+-Was ist Methylphenidat Sandoz und wann wird es angewendet?

Methylphenidat Sandoz Retardtabletten enthalten 18 mg, 27 mg, 36 mg oder 54 mg des Wirkstoffes Methylphenidathydrochlorid. Die Tabletten besitzen eine verzögerte Freisetzung, das heisst, sie geben den Wirkstoff langsam ab.

Die äussere Schicht der Methylphenidat Sandoz Retardtabletten löst sich gleich nach dem Hinunterschlucken am Morgen auf und gibt so eine Erstdosis Methylphenidat ab. Die spezielle, den inneren Kern umhüllende, Membran ermöglicht eine stufenweise Freisetzung des restlichen Wirkstoffes.

Methylphenidat Sandoz enthält als Wirkstoff Methylphenidat, ein zentralnervöses Stimulans, das die Aufmerksamkeit erhöht und die Impulsivität und Hyperaktivität bei Kindern, Jugendlichen und Erwachsenen mit einer so genannten Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung (ADHS) senkt.

Methylphenidat Sandoz ist indiziert zur Behandlung einer seit dem Kindesalter fortbestehenden Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung (ADHS) als Teil eines Behandlungsprogramms, das auch psychologische, pädagogische und soziale Massnahmen umfasst.

Ein Teil der Kinder, bei denen ADHS festgestellt wurde, haben die Symptome auch im Erwachsenenalter. Bei Erwachsenen soll die Symptomatik also bereits in der Kindheit begonnen haben, wenn eine Behandlung eingeleitet wird.

Die Symptome von ADHS bestehen in rasch nachlassender Aufmerksamkeit, Flüchtigkeitsfehlern, nicht zuhören, Aufgaben nicht beenden, Anweisungen nicht befolgen, leichter Ablenkbarkeit, impulsiv, unruhig, zappelig sein, übertrieben viel reden, zu unpassenden Gelegenheiten herumrennen, andere stören und sich auf Dinge nur schwer konzentrieren. Das Lernen ist beeinträchtigt. Das alles kann zu Schwierigkeiten im sozialen Umfeld, in der Schule, bei der Arbeit führen. Es kann sein, dass die Betroffenen nicht alle diese Symptome haben.

Viele Menschen haben diese Symptome von Zeit zu Zeit, aber bei Patienten oder Patientinnen mit ADHS beeinträchtigen diese Symptome deren Alltagstätigkeiten und treten in mehr als einer Umgebung auf (d.h. zuhause, in der Schule, bei der Arbeit). ADHS kann sich bis in das Erwachsenenalter fortsetzen.

Nach einer gründlichen Untersuchung hat sich der behandelnde Arzt bzw. die behandelnde Ärztin für Methylphenidat Sandoz als Teil eines Behandlungsprogramms entschieden.

Methylphenidat Sandoz darf nur auf Verschreibung eines Arztes bzw. einer Ärztin mit Erfahrung in der Behandlung von Verhaltensstörungen bei Kindern und Jugendlichen oder aber Erwachsenen und unter ihrer regelmässigen Kontrolle verwendet werden.

Durch die Behandlung mit Methylphenidat Sandoz können die Hauptsymptome des ADHS wie mässige bis starke Ablenkbarkeit, rasch nachlassende Aufmerksamkeit, Impulsivität, verstärkte motorische Aktivität und gestörtes Sozialverhalten gemildert werden.

Abklärungen vor Behandlungsbeginn

Vor Beginn der Behandlung muss Ihr Arzt/Ihre Ärztin abklären, ob Sie oder Ihr Kind an einer Herz-Kreislauf-Erkrankung leiden, die einer Behandlung mit Methylphenidat Sandoz entgegensteht. Bei Ihrem Kind misst der Arzt/die Ärztin ausserdem vor der Behandlung und im Verlauf der Behandlung das Gewicht und die Grösse.

+-Was sollte dazu beachtet werden?

Die Retardtablette löst sich nach Abgabe der gesamten Wirkstoffmenge nicht vollständig auf, deswegen kann die Tablettenhülle manchmal im Stuhl erscheinen. Das ist kein Grund zur Beunruhigung.

Die Behandlung mit Methylphenidat Sandoz soll von einem mit der Behandlung von Verhaltensstörungen bei Kindern und Jugendlichen resp. Erwachsenen erfahrenen Arzt bzw. Ärztin durchgeführt und überwacht werden. Neben der medikamentösen Behandlung sollte die umfassende Therapie psychologische, erzieherische und soziale Massnahmen einschliessen. Die Diagnose sollte gemäss anerkannten Kriterien gestellt worden sein.

+-Wann darf Methylphenidat Sandoz nicht angewendet werden?

Sie dürfen Methylphenidat Sandoz nicht einnehmen

wenn Sie an ausgeprägten Angst-, Spannungs- und Erregungszuständen leiden, weil die Einnahme des Arzneimittels diese Zustände verschlimmern kann;
wenn Sie an einer vergrösserten Prostata mit Restharnbildung leiden;
wenn bei Ihnen oder einem Familienmitglied das Tourette Syndrom (Tics) diagnostiziert wurde;
wenn Sie ein Glaukom (grüner Star) haben (erhöhter Augeninnendruck);
wenn Sie unter einem Phäochromozytom (seltene Tumorerkrankung der Nebenniere) leiden;
wenn Sie an arteriellen Durchblutungsstörungen leiden;
wenn Sie an Veränderungen der Blutgefässe des Kopfes leiden oder einen Schlaganfall erlitten haben;
wenn Sie gegenwärtig sogenannte Monoaminoxidase-Hemmer (z.B. Arzneimittel gegen Depressionen mit dem Wirkstoff Moclobemid) einnehmen oder innerhalb der letzten 14 Tage vor der Behandlung mit Methylphenidat Sandoz eingenommen haben;
wenn Sie an einer Erkrankung der Schilddrüse (Hyperthyreose) leiden
wenn Sie an Herzrhythmusstörungen leiden;
wenn Sie an Erkrankungen des Herzens (schwerer Bluthochdruck, Herzschwäche, Angina Pectoris, angeborene Herzfehler, Herzmuskelerkrankungen, Herzinfarkt oder Erkrankungen, die lebensbedrohliche Herzrhythmusstörungen auslösen können) leiden;
wenn Sie übermässig Alkohol trinken, übermässig Arzneimittel oder Drogen nehmen oder abhängig sind oder wenn Sie früher übermässig Alkohol getrunken, übermässig Arzneimittel zugeführt oder Drogen genommen haben oder abhängig waren;
wenn Sie unter einer schweren Depression mit depressiven Symptomen wie Traurigkeit, Minderwertigkeitsgefühlen und Hoffnungslosigkeit leiden; abnorme Gedanken oder Visionen haben, abnorme Laute hören oder wenn bei Ihnen eine Psychose diagnostiziert wurde; oder wenn Sie in der Vorgeschichte Suizidneigungen gezeigt haben;
wenn Sie gegen Methylphenidat oder einen der lnhaltsstoffe des Arzneimittels allergisch sind.
Kinder unter 6 Jahren dürfen Methylphenidat Sandoz nicht einnehmen.
Methylphenidat Sandoz darf nicht eingenommen werden, um eine normale Müdigkeit zu beseitigen.

+-Wann ist bei der Einnahme von Methylphenidat Sandoz Vorsicht geboten?

Teilen Sie Ihrem Arzt bzw. Ihrer Ärztin mit, wenn Sie bzw. Ihr Kind

früher übermässig Alkohol getrunken, übermässig Arzneimittel zugeführt oder Drogen genommen haben oder abhängig waren oder wenn Sie bzw. Ihr Kind gegenwärtig übermässig Alkohol, Arzneimittel oder Drogen nehmen oder abhängig sind;
gegenwärtig gegen Depressionen behandelt werden oder unter depressiven Symptomen wie Traurigkeit, Minderwertigkeitsgefühlen und Hoffnungslosigkeit leiden;
neues oder verstärktes aggressives Verhalten zeigen;
Bewegungstics (schwer zu kontrollierende, wiederholte Muskelzuckungen beliebiger Körperteile) oder verbale Tics (schwer zu kontrollierende, wiederholte Äusserungen von Lauten oder Wörtern) haben;
abnorme Gedanken oder Visionen haben, abnorme Laute hören oder wenn bei Ihnen bzw. Ihrem Kind eine Psychose diagnostiziert wurde;
Krampfanfälle (Epilepsie) oder anomale EEG-Befunde (Elektroenzephalogramme = Hirnscans) haben;
Ungewöhnliche lang anhaltende (mehr als 2 Stunden) oder häufige schmerzhafte Erektionen haben, eventuell auch nach einer Pause oder dem Ende einer Behandlung mit Methylphenidat Sandoz. Diese erfordern eine dringliche ärztliche Behandlung;
hohen Blutdruck haben. Der Blutdruck sollte kontrolliert werden, insbesondere bei Patienten bzw. Patientinnen mit hohem Blutdruck;
eine Verengung oder Blockierung Ihres Verdauungskanals (Speiseröhre, Magen, Dünn- oder Dickdarm) haben;
Schwierigkeiten mit dem Schlucken von Tabletten haben;
Sehtrübung oder andere Sehstörungen haben;
an Galactoseunverträglichkeit, Lactasemangel oder Glucose Galactose-Malabsorption (seltene Stoffwechselerkrankungen) leiden;
eine Herzkrankheit oder andere schwerwiegende Herzprobleme haben oder hatten.

Ihr Arzt bzw. Ihre Ärztin wird Sie fragen, ob Sie oder Ihr Kind andere Arzneimittel einnehmen oder ob Sie, Ihr Kind oder Ihre Familie andere Krankheiten haben (wie z.B. Herzkrankheiten).

Es ist wichtig, dass Sie alle Informationen weitergeben, damit Ihr Arzt bzw. Ihre Ärztin entscheiden kann, ob Methylphenidat Sandoz das richtige Arzneimittel für Sie ist. Ihr Arzt bzw. Ihre Ärztin kann entscheiden, dass Sie weitere medizinische Untersuchungen benötigen, bevor Sie dieses Arzneimittel einnehmen.

Ihr Arzt bzw. Ihre Ärztin wird bei jeder Dosisänderung den Blutdruck und die Herzfrequenz messen oder wenn er bzw. sie entscheidet, dass diese Messungen notwendig sind.

Ihr Arzt bzw. Ihre Ärztin wird zudem kontrollieren, ob Sie Probleme mit Ihrer Stimmung oder Ihrer Gemütsverfassung oder ob Sie andere ungewöhnlichen Gefühle haben oder ob sich diese während der Zeit, in der Sie Methylphenidat Sandoz eingenommen haben, verschlimmert haben.

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie bzw. Ihr Kind an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden!

Bei lang andauernder Einnahme von Methylphenidat wurde bei Kindern ein verlangsamtes Wachstum (Gewichts- und/oder Grössenzunahme) beobachtet. Wenn Patienten oder Patientinnen in einem Alter sind, in dem sie noch wachsen, wird Ihr Arzt bzw. Ihre Ärztin Ihre Grösse und Ihr Gewicht sorgfältig überwachen. Wenn Patienten und Patientinnen nicht wie erwartet weiter wachsen oder an Gewicht zunehmen, kann der Arzt bzw. die Ärztin die Behandlung mit Methylphenidat Sandoz abbrechen.

Es gibt klinische Hinweise darauf, dass während der Verabreichung von Arzneimitteln mit dem Wirkstoff Methylphenidat vermehrt psychiatrische Störungen (einschliesslich Suchtverhalten und suizidales Verhalten) sowie Gewichts- und Appetitverlust auftreten.

Wird das Arzneimittel abgesetzt, kann es zu Entzugserscheinungen, Stimmungsschwankungen oder Überaktivität kommen.

Bei der Verwendung von anderen Methylphenidat-Produkten wurden Akkommodationsstörungen (Schwierigkeiten für die Umstellung von Fern- auf Nahsehen) und Sehstörungen (z.B. verschwommenes Sehen) beobachtet.

Methylphenidat Sandoz kann Benommenheit verursachen. Daher kann dieses Arzneimittel die Reaktionsfähigkeit, die Fahrtüchtigkeit und Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigen! Ausserdem ist es nicht ratsam, während der Behandlung mit Methylphenidat Sandoz Alkohol zu trinken.

Wenn andere Arzneimittel angewendet werden, kann Methylphenidat Sandoz die Wirkung der anderen Arzneimittel beeinflussen oder Nebenwirkungen verursachen.

Wenn eines der nachfolgend aufgeführten Arzneimittel verwendet wird, muss mit dem behandelnden Arzt bzw. der behandelnden Ärztin gesprochen werden, bevor Methylphenidat Sandoz eingenommen werden darf:

Arzneimittel zur Behandlung von Depressionen und Arzneimittel zur Verhinderung von Epilepsie oder von Blutgerinnseln (Blutverdünner), weil Methylphenidat Sandoz die Art und Weise, wie sie vom Körper verarbeitet werden, negativ beeinflussen kann;
Arzneimittel, die den Blutdruck erhöhen, wegen einer möglichen Verstärkung der Blutdruckerhöhung bei gleichzeitiger Verwendung mit Methylphenidat Sandoz;
blutdrucksenkende Arzneimittel mit dem Wirkstoff Clonidin, weil es Berichte über schwere Nebenwirkungen gibt, wenn Methylphenidat und Clonidin gleichzeitig verabreicht werden.

Wenn Sie sich nicht sicher sind, ob Arzneimittel, die Sie bzw. Ihr Kind zu sich nehmen, oben in der Liste aufgeführt sind, fragen Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, bevor Sie bzw. Ihr Kind Methylphenidat Sandoz einnehmen.

+-Darf Methylphenidat Sandoz während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Während der Schwangerschaft sollte Methylphenidat Sandoz nur genommen werden, wenn Ihr Arzt bzw. Ihre Ärztin entschieden hat, dass dies zwingend erforderlich ist.

Wenn Sie Methylphenidat Sandoz nehmen, kann Ihre Muttermilch Methylphenidat enthalten, deshalb sollten stillende Mütter Methylphenidat Sandoz nicht einnehmen.

+-Wie verwenden Sie Methylphenidat Sandoz?

Methylphenidat Sandoz sollte 1× täglich morgens eingenommen werden, weil sich erwiesen hat, dass die Wirkung bis etwa 12 Stunden nach der Einnahme der Retardtabletten anhält. Sie bzw. Ihr Kind muss sich an die vom Arzt bzw. der Ärztin verordnete Dosierung halten. Der Arzt bzw. die Ärztin kann die Menge des Arzneimittels einstellen, bis sie für Sie bzw. Ihr Kind die richtige ist.

Die Retardtablette muss als Ganzes mit einem Glas Wasser geschluckt werden. Die Retardtablette darf nicht zerkaut, zerbrochen oder zerkleinert werden. Die Retardtablette kann mit oder ohne Essen eingenommen werden.

Für Patienten bzw. Patientinnen, die nicht bereits Methylphenidat nehmen oder von einem anderen Stimulans umgestellt werden, beträgt die Anfangsdosis bei Kindern und Jugendlichen 18 mg am Morgen und 18 oder 36 mg am Morgen bei Erwachsenen. Wenn Sie bzw. Ihr Kind gegenwärtig andere Darreichungsformen von Methylphenidat nehmen, wird Ihr Arzt bzw. Ihre Ärztin Ihnen bzw. Ihrem Kind die beste Anfangsdosis verschreiben. Ihr Arzt bzw. Ihre Ärztin wird Ihnen auch sagen, wie lange die Behandlung mit Methylphenidat Sandoz dauern soll. Die Höchstdosis ist eine einmalige Einnahme von 54 mg täglich für Kinder zwischen 6 und 12 Jahren. Die Höchstdosis für Jugendliche und Erwachsene zwischen 13 und 65 Jahren beträgt 1× täglich 72 mg. Die Einnahme des Arzneimittels sollte ohne ärztlichen Rat nicht frühzeitig abgebrochen werden, weil die Symptome sonst wieder auftreten können.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt, Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

Methylphenidat Sandoz soll nicht Kindern unter sechs Jahren gegeben werden. Das Präparat wurde für diese Altersgruppe nicht untersucht.

Was ist zu tun, wenn Sie bzw. Ihr Kind eine oder mehrere Retardtabletten nicht eingenommen haben?

Wenn Sie bzw. Ihr Kind eine Dosis nicht eingenommen haben, ist es am besten, bis zum nächsten Morgen zu warten und dann die nächste Dosis zu nehmen. Denken Sie daran, dass das Arzneimittel bis etwa 12 Stunden nach der Einnahme wirkt. Wenden Sie sich an Ihren Arzt oder Ihre Ärztin, wenn Sie Fragen haben oder wenn mehr als eine Dosis ausgelassen wurde.

Was ist zu tun, wenn Sie bzw. Ihr Kind zu viele Retardtabletten genommen haben?

Wenn Sie zu viele Retardtabletten genommen haben, wenden Sie sich sofort an Ihren Arzt bzw. Ihre Ärztin oder an die Notaufnahme des nächsten Krankenhauses und sagen Sie, wie viele Retardtabletten Sie genommen haben.

Wenn Ihr Kind zu viele Retardtabletten genommen hat, stellen Sie sicher, dass es sich nicht verletzen kann und schützen Sie es vor übermässigem Lärm und grellem Licht. Wenden Sie sich sofort an Ihren Arzt bzw. Ihre Ärztin oder an die Notaufnahme des nächsten Krankenhauses und sagen Sie, wie viele Retardtabletten es genommen hat.

Ältere Patienten

Die Anwendung von Methylphenidat bei älteren Patienten über 65 Jahre ist in kontrollierten Studien nicht untersucht worden.

+-Welche Nebenwirkungen kann Methylphenidat Sandoz haben?

Die häufigsten unerwünschten Wirkungen von methylphenidathaltigen Arzneimitteln wie z.B. Methylphenidat Sandoz sind:

Kopfschmerzen, Nervosität, Schlaflosigkeit, Gelenkschmerzen, Mundtrockenheit, hohe Körpertemperatur (Fieber), aussergewöhnlicher Haarausfall oder dünner werdendes Haar, aussergewöhnliche Schläfrigkeit oder juckender Hautausschlag mit Rötung (Nesselsucht), Husten, Halsschmerzen, Reizungen der Nase und des Rachens, hoher Blutdruck, schneller Herzschlag (Tachykardie), Schwindel, unkontrollierbare Bewegungen, Überaktivität, Aggressivität, Erregtheit, Ängstlichkeit, depressive Stimmung, Reizbarkeit und unnormales Verhalten, Magenschmerzen, Durchfall, Übelkeit, Unwohlsein im Magen und Erbrechen.

Über die weiteren bedeutsamen oder schwerwiegenden Nebenwirkungen bei methylphenidathaltigen Arzneimitteln sollten Sie von Ihrem Arzt/Ihrer Ärztin informiert worden sein. Es sind:

Suizid, Suizidversuch, Suizidgedanken; krankhaft häufig wiederholte Verhaltensweise, Berührungshalluzinationen;
Bewegungsstörungen, Tics oder Verschlechterung bestehender Tics (Tourette-Syndrom), Migräne, Nervenschädigungen, bestimmte Muskel- und Nervenausfälle, Zustände, die mit hohem Fieber und Muskelsteife einhergehen und bis zur Bewusstlosigkeit führen können («malignes neuroleptisches Syndrom»). Wenn dies geschieht, wenden Sie sich sofort an einen Arzt bzw. eine Ärztin oder ein Krankenhaus;
verschwommenes Sehen;
plötzlicher Herztod, Probleme mit den Blutgefässen des Gehirns (Schlaganfall, Entzündung oder Verschluss der Blutgefässe im Gehirn);
Störungen der Blutgefässe oder der Durchblutung von Extremitäten mit Taubheitsgefühl der Finger oder Zehen oder kalten Händen oder Füssen;
Leberfunktionsstörung;
spezielle Entzündungen bzw. Veränderungen der Haut (Exfoliative Dermatitis, Steven-Johnson-Syndrom), allergische Hautauschläge;
Blut im Urin, Brustdrüsenschwellungen beim Mann.
Auftreten einer lang anhaltenden (mehr als 2 Stunden) und schmerzhaften Erektion des Penis, eventuell auch nach Ende der Behandlung.

Folgende Nebenwirkungen wurden in klinischen Studien bei Kindern und Jugendlichen beobachtet:

Häufig: Entzündung des Nasen-Rachenraums, Schlaflosigkeit, Schwindel, Husten, Schmerzen im Nasen-Rachenraum, Oberbauchschmerzen, Erbrechen, Fieber.

Folgende Nebenwirkungen wurden in klinischen Studien bei Erwachsenen beobachtet:

Sehr häufig: verminderter Appetit, Schlaflosigkeit, Kopfschmerzen, Mundtrockenheit, Übelkeit.

Häufig: Infektion des oberen Respirationstraktes, Nasennebenhöhlenentzündung (Sinusitis), Appetitlosigkeit, Angstgefühle, anfängliche Schlafstörungen, depressive Verstimmung, Unruhe, Agitiertheit, Nervosität, Zähneknirschen, Depressionen, Affektlabilität, verminderte Libido, Panik Attacken, Spannung, Aggressionen, Verwirrung, Schwindelgefühl, Zittern, Ameisenlaufen (Kribbeln), Spannungskopfschmerzen, Schwierigkeiten in Scharfsehen (Akkomodationsstörungen), verschwommenes Sehen, Drehschwindel, Tachykardie, Herzklopfen (Palpitationen), Schmerzen in Nasen-Rachenraum, Husten, Atemnot, Aufstossen (Dyspepsie), Erbrechen, Verstopfung, übermässiges Schwitzen, Muskelspannung, Muskelkrämpfe, erektile Dysfunktion, Reizbarkeit, Müdigkeit, Durst, Kraftlosigkeit (Asthenie), Gewichtsverlust, erhöhte Herzfrequenz, erhöhter Blutdruck, erhöhte Alaninaminotransferase-Werte im Blut, Schläfrigkeit.

Folgende Nebenwirkungen wurden in klinischen Studien bei Kindern, Jugendlichen und Erwachsenen beobachtet:

Häufig: nervöses Zucken (motorische Tics), Stimmungsschwankungen, Schläfrigkeit, Durchfall, Bauchbeschwerden, Hautausschlag, Nervosität (Zappeligkeit).
