Metopiron hemmt die Bildung von Hormonen der Nebennierenrinde.
Diagnostisch wird Metopiron zur Abklärung von Störungen im Bereich der Hirnanhangsdrüse eingesetzt.
Therapeutisch wird es bei Krankheitszuständen, die mit einer Überproduktion von Nebennierenrindenhormonen verbunden sind, angewendet.
Metopiron darf nur auf Verschreibung des Arztes bzw. der Ärztin angewendet werden.
Bevor der Test mit Metopiron bei Ihnen durchgeführt wird, wird Ihr Arzt bzw. Ihre Ärztin bei Ihnen durch Gabe einer entsprechenden Hormonmedikation die Ansprechbarkeit der Nebennierenrinde (NNR) auf äusserlich zugeführtes Hormon prüfen. Da Metopiron die Hormonproduktion der Nebenniere senkt, ist so zu ermitteln, ob durch den Metopiron-Test eine vorübergehende Nebennierenunterfunktion bei Ihnen bewirkt werden kann.
Vor der Durchführung des Metopiron-Tests bei Ihnen sind nach Rücksprache mit Ihrem Arzt oder Ihrer Ärztin Medikamente, welche die Hirnanhangsdrüse - oder Nebennierenfunktion beeinflussen, abzusetzen (s. «Wann ist bei der Einnahme von Metopiron Vorsicht geboten?»).
Wenn die Funktion der Nebennierenrinde bzw. der Hirnanhangsdrüse stärker eingeschränkt ist als aufgrund der vorgängig gewonnenen Testresultate zu erwarten ist, kann durch Metopiron eine vorübergehende NNR-Unterfunktion bei Ihnen ausgelöst werden. Ihr Arzt oder Ihre Ärztin wird dieser NNR-Unterfunktion durch entsprechende Hormongaben rasch entgegen wirken können.
Leiden Sie an einer bestimmten seltenen Form von körperlichen Veränderungen, die durch einen hohen Kortisolspiegel im Blut verursacht werden (ektopisches Cushing Syndrom) kommt es zur Unterdrückung Ihres Immunsystems (Immunsuppression). Dies kann zu Infektionen führen, zu deren Abwehr Ihr Immunsystem nicht mehr in der Lage ist (opportunistischen Infektionen), z.B. zu bestimmten Formen von Lungenentzündungen (s. «Wann ist bei der Einnahme von Metopiron Vorsicht geboten?»). Bei Behandlung mit Metopiron wird Ihr Immunsystem beeinflusst, dies kann in der Folge bei Ihnen starke Entzündungen nach Infektionen auslösen.
Metopiron kann eine angeborene Stoffwechselerkrankung auslösen, bei der ein lebenswichtiger Blutfarbstoff nicht in ausreichenden Mengen produziert wird (akute Porphyrie). Zeichen dafür können intensive dauerhafte Bauchschmerzen, Erbrechen, Herzrasen, Muskelschwäche, Lähmungserscheinungen, Angstzustände und Verwirrtheit sein. Informieren Sie Ihren Arzt oder Ihre Ärztin, wenn bei Ihnen solche Symptome auftreten.
Bei bekannter Überempfindlichkeit auf den Wirkstoff und bei Unterfunktion der Nebennierenrinde.
Metopiron kann die Verträglichkeit von Paracetamol (Schmerzmittel) verringern.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie
- an anderen Krankheiten leiden,
- Allergien haben oder
- andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden.
Während der Behandlung mit Metopiron ist das folgende zu beachten:
- Metopiron kann zeitweise die Hormonproduktion der Nebenniere senken. Ihr Arzt bzw. Ihre Ärztin wird dies durch Gabe einer entsprechenden Hormonmedikation korrigieren.
- Wenn Sie kurzatmig werden mit Fieber über mehrere Stunden oder Tage, informieren Sie Ihren Arzt bzw. Ihre Ärztin so schnell wie möglich, da Sie vielleicht eine schwere Lungeninfektion ausbilden könnten.
Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie schwanger sind oder Ihr Kind stillen möchten.
Während der Schwangerschaft soll Metopiron nicht angewendet werden, ausser wenn der Arzt bzw. die Ärztin dies für unbedingt nötig hält.
Es ist nicht bekannt, ob Metopiron in die Muttermilch übertritt. Deshalb sollen Mütter, die unter Behandlung mit Metopiron stehen, ihr Kind nicht stillen.
Diagnostische Anwendung
Die Voraussetzungen zur Durchführbarkeit des Metopiron-Testes sind in «Was sollte dazu beachtet werden?» oben aufgeführt.
Einzeldosis-Kurztest: 1-2 g Metopiron (30 mg/kg, auch bei Kindern) werden um Mitternacht mit Joghurt oder Milch verabreicht. Das zur Hormonbestimmung vorgesehene Blut wird am folgenden Morgen durch Ihren Arzt bzw. Ihre Ärztin entnommen.
Patienten mit Verdacht auf NNR-Unterfunktion, sollten sicherheitshalber über Nacht hospitalisiert werden, obwohl mit dem Einzeldosis-Kurztest bisher keine Fälle von akuter NNR- Unterfunktion bekannt geworden sind.
Mehrdosentest: Dieser wird nur im Spital durchgeführt. Detaillierte Angaben befinden sich in der Fachinformation.
Therapeutische Anwendung
Die Dosierung von Metopiron wird vom Arzt bzw. der Ärztin für jeden Fall einzeln festgesetzt. Zur Normalisierung erhöhter Werte von Nebennierenrindenhormon (Cortisol) schwankt die Tagesdosis zwischen 1 und 24 Kapseln. Bei erhöhtem Aldosteron (ein anderes Nebennierenhormon) werden im Allgemeinen 12 Kapseln Metopiron täglich verordnet, verteilt auf mehrere Einzeldosen sowie weitere Arzneimittel.
Bitte nehmen Sie Metopiron nach dem Essen, um die Möglichkeit von Übelkeit und Erbrechen so gering wie möglich zu halten.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin.
Die Anwendung und Sicherheit von Metopiron, Kapseln bei Kindern und Jugendlichen zu therapeutischen Zwecken ist bisher nicht geprüft worden.
Folgende Nebenwirkungen können häufig bei der Einnahme von Metopiron auftreten:
Schwindel, Müdigkeit, Kopfschmerzen, tiefer Blutdruck, Übelkeit, Erbrechen.
Folgende Nebenwirkungen können selten bei der Einnahme von Metopiron auftreten: reduzierte Funktion der Nebennieren (führt zu hormonellem Ungleichgewicht), verstärkter Haarwuchs, hoher Blutdruck, Bauchschmerzen, allergische Hautreaktionen.
Folgende Nebenwirkungen können sehr selten bei der Einnahme von Metopiron auftreten: Knochenmark-Störungen (führt zu einem Mangel der Blutkörperchen), Haarausfall.
Informieren Sie beim Auftreten solcher Beschwerden bitte Ihren Arzt bzw. Ihre Ärztin.
