Mirtazapin Spirig HC gehört in die Arzneimittelgruppe der Antidepressiva, mit denen sich einzelne und wiederkehrende Episoden einer unipolaren depressiven Erkrankung behandeln lassen, d.h. dass bei Ihnen bisher keine manischen Episoden aufgetreten bzw. von Ihrem Arzt/Ihrer Ärztin festgestellt worden sind. Depressionen sind Störungen des emotionalen Empfindens. Während einer Depression finden im Gehirn Veränderungen statt. Im Gehirn kommunizieren Nervenzellen untereinander mit Hilfe von chemischen Substanzen. Während einer Depression ist die normale Versorgung mit diesen Substanzen reduziert. Antidepressiva beheben diese Mangelzustände und stellen die normale Hirnfunktion wieder her. Generell kann es drei Wochen dauern bis eine Zustandsverbesserung spürbar wird.
Besprechen Sie mit Ihrem Arzt bzw. Ihrer Ärztin, ob es sich bei Ihrer Erkrankung um eine Depression mit oder ohne manische Episoden handelt.
Mirtazapin Spirig HC darf nur auf Verschreibung des Arztes bzw. der Ärztin eingenommen werden.
Mirtazapin Spirig HC ist ausschliesslich für Ihr gegenwärtiges medizinisches Problem (Depression) verschrieben worden und darf nicht für die Behandlung anderer medizinischer Leiden verwendet werden.
Bei einer Allergie (Überempfindlichkeit) gegenüber Mirtazapin oder einem der Inhaltsstoffe von Mirtazapin Spirig HC sollte dieses Präparat nicht verwendet werden. Sprechen Sie gegebenenfalls vor der Einnahme mit Ihrem Arzt oder Ihrer Ärztin.
Falls Sie Arzneimittel mit Monoaminooxidase-Hemmer (MAO-Hemmer) einnehmen oder vor kurzem eingenommen haben, dürfen Sie Mirtazapin Spirig HC nicht einnehmen.
Mirtazapin Spirig HC ist nicht für die Behandlung von Kindern und Jugendlichen unter 18 Jahren geeignet, da die Wirksamkeit nicht nachgewiesen wurde und Bedenken zur Sicherheit vorliegen. Sie sollten wissen, dass Patienten bzw. Patientinnen unter 18 Jahren ein erhöhtes Risiko für Nebenwirkungen wie lebensmüde Gedanken, Suizidversuch und Feindseligkeit (vor allem aggressives Verhalten, Aufsässigkeit und Wutausbrüche) unter der Behandlung mit Antidepressiva haben. Sie sollten unbedingt Ihren Arzt bzw. Ihre Ärztin konsultieren, wenn eines der oben erwähnten Symptome auftritt oder sich verschlechtert. Langzeitdaten in Bezug auf das Wachstum, die Reifung oder die geistige und soziale Entwicklung von Patienten bzw. Patientinnen unter 18 Jahren fehlen.
Es kommt vereinzelt vor, dass Patienten bzw. Patientinnen eine Tendenz aufweisen, Suizidgedanken zu entwickeln bzw. an Selbstverletzung zu denken, vor allem in der Anfangsphase einer Behandlung (normalerweise während den ersten zwei Wochen, manchmal aber auch länger).
Es ist wahrscheinlicher, dass Sie diese Gedanken haben, wenn:
Falls Sie Gedanken über Suizid oder Selbstverletzungen haben, sollten Sie unverzüglich Ihren Arzt bzw. Ihre Ärztin konsultieren oder direkt ein Krankenhaus aufsuchen.
Eventuell ist es hilfreich, wenn Sie einem Verwandten oder einem guten Freund erzählen, dass Sie niedergeschlagen sind und ihn/sie bitten diese Packungsbeilage zu lesen. Sie können diese Person bitten Ihnen mitzuteilen, falls Ihre Depression schlimmer wird oder falls diese Person besorgt ist über eine Änderung in Ihrem Verhalten.
In bestimmten Situationen kann besondere Vorsicht und ärztliche Überwachung angezeigt sein.
Informieren Sie Ihren Arzt bzw. Ihre Ärztin, falls Sie an einer der folgenden Krankheiten leiden oder gelitten haben:
In seltenen Fällen tritt unter Mirtazapin Spirig HC eine Störung in der Produktion von weissen Blutkörperchen im Knochenmark auf, was den Körper anfälliger für Infektionen macht. Bei unerklärlich hohem Fieber, Entzündungen im Halsbereich, Geschwüren im Mundbereich oder anderen Infektionssymptomen sollten Sie die Mirtazapin Spirig HC Einnahme beenden und Ihren Arzt bzw. Ihre Ärztin sofort informieren. Ihr Arzt oder Ihre Ärztin wird dann Ihr Blut kontrollieren. Diese Symptome treten hauptsächlich 4?6 Wochen nach Behandlungsbeginn auf.
Ältere Patienten bzw. Patientinnen können empfindlicher auf Nebenwirkungen von Antidepressiva reagieren.
Andere Arzneimittel können die Wirkungen von Mirtazapin Spirig HC beeinflussen und umgekehrt. Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte) einnehmen oder äusserlich anwenden!
Nehmen Sie Mirtazapin Spirig HC nicht ein in Kombination mit (siehe auch «Wann darf Mirtazapin Spirig HC nicht angewendet werden?»):

In Kombination mit diesen Arzneimitteln kann Mirtazapin Spirig HC den sedativen, schlafanstossenden Effekt dieser Arzneimittel verstärken;
In Kombination mit diesen Arzneimitteln kann die Konzentration von Mirtazapin Spirig HC im Blut erhöht sein;
In Kombination mit diesen Arzneimitteln kann die Konzentration von Mirtazapin Spirig HC im Blut verringert sein;
Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie solche Arzneimittel einnehmen. Möglicherweise muss die Dosierung ? auch nach Beendigung der Behandlung ? entsprechend angepasst werden.
Mirtazapin Spirig HC kann die Wirkung von Alkohol verstärken. Verzichten Sie deshalb während der Mirtazapin Spirig HC-Therapie auf alkoholische Getränke.
Mirtazapin Spirig HC kann die Reaktionsfähigkeit, die Fahrtüchtigkeit und die Fähigkeit Werkzeuge oder Maschinen zu bedienen, beeinträchtigen. Stellen Sie sicher bzw. klären Sie mit Ihrem Arzt oder Ihrer Ärztin ab, dass diese Fähigkeiten nicht beeinträchtigt sind, bevor Sie Autofahren, Maschinen bedienen oder andere Tätigkeiten ausführen, die diese Fähigkeiten erfordern.
Ein Abbruch der Behandlung darf nicht plötzlich erfolgen und darf nur in Absprache mit Ihrem Arzt bzw. Ihrer Ärztin erfolgen, da es sonst zu Entzugssymptomen kommen kann.
Die abrupte Beendigung einer Langzeit-Therapie mit Mirtazapin Spirig HC kann Übelkeit, Kopfschmerzen, Schwindel, Unruhe und Angstgefühle verursachen. Die Dosis sollte in Absprache mit Ihrem Arzt bzw. Ihrer Ärztin allmählich reduziert werden.
Es gibt limitierte Erfahrungen über die Einnahme von Mirtazapin Spirig HC während der Schwangerschaft, diese weisen auf kein erhöhtes Risiko hin. Bei der Anwendung während der Schwangerschaft ist jedoch Vorsicht geboten. Während der Behandlung sollte bei gebärfähigen Patientinnen eine wirksame Empfängnisverhütung durchgeführt werden.
Falls Sie während der Mirtazapin Spirig HC-Einnahme schwanger werden oder beabsichtigen schwanger zu werden, fragen Sie Ihren Arzt bzw. Ihre Ärztin, ob Sie mit der Behandlung weiterfahren sollen. Falls Sie Mirtazapin Spirig HC bis zur oder kurz vor der Geburt einnehmen, sollte Ihr Kind auf mögliche unerwünschte Wirkungen untersucht werden.
Ist eine Behandlung während der Stillzeit notwendig, soll abgestillt werden.
Nehmen Sie Mirtazapin Spirig HC nur gemäss den Angaben Ihres Arztes oder Apothekers bzw. Ihrer Ärztin oder Apothekerin ein. Fragen Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie unsicher sind wie Sie Mirtazapin Spirig HC einnehmen sollen.
Normalerweise beginnen Sie mit einer Dosierung von 15 mg oder 30 mg pro Tag, die dann schrittweise durch Ihren Arzt bzw. Ihre Ärztin bis zur optimalen Tagesdosis erhöht werden kann (zwischen 15 mg und 45 mg pro Tag). Die Dosis ist normalerweise für Personen jeden Alters gleich. Bei älteren Patienten oder bei Patienten mit Nieren- oder Lebererkrankungen wird der Arzt bzw. die Ärztin eventuell die Dosis anpassen.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Die Filmtabletten sollten jeden Tag zur selben Zeit, vorzugsweise als Einmaldosis vor dem Schlafengehen, eingenommen werden. Mirtazapin Spirig HC kann auch auf ärztliche Anordnung in zwei Dosen eingenommen werden (morgens nach dem Frühstück und abends vor dem Schlafengehen). Die höhere Dosis soll abends vor dem Schlafengehen eingenommen werden. Schlucken Sie die Filmtablette(n) unzerkaut mit etwas Wasser oder einer anderen Flüssigkeit.
Normalerweise beginnt Mirtazapin Spirig HC nach 1?2 Wochen zu wirken und nach 2?4 Wochen fühlen Sie sich eventuell besser.
Der Behandlungseffekt sollte 2?4 Wochen nach Therapiebeginn mit Ihrem Arzt oder Ihrer Ärztin besprochen werden. Sollte dieser ungenügend sein, wird Ihnen der Arzt bzw. die Ärztin eine höhere Dosis verschreiben. In diesem Fall müssen Sie nach weiteren 2?4 Wochen nochmals mit Ihrem Arzt bzw. Ihrer Ärztin sprechen.
Normalerweise ist es notwendig Mirtazapin Spirig HC für 4?6 Monate einzunehmen bis die Symptome der Depression verschwunden sind.

In diesem Fall ist sofort ein Arzt bzw. eine Ärztin beizuziehen. Die wahrscheinlichsten Symptome einer Überdosierung, wenn Mirtazapin Spirig HC allein ohne andere Arzneimittel oder Alkohol eingenommen wurde, sind: Müdigkeit, Desorientiertheit und Schläfrigkeit sowie Erhöhung des Pulses und eine Veränderung des Blutdruckes.
Setzen Sie Mirtazapin Spirig HC nur in Absprache mit Ihrem Arzt oder Ihrer Ärztin ab.
Falls Sie Mirtazapin Spirig HC zu früh absetzen, kann dies zu einem Wiederauftreten der Depression führen. Sprechen Sie mit Ihrem Arzt oder Ihrer Ärztin, sobald Sie sich besser fühlen. Ihr Arzt oder Ihre Ärztin wird dann entscheiden, wann die Behandlung beendet werden kann.
Mirtazapin Spirig HC soll nicht abrupt abgesetzt werden, auch wenn die Depression sich gebessert hat. Eine abrupte Beendigung der Therapie kann zu Übelkeit, Kopfschmerzen, Schwindel, Unruhe und Angstgefühlen führen. Diese Symptome können durch schrittweises Absetzen von Mirtazapin Spirig HC vermieden werden. Ihr Arzt oder Ihre Ärztin wird Ihnen sagen wie Sie die Dosis schrittweise verringern sollen.

Folgende Nebenwirkungen können bei der Einnahme von Mirtazapin Spirig HC auftreten.

Gesteigerter Appetit und Gewichtszunahme, Müdigkeit oder Schläfrigkeit, Kopfschmerzen, trockener Mund.
Teilnahmslosigkeit (Lethargie), Schwindel, Zittern oder Unsicherheit, Übelkeit, Durchfall, Erbrechen, Hautausschlag, Gelenkschmerzen (Arthralgie) oder Muskelschmerzen (Myalgie), Rückenschmerzen, Blutdruckabfall bei Lagewechsel (z.B. bei schnellem Aufstehen), Schwellungen (typischerweise an Knöcheln und Füssen), bedingt durch Wasseransammlung im Gewebe (Ödem), lebhafte Träume, Verwirrtheit, Angstgefühle, Schlaflosigkeit, Erschöpfung.
Gesteigerte euphorische Stimmung (Manie), abnormale Sinnesempfindungen der Haut z.B. Brennen, Stechen, Kitzeln, Kribbeln (Parästhesie), Syndrom der unruhigen Beine «Restless legs Syndrom» (attackenförmig auftretende schmerzhafte Empfindungen und Drang zu Bewegung in den Beinen, vor allem nachts bzw. im Liegen), Ohnmachtsanfall (Synkope), Gefühllosigkeit im Mund, niedriger Blutdruck, Alpträume, Unruhe, Halluzinationen, Bewegungsdrang.
Gelbe Verfärbung der Augen oder Haut, was auf eine gestörte Leberfunktion hindeutet (Gelbsucht), Muskelzucken oder Muskelkontraktionen (Myoklonus), Herzinfarkt, Aggression.
Infektionszeichen wie plötzliches hohes Fieber, Halsentzündung und Mundinfektionen (Agranulozytose). In seltenen Fällen kann Mirtazapin Spirig HC Störungen in der Produktion der Blutzellen (Knochenmarksdepression) verursachen. Einige Patienten werden weniger widerstandsfähig gegen Infektionen, da Mirtazapin Spirig HC einen vorübergehenden Mangel an weissen Blutzellen (Granulozytopenie) verursachen kann. In seltenen Fällen kann Mirtazapin Spirig HC einen Mangel an roten und weissen Blutzellen und der Blutplättchen (aplastische Anämie) verursachen, einen Mangel an Blutplättchen (Thrombozytopenie) oder einen Anstieg der Zahl der weissen Blutkörperchen (Eosinophilie).
Epileptischer Anfall (Konvulsionen).
Eine Kombination von Symptomen wie unerklärliches Fieber, Schwitzen, erhöhter Puls, Durchfall, (unkontrollierbare) Muskelkontraktionen, Schüttelfrost, überaktive Reflexe, Ruhelosigkeit, Stimmungsveränderungen, Bewusstlosigkeit und erhöhte Speichelbildung. In sehr seltenen Fällen können dies Zeichen eines Serotonin-Syndroms sein.
Suizidgedanken oder Selbstverletzungsgedanken.
Sprachstörungen.
Abnormale Sinnesempfindungen im Mund.
Schwellungen im Mund (Mund-Ödeme).
Schwellungen, die das Gewebe des ganzen Körpers betreffen (generalisierte Ödeme).
Lokale Schwellungen (lokale Ödeme).
Herzrhythmusstörungen.
Brustvergrösserung bei Mann und Frau.
Bauchspeicheldrüsenentzündung.
Es sind Störungen des Flüssigkeits- und Salzhaushaltes zum Beispiel mit Verwirrtheit, Halluzinationen, Krampfanfällen und Hirnödem (Schwellung infolge Ansammlung wässriger Flüssigkeit im Hirn) beobachtet worden. Kontaktieren Sie in diesem Fall sofort Ihren Arzt bzw. Ihre Ärztin.
Es können schwere Hautreaktionen (Stevens-Johnson Syndrom, Dermatitis bullosa, Erythema multiforme, toxische epidermale Nekrolyse) auftreten. Diese äussern sich z.B. durch schmerzhafte Blasenbildung an Haut und Schleimhäuten, Fieber und Augenbindehautentzündungen. In diesem Fall muss umgehend der Arzt bzw. die Ärztin kontaktiert werden und Sie sollten die Einnahme des Arzneimittels sofort unterbrechen.
Sollten diese Erscheinungen in schwerer oder störender Art auftreten oder andauern, so ist der Arzt bzw. die Ärztin zu konsultieren.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Untersagen Sie anderen Personen die Einnahme Ihres Arzneimittels und nehmen Sie keine Arzneimittel anderer Personen ein.
Informieren Sie jeden Arzt bzw. Ärztin, der/die Sie behandelt, welche Arzneimittel Sie einnehmen. Tragen Sie stets einen Medikamentenpass mit sich, auf dem die von Ihnen eingenommenen Arzneimittel aufgeführt sind. Das kann z.B. bei einem Unfall von grosser Bedeutung sein.
Geben Sie angebrauchte oder nicht mehr benötigte Arzneimittel Ihrer Apotheke zur Entsorgung zurück.

Die Filmtabletten bei Raumtemperatur (15-25 °C), in der Originalpackung vor Licht und Feuchtigkeit geschützt und für Kinder unerreichbar aufbewahren.
Das Arzneimittel darf nur bis zu dem auf der Packung mit «EXP.» bezeichneten Datum verwendet werden.
Weitere Auskünfte erteilt Ihnen Ihr Arzt oder Apotheker, bzw. Ihre Ärztin oder Apothekerin. Diese Personen verfügen über die ausführliche Fachinformation.
Die ovalen und bikonvexen Mirtazapin Spirig HC-Filmtabletten enthalten 30 mg oder 45 mg des Wirkstoffes Mirtazapin sowie Hilfsstoffe.
Die Filmtabletten zu 30 mg sind rotbraun und auf einer Seite mit dem Buchstaben «A» gekennzeichnet. Auf der anderen Seite sind sie mit den Ziffern «0» und «9» gekennzeichnet und weisen zwischen diesen Ziffern eine Bruchrille auf, womit die Filmtabletten bei Bedarf leicht in zwei Hälften geteilt werden können.
Die Filmtabletten zu 45 mg sind weiss und auf einer Seite mit dem Buchstaben «A», sowie auf der anderen Seite mit den Ziffern «10» gekennzeichnet.
