Litarex ist ein Arzneimittel, das auf Verschreibung des Arztes oder der Ärztin gegen manische Depressionen eingesetzt wird. Der Wirkstoff von Litarex, das Lithium, befindet sich in einer speziellen Tablette, welche eine allmähliche Freisetzung über mehrere Stunden garantiert.
Wenn Litarex eingenommen wird, darf nicht gestillt werden (siehe «Darf Litarex während einer Schwangerschaft oder in der Stillzeit eingenommen werden?»).
Salzmangel vermindert die Ausscheidung von Lithium aus dem Körper und kann dadurch die Nebenwirkungen von Lithium verstärken. Salzmangel kann entstehen durch salzarme Diät, Salzverlust beim Schwitzen oder durch harntreibende Arzneimittel.
Teilen Sie Ihrem Arzt oder Ihrer Ärztin mit, wenn Sie an Herzinsuffizienz oder an anderen Herzkrankheiten, an der Parkinson-Krankheit, an einer Schilddrüsenerkrankung oder an Epilepsie leiden.
Der in Litarex enthaltene Wirkstoff Lithium ist bei erhöhten Konzentrationen im Blut schädlich. Deshalb führt Ihr Arzt bzw. Ihre Ärztin eine regelmässige Blutuntersuchung zur Bestimmung des Lithium-Blutspiegels durch, um unerwünschte Wirkungen zu vermeiden.
Die Empfindlichkeit auf Lithium ist von Patient zu Patient verschieden. Brechen Sie die Therapie mit Litarex sofort ab und informieren Sie oder Ihre Angehörigen ohne Verzug Ihren Arzt bzw. Ihre Ärztin, falls Sie eines oder mehrere der folgenden Symptome deutlich verspüren:
Ständiger starker Durst und Harndrang, Durchfall, Übelkeit, Bewegungs- und Sprachstörungen, starkes und unter Umständen unregelmässiges Zittern der Hände, Muskelzuckungen und starke Schläfrigkeit oder Trägheit.
Die Nierenfunktion sollte von Ihrem Arzt bzw. von Ihrer Ärztin überprüft werden, da es bei Anwendung von Litarex sehr selten zu Nierenschädigungen kommen kann.
Bei gleichzeitiger Einnahme von Lithium und entzündungshemmenden Schmerzmitteln, Metronidazol, gewissen Arzneimitteln zur Behandlung von Bluthochdruck oder Herzinsuffizienz, wie ACE-Hemmer (z.B. Captopril, Enalapril) oder Angiotensin-II-Rezeptor-Antagonisten, sogenannter Sartane (z.B. Candesartan, Valsartan), kann sich der Lithiumspiegel erhöhen und zu starken Nebenwirkungen führen. Sie müssen Ihren Arzt bzw. Ihre Ärztin informieren, wenn Sie solche Arzneimittel einnehmen.
Auch die gleichzeitige Einnahme weiterer Arzneimittel wie harntreibende Mittel, Antidepressiva, Arzneimittel bei psychischen Störungen, Antiepileptika oder Calcitonin ist Ihrem Arzt oder Ihrer Ärztin zu melden bzw. mit ihm oder ihr zu besprechen.
Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fahrtüchtigkeit und die Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigen, da es bei der Einnahme von Litarex unter anderem zu Schläfrigkeit, Schwindel und Halluzinationen kommen kann.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbst gekaufte!) einnehmen oder äusserlich anwenden.
Wenn Sie schwanger sind, dürfen Sie Litarex nicht einnehmen. Wenn Sie planen schwanger zu werden, besprechen Sie dies mit Ihrem Arzt bzw. Ihrer Ärztin. Wenn Sie im gebärfähigen Alter sind und Litarex einnehmen, sollten Sie auf eine sichere Empfängnisverhütung achten. Falls Sie trotzdem schwanger werden, müssen Sie umgehend Ihren Arzt bzw. Ihre Ärztin kontaktieren.
Während der Behandlung mit Litarex darf nicht gestillt werden.
Ihr Arzt bzw. Ihre Ärztin ermittelt die für Sie richtige Dosierung. Diese erfolgt durch die Bestimmung des Lithiumgehaltes in Ihrem Blut und die dabei beobachtete Wirkung.
Die Behandlung wird begonnen mit je 1 Retardtablette Litarex morgens und abends. Die Retardtabletten sollen unzerkaut mit etwas Wasser geschluckt werden. Die Retardtablette verfügt über eine Zierrille, die nicht zum Teilen der Retardtabletten in gleiche Dosen bestimmt ist.
Nach einer Woche wird die Lithiumkonzentration im Blut gemessen, und zwar 12 Stunden nach Einnahme der letzten Retardtablette. Aufgrund dieses Blutwertes bestimmt der Arzt bzw. die Ärztin die für Sie notwendige Dosierung. Nach optimaler Einstellung genügt es in der Regel, den Gehalt von Lithium im Blut alle 2?3 Monate zu kontrollieren.
Da die Aufrechterhaltung des richtigen Blutspiegels von Lithium so wichtig für den Behandlungserfolg ist ? zu tiefe Konzentrationen wirken nicht genug, zu hohe Konzentrationen erzeugen Nebenwirkungen ?, sollten Sie den Anweisungen des Arztes bzw. der Ärztin unbedingt Folge leisten.
Nehmen Sie das Arzneimittel zu der von Ihrem Arzt bzw. Ihrer Ärztin festgesetzten Zeit ein.
Falls Sie eine Dosis vergessen haben, warten Sie den nächsten vorgeschriebenen Zeitpunkt der Einnahme ab. Nehmen Sie keine doppelten Dosen zur Kompensation vergessener Dosierungen ein.
Ältere Patienten benötigen oft kleinere Dosen, um die gewünschte therapeutische Wirkung zu erreichen, und Nebenwirkungen können sich bei älteren Patienten bei Dosierungen äussern, die von jüngeren Patienten toleriert werden.
Litarex ist zur Behandlung von Kindern nicht geeignet.
Eine Anwendung des Präparates bei Jugendlichen soll nur in Spitälern mit entsprechender Erfahrung erfolgen.
Ändern Sie deshalb nicht von sich aus die verschriebene Dosierung.
Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Zu Beginn einer Behandlung mit Litarex können auftreten: leichtes Zittern der Hände, erhöhte Harnmenge, Durst und Übelkeit. Diese Nebenwirkungen verschwinden aber in der Regel nach einiger Zeit. Sollten sie länger anhalten oder fühlen Sie sich dadurch zu stark beeinträchtigt, so benachrichtigen Sie Ihren Arzt oder Ihre Ärztin.
Bei Dauerbehandlung wurden folgende Nebenwirkungen beobachtet:
Ganzer Körper: Ödeme (Wasseransammlung im Gewebe).
Herz und Kreislauf: Herzrhythmusstörungen, niedriger Blutdruck, EKG-Veränderungen, Kreislaufversagen, Raynaud-Symptomatik (anfallsartige stärkere Durchblutungsstörungen mit z.B. Blässe von Fingern oder Zehen).
Nervensystem: Zittern der Hände, Muskelzuckungen oder Krämpfe, Bewegungsstörungen, Anfälle, verwaschene Sprache, Schwindel, Augenzittern, Bewusstlosigkeit, Kopfschmerzen.
Haut: Haarausfall, Akne, Ausschläge, Juckreiz, Auslösen oder Verstärken der Psoriasis (Schuppenflechte).
Hormonell: Kropfbildung, Störung der Schilddrüsenfunktion.
Magen und Darm: Appetitmangel, Übelkeit und Erbrechen, Durchfall, Bauchschmerzen, übermässiger Speichelfluss, Mundtrockenheit.
Blut: Anstieg der weissen und roten Blutkörperchen sowie der Blutplättchen.
Stoffwechsel: Anstieg des Blutzuckers, Anstieg von Kalzium im Blut, Gewichtszunahme.
Muskulatur: Gelenkschmerzen, Muskelschmerzen.
Gehirn: Halluzinationen, Schläfrigkeit, Gedächtnisverlust.
Nieren: Durst, erhöhte Harnmenge, Harninkontinenz, Nierenstörung (nephrotisches Syndrom). Sehr selten bleibende Nierenschädigung (Fibrose).
Geschlechtsorgane: Impotenz, sexuelle Funktionsstörungen.
Sinnesorgane: Sehstörungen.
Falls Nebenwirkungen auftreten, von denen Sie einen Zusammenhang mit der Einnahme von Litarex vermuten, konsultieren Sie umgehend Ihren Arzt bzw. Ihre Ärztin. Die ersten Anzeichen einer Überdosierung können sich durch starkes Zittern, Redeschwierigkeiten, starkes Erbrechen, Durchfälle äussern. Bitte konsultieren Sie in diesem Fall sofort Ihren Arzt oder Ihre Ärztin.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP:» bezeichneten Datum verwendet werden.
Die Retardtabletten sind in Kunststoffflaschen abgepackt. Die Flasche ist nach Entnahme einer Retardtablette wieder gut zu verschliessen.
Bei Raumtemperatur (15-25 °C) in der Originalverpackung, vor Feuchtigkeit geschützt und ausser Reichweite von Kindern aufbewahren.
