit anderen Arzneimitteln sind bisher nur in Einzelfällen untersucht worden.

    Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbst gekaufte) einnehmen oder äusserlich anwenden.

    Bei Anwendung von Neupro zusammen mit Nahrungsmitteln und Getränken

    Da Rotigotin über die Haut in die Blutbahn gelangt, wird es durch Nahrungsmittel oder Getränke in seiner Wirkung nicht beeinflusst. Besprechen Sie mit Ihrem Arzt bzw. Ihrer Ärztin, ob Sie während der Anwendung von Neupro Alkohol trinken können.

    +-Darf Neupro während einer Schwangerschaft oder in der Stillzeit angewendet werden?

    Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie schwanger sind oder eine Schwangerschaft planen.

    Die Wirkungen von Rotigotin auf die Schwangerschaft und das ungeborene Kind sind nicht bekannt. Eine Anwendung von Neupro während der Schwangerschaft darf daher nur dann erfolgen, wenn Ihnen der Arzt oder die Ärztin Neupro ausdrücklich verordnet.

    Eine Anwendung von Neupro während der Stillzeit wird nicht empfohlen, da das Arzneimittel höchstwahrscheinlich die Bildung der Muttermilch unterdrückt. Es liegen keine Daten zum Übergang von Rotigotin in die Muttermilch beim Menschen vor. Falls eine Behandlung mit Neupro erforderlich ist, sollte daher das Stillen beendet werden.

    +-Wie verwenden Sie Neupro?

    Bei Neupro handelt es sich um ein dünnes, transdermales Pflaster. Es ist quadratisch mit abgerundeten Ecken.

    Nach Entfernung der transparenten Schutzfolie wird die Klebeseite des Pflasters auf die Haut aufgeklebt. Der Klebeteil des transdermalen Pflasters enthält den arzneilich wirksamen Bestandteil, der konstant über die Haut in den Blutstrom abgegeben wird.

    Wenden Sie Neupro immer genau nach Anweisung des Arztes bzw. der Ärztin an.

    Neupro wird in der Regel langfristig angewendet. Normalerweise beginnt die Behandlung mit einer niedrigen Dosis, die falls notwendig nach Anweisung Ihres Arztes bzw. Ihrer Ärztin in wöchentlichen Schritten erhöht werden kann, bis die für Ihren Bedarf richtige Dosis erreicht ist. Daraufhin wird die Behandlung mit dieser Dosis, die als Erhaltungsdosis bezeichnet wird, fortgesetzt.

    Wechseln Sie das Neupro-Pflaster einmal täglich. Um die benötigte Dosis zu erreichen, sind verschiedene Neupro-Pflaster erhältlich, die jeweils unterschiedliche Mengen des Wirkstoffs pro Tag (also über 24 Stunden) abgeben: 1 mg/24 h, 2 mg/24 h, 3 mg/24 h, 4 mg/24 h, 6 mg/24 h und 8 mg/24 h. Um eine höhere Dosis zu erreichen, müssen Sie mehrere Pflaster benutzen. So kann beispielsweise die Dosis von 10 mg durch die Anwendung eines 6 mg/24 h Pflasters und eines 4 mg/24 h Pflasters erreicht werden.

    Behandlung des Restless-Legs-Syndroms

    Zu Beginn der Behandlung wenden Sie täglich ein Neupro 1 mg/24 h Pflaster an. Falls notwendig, kann diese Tagesdosis in wöchentlichen Schritten von 1 mg erhöht werden, bis die für Ihren Bedarf richtige (Erhaltungs-) Dosis erreicht ist.

    Die Höchstdosis beträgt 3 mg pro Tag.

    Behandlung der Parkinson-Erkrankung

    Patienten und Patientinnen, die kein Levodopa einnehmen (Frühstadium der Parkinson-Erkrankung):

    Zu Beginn der Behandlung wenden Sie täglich ein Neupro 2 mg/24 h Pflaster an. Ab der zweiten Woche wird die Tagesdosis in wöchentlichen Schritten von 2 mg erhöht, bis die für Ihren Bedarf richtige (Erhaltungs-) Dosis erreicht ist.

    Bei den meisten Patienten und Patientinnen liegt die richtige Dosis zwischen 6 mg und 8 mg pro Tag (erreicht innerhalb von 3 bis 4 Wochen).

    Die Höchstdosis beträgt 8 mg pro Tag.

    Patienten und Patientinnen, die Levodopa einnehmen (fortgeschrittenes Stadium der Parkinson-Erkrankung):

    Zu Beginn der Behandlung wenden Sie täglich ein Neupro 4 mg/24 h Pflaster an. Ab der zweiten Woche wird die Tagesdosis in wöchentlichen Schritten von 2 mg erhöht, bis die für Ihren Bedarf richtige (Erhaltungs-) Dosis erreicht ist. Bei den meisten Patienten und Patientinnen im fortgeschrittenen Stadium mit Schwankungen der Beweglichkeit ist 8 mg täglich die geeignete Dosis. Bei manchen Patienten und Patientinnen kann schon eine Dosis von 4 mg oder 6 mg täglich ausreichend sein.

    Die Höchstdosis beträgt 16 mg pro Tag.

    Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

    Besondere Patientengruppen

    Bei älteren Patienten und Patientinnen sowie bei Patienten und Patientinnen mit leichter bis mittelschwerer Leberfunktionsstörung oder bei Patienten und Patientinnen mit Nierenfunktionsstörung ist keine Dosisanpassung erforderlich. Bei schwerwiegender Leberfunktionsstörung darf Neupro nicht angewendet werden.

    Es ist nicht notwendig, die Dosierung aufgrund Ihres Geschlechts, Gewichts oder Alters anzupassen.

    Die Anwendung und Sicherheit von Neupro bei Kindern und Jugendlichen wurde nicht untersucht, daher sollte Neupro bei diesen nicht angewendet werden.

    Beachten Sie bitte folgende Anweisungen bei der Anwendung von Neupro:

    Kleben Sie einmal täglich ein neues Neupro-Pflaster auf die Haut. Lassen Sie das Pflaster 24 Stunden lang auf der Haut, bevor Sie es entfernen und ein neues anbringen. Stellen Sie sicher, dass Sie das alte Pflaster entfernt haben, bevor Sie das neue aufkleben, und bringen Sie das neue Pflaster in einem anderen Hautbereich an.

    Tauschen Sie das Pflaster jeden Tag etwa um dieselbe Uhrzeit aus.

    Zerschneiden Sie das Neupro-Pflaster nicht.

    Wo soll das Pflaster angebracht werden?

    Befestigen Sie die Klebeseite des Pflasters auf sauberer, trockener, gesunder Haut in den folgenden, im Bild grau markierten Bereichen:

    • Schulter
    • Oberarm
    • Bauch
    • Oberschenkel
    • Hüfte
    • Flanke (seitlich zwischen Rippen und Hüfte)

    Um Hautreizungen zu vermeiden:

    • Kleben Sie das Pflaster jeden Tag auf einen anderen Hautbereich, zum Beispiel an einem Tag auf die rechte, am folgenden Tag auf die linke Körperseite sowie an einem Tag auf den Oberkörper, dann auf die untere Körperhälfte.
    • Kleben Sie Neupro innerhalb von 14 Tagen nicht zweimal auf denselben Hautbereich.
    • Kleben Sie das Pflaster nicht auf offene oder verletzte Haut oder auf Bereiche, in denen die Haut gerötet oder gereizt ist.

    Wenn Sie dennoch wegen des Pflasters Hautprobleme bekommen, lesen Sie bitte in Abschnitt «Welche Nebenwirkungen kann Neupro haben?» nach, was Sie tun sollen.

    Um zu verhindern, dass sich das Pflaster lockert oder ablöst:

    • Bringen Sie das Pflaster nicht an einer Stelle an, wo enge Kleidung scheuert.
    • Verwenden Sie an der Stelle, an der Sie das Pflaster anbringen wollen, oder in der Nähe eines bereits vorhandenen Pflasters keine Cremes, Öle, Lotionen, Puder oder andere Hautpflegeprodukte.
    • Falls Sie das Pflaster auf einen behaarten Hautbereich kleben, müssen Sie die betreffende Stelle mindestens drei Tage vor dem Anbringen des Pflasters rasieren.

    Falls sich das Pflaster ablöst, bringen Sie für den Rest des Tages ein neues Pflaster an und tauschen Sie dann das Pflaster zur gewohnten Tageszeit aus.

    HINWEIS

    • Baden, Duschen und körperliche Betätigungen sollten die Wirkungsweise von Neupro nicht beeinflussen. Stellen Sie dennoch sicher, dass sich das Pflaster danach nicht abgelöst hat.
    • Vermeiden Sie im Bereich des Pflasters die äussere Anwendung von Wärme (z.B. übermässiges Sonnenlicht, Saunabesuche, heisse Bäder, Wärmepflaster oder Wärmflaschen).
    • Hat das Pflaster Hautreizungen verursacht, schützen Sie den Bereich vor direkter Sonneneinstrahlung, da diese zu Veränderungen der Hautfarbe führen kann.

    Wie soll das Pflaster aufgeklebt werden?

    Jedes Pflaster ist einzeln in einem Beutel verpackt. Kleben Sie Neupro auf die Haut auf, sobald Sie den Beutel geöffnet und die Schutzfolie vom Pflaster entfernt haben.

    Um den Beutel zu öffnen, halten Sie ihn an beiden Seiten fest. Ziehen Sie die Folie auseinander und öffnen Sie den Beutel.

    2.
    Nehmen Sie das Pflaster aus dem Beutel.

    3.
    Die Klebeseite des Pflasters ist von einer transparenten Schutzfolie bedeckt. Halten Sie das Pflaster mit beiden Händen so, dass die Schutzfolie zu Ihnen zeigt.

    4.
    Knicken Sie das Pflaster in der Mitte, so dass sich der S-förmige Schnitt in der Folie öffnet.

    5.
    Ziehen Sie eine Seite der Schutzfolie ab. Berühren Sie die Klebefläche des Pflasters nicht mit den Fingern.

    6.
    Halten Sie das Pflaster an der verbleibenden Hälfte der starren Schutzfolie fest und legen Sie die Klebefläche des Pflasters auf die Haut auf. Drücken Sie die Klebefläche des Pflasters fest an.

    7.
    Klappen Sie die andere Hälfte des Pflasters zurück und entfernen Sie die zweite Hälfte der Schutzfolie.

    8.
    Drücken Sie das Pflaster mit der Handfläche etwa 20 bis 30 Sekunden fest an, um sicher zu stellen, dass das Pflaster Kontakt mit der Haut hat und die Ränder gut haften.

    Waschen Sie sich direkt nach dem Aufkleben des Pflasters die Hände mit Wasser und Seife.

    Wie wird ein gebrauchtes Pflaster entfernt?

    Ziehen Sie das gebrauchte Pflaster langsam und vorsichtig von der Haut ab.

    Waschen Sie den Bereich vorsichtig mit warmem Wasser und milder Seife, um möglicherweise nach dem Abziehen des Pflasters noch auf der Haut verbleibenden Klebstoff zu entfernen. Falls sich die Klebstoffreste nicht abwaschen lassen, können sie auch mit ein wenig Babyöl entfernt werden.

    Verwenden Sie weder Alkohol noch andere Lösungsmittel wie z.B. Nagellackentferner, da diese zu Hautreizungen führen können.

    Wählen Sie einen anderen Hautbereich für die Anbringung eines neuen Pflasters und befolgen Sie die obige Anleitung.

    Wenn Sie eine grössere Menge von Neupro angewendet haben, als Sie sollten

    Wenn Sie Neupro in einer höheren Dosierung anwenden, als der Arzt bzw. die Ärztin Ihnen verordnet hat, können Nebenwirkungen wie Übelkeit, Erbrechen, niedriger Blutdruck, Halluzinationen (Sehen oder Hören von Dingen, die nicht wirklich sind), Verwirrtheit, extreme Schläfrigkeit, unwillkürliche Bewegungen und Krampfanfälle auftreten.

    Haben Sie mehr Pflaster angewendet, als vom Arzt bzw. der Ärztin verordnet, wenden Sie sich unverzüglich an Ihren Arzt bzw. Ihre Ärztin oder ein Krankenhaus und befolgen Sie deren Anweisungen zur Entfernung der Pflaster.

    Wenn Sie das Wechseln von Neupro zur gewohnten Zeit vergessen haben

    Falls Sie vergessen haben, das Pflaster zu Ihrer gewohnten Zeit zu wechseln, holen Sie dies nach, sobald es Ihnen auffällt: Entfernen Sie das alte Pflaster und verwenden Sie ein neues.

    Haben Sie vergessen, ein neues Pflaster aufzukleben, nachdem Sie das alte entfernt haben, verwenden Sie ein neues Pflaster, sobald Ihnen dies auffällt.

    In beiden Fällen sollten Sie am folgenden Tag ein neues Pflaster zur gewohnten Zeit aufkleben. Wenden Sie nicht die doppelte Dosis an, wenn Sie die vorherige Anwendung vergessen haben.

    Falls Sie ein anderes Pflaster angewendet haben, als der Arzt bzw. die Ärztin Ihnen verordnet hat (z.B. Neupro 4 mg/24 h anstatt Neupro 2 mg/24 h), wenden Sie sich unverzüglich an Ihren Arzt bzw. Ihre Ärztin oder ein Krankenhaus und befolgen Sie deren Anweisungen zum Wechsel der Pflaster.

    Brechen Sie die Anwendung von Neupro nicht plötzlich ab, ohne zuvor mit Ihrem Arzt oder Ihrer Ärztin gesprochen zu haben, da es sonst unter Umständen zu sehr schwerwiegenden Symptomen wie Fieber, Muskel-/Gelenksteifigkeit, Bewegungsunfähigkeit, schwankendem Blutdruck, beschleunigtem Herzschlag, Verwirrtheit oder Bewusstseinstrübung (z.B. Koma) kommen kann. Kommt es zu solchen Symptomen, müssen Sie unverzüglich Ihren Arzt bzw. Ihre Ärztin kontaktieren.

    Um derartige Symptome zu vermeiden, sollte Ihre tägliche Neupro-Dosis schrittweise herabgesetzt werden:

    • um 1 mg jeden zweiten Tag, wenn Sie Neupro gegen das Restless-Legs-Syndrom anwenden,
    • oder um 2 mg jeden zweiten Tag, wenn Sie Neupro gegen die Parkinson-Erkrankung anwenden.
    +-Welche Nebenwirkungen kann Neupro haben?

    Zu Beginn der Behandlung können Übelkeit und Erbrechen auftreten. Diese sind in der Regel leicht bis mittelschwer ausgeprägt und dauern nur kurze Zeit an. Sie sollten sich an Ihren Arzt bzw. Ihre Ärztin wenden, wenn diese Nebenwirkungen längere Zeit andauern oder Sie beunruhigen.

    Durch das Pflaster verursachte Hautprobleme

    Das Pflaster kann bei Ihnen Hautreaktionen wie Rötung und Juckreiz hervorrufen. Diese sind in der Regel leicht bis mittelschwer ausgeprägt und betreffen lediglich den Hautbereich, auf dem das Pflaster aufgeklebt war. Normalerweise klingen die Reaktionen wenige Stunden nach Entfernen des Pflasters wieder ab.
