+-Was ist Glimepirid Actavis und wann wird es angewendet?

Glimepirid Actavis Tabletten enthalten den Wirkstoff Glimepirid, ein Sulfonylharnstoffderivat. Glimepirid senkt den Blutzuckerspiegel, indem es die Bauchspeicheldrüse stimuliert, mehr Insulin ins Blut abzugeben. Glimepirid Actavis wird zur Behandlung des Typ 2-Diabetes (Erwachsenen-Diabetes) angewendet, wenn Diät, körperliche Aktivität und Gewichtsreduktion allein den Blutzucker nicht mehr zu kontrollieren vermögen.

Glimepirid Actavis Tabletten dürfen nur auf Verschreibung des Arztes bzw. der Ärztin angewendet werden.

Eine häufige Kontrolle der Blut- und Urinzuckerspiegel, vor allem in der Anfangsphase der Behandlung, ist wichtig.

Für eine erfolgreiche Behandlung mit Glimepirid Actavis ist eine Diabetes-Diät, die Sie mit Ihrem Arzt bzw. Ihrer Ärztin oder Ihrer Beratungsstelle besprechen, eine Grundvoraussetzung. Dies ist besonders wichtig während der Anfangsphase der Behandlung mit Glimepirid Actavis, wenn Sie zusammen mit Ihrem Arzt bzw. Ihrer Ärztin die für Sie optimale Dosierung suchen.

Diät ist die wichtigste Massnahme gegen Zuckerkrankheit.

Trotz der Einnahme von blutzuckersenkenden Tabletten müssen Sie strikt auf Ihre Diät achten. Wenn Sie Übergewicht haben, kann es unerlässlich sein, dass Sie die Kalorienzufuhr senken und abnehmen.

Es ist sehr wichtig, dass Sie die Tabletten regelmässig einnehmen und die Diät einhalten, damit Ihr Blutzuckerspiegel nicht zu hoch ansteigt (Hyperglykämie) oder zu tief absinkt (Hypoglykämie), beides Zustände, die Ihre Leistungsfähigkeit erheblich beeinträchtigen oder sogar zu ernsthaften Komplikationen führen können. Aus diesen Gründen dürfen Sie weder die verordnete Dosierung ändern, noch von den mit Ihrem Arzt bzw. Ihrer Ärztin besprochenen Einnahmeregeln abweichen, ohne dies vorher mit ihm bzw. ihr zu besprechen.

+-Wann darf Glimepirid Actavis nicht angewendet werden?

Glimepirid Actavis darf nicht eingenommen werden

von zuckerkranken Patienten oder Patientinnen, deren Bauchspeicheldrüse gar kein Insulin mehr bilden kann, was insbesondere bei Entwicklung der Zuckerkrankheit im Kindes- oder Jugendlichen-Alter zutrifft (sogenannter Typ 1-Diabetes),
bei unstabilem Diabetes mit häufigen, schweren Stoffwechselentgleisungen (insbesondere bei Überzuckerungen, die begleitet sind von Azetongeruch in der Atemluft, Nachweis von Ketokörpern im Urin oder Bewusstseinstrübung bis zur Bewusstlosigkeit),
bei schweren Funktionsstörungen der Nieren, Leber, Nebennierenrinde, des Hypophysenvorderlappens und der Schilddrüse,
bei Überempfindlichkeit auf den Wirkstoff Glimepirid oder andere Bestandteile der Tabletten, andere Sulfonylharnstoffpräparate oder Sulfonamide.

Glimperid Actavis Tabletten zu 2 mg enthalten Azofarbstoffe. Patientinnen und Patienten, die auf Azofarbstoffe, Acetylsalicylsäure sowie Rheuma- und Schmerzmittel (Prostaglandin-Hemmer) überempfindlich reagieren, sollen die Tabletten zu 2 mg nicht anwenden.

+-Wann ist bei der Einnahme von Glimepirid Actavis Vorsicht geboten?

Bitte beachten Sie die folgenden Anzeichen von unerwünschten Blutzuckerveränderungen:

Zu hoher Blutzucker: starkes Durstgefühl, Mundtrockenheit, häufiges Wasserlassen und trockene Haut. Informieren Sie Ihren Arzt bzw. Ihre Ärztin, damit er bzw. sie mit Ihnen Ihre Diät besprechen oder die Tabletteneinnahme korrigieren kann.

Zu tiefer Blutzucker: Heisshunger, Übelkeit, Erbrechen, Schwitzen, Zittern, Unruhe, Reizbarkeit, Kopfschmerzen, Schlafstörungen, Verwirrtheit, Schwindel, Herzklopfen und andere. Dieser Zustand lässt sich häufig rasch durch Einnahme von Zucker (z.B. Würfelzucker [2-4 Stück]), süsse Fruchtsäfte oder gezuckerten Tee) beheben. Beachten Sie, dass künstliche Süssstoffe unwirksam sind. Wenn die Anzeichen nicht sofort verschwinden, so informieren Sie unverzüglich Ihren Arzt bzw. Ihre Ärztin. Wird die Behandlung unterlassen, kann es infolge extremer Blutzuckersenkungen zu Sprech- und Sehstörungen, Lähmungserscheinungen oder Empfindungsstörungen und Bewusstlosigkeit kommen.

Bei Durchfall, unvorhergesehenem Auslassen einer Mahlzeit oder bei körperlicher Anstrengung ist der Blutzuckerspiegel normalerweise tiefer. Eine Einnahme von Glimepirid Actavis kann dann den Blutzuckerspiegel zu tief absinken lassen, was Sie mit Einnahme von Zucker ausgleichen können. Wenn die Zeichen nicht sofort verschwinden, so informieren Sie unverzüglich Ihren Arzt bzw. Ihre Ärztin.

In aussergewöhnlichen Stress-Situationen (z.B. bei Verletzungen, fieberhaften Infektionen, Operationen) kann es zu einer Entgleisung des Blutzuckers kommen und eine temporäre Behandlung mit Insulin kann notwendig werden.

Bei der Anwendung von blutzuckersenkenden Arzneimitteln wie Glimepirid Actavis kann als Folge eines zu tiefen oder zu hohen Blutzuckers Ihr Reaktionsvermögen vermindert sein. Dadurch kann die Fahrtüchtigkeit und die Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigt sein. Dies gilt besonders zu Beginn der Behandlung bzw. wenn zusätzlich Alkohol konsumiert wird.

Durch die gleichzeitige Anwendung anderer Arzneimittel kann die Wirkung von Glimepirid Actavis verstärkt oder abgeschwächt werden. Dies gilt vor allem für bestimmte Schmerzmittel und Entzündungshemmer (z.B. Salicylate), gewisse Antibiotika (Rifampicin, Clarithromycin, Sulfonamide [z.B. Cotrimoxazol], Chinolone [z.B. Ciprofloxacin, Ofloxacin, Levofloxacin], Tetracycline, Chloramphencol), Pilzmittel (z.B. Miconazol, Fluconazol), Kortikosteroide (z.B. Cortison), Adrenalin, Diazoxid (blutdrucksteigerndes und blutdrucksenkendes Arzneimittel), bestimmte Hormone (Anabolika, männliche Sexualhormone, Danazol, Arzneimittel zur Empfängnisverhütung oder gegen Beschwerden in den Wechseljahren, Schilddrüsenhormone), harntreibende Arzneimittel, magensäurehemmende Arzneimittel (Cimetidin, Ranitidin), bestimmte Asthmamittel (z.B. Salbutamol, Terbutalin), bestimmte Arzneimittel gegen Herz- und Gefässerkrankungen (Disopyramid, Betablocker, ACE-Hemmer [z.B. Enalapril, Captopril, Lisinopril]), bestimmte Arzneimittel zur Senkung des erhöhten Cholesterinspiegels (Fibrate, Nicotinsäure), bestimmte blutgerinnungshemmende Arzneimittel, bestimmte Arzneimittel gegen Depressionen und andere psychische Erkrankungen (z.B. Fluoxetin, Chlorpromazin), MAO-Hemmer zur Behandlung der Parkinsonkrankheit und von Depressionen, bestimmte Arzneimittel gegen Epilepsie (Phenytoin, Phenobarbital), Probenecid gegen Gicht, Abführmittel (bei missbräuchlicher Anwendung).

Gewisse blutdrucksenkende Arzneimittel (z.B. Betablocker, Clonidin, Reserpin) können die Wahrnehmung der Warnzeichen einer Unterzuckerung beeinträchtigen.

Glimepirid Actavis kann die Wirkung von gewissen blutgerinnungshemmenden Arzneimitteln verstärken oder abschwächen.

Alkohol kann die blutzuckersenkende Wirkung von Glimepirid Actavis sowohl verstärken als auch abschwächen.

Fragen Sie immer nach möglichen Wechselwirkungen, wenn Ihnen ein Arzneimittel neu verschrieben wird oder Sie in einer Apotheke ein Arzneimittel kaufen.

Bei Patienten mit G6PD-Mangel (Anämie-Form) kann das Risiko einer hämolytischen Anämie nach der Einnahme von Glimepirid Actavis erhöht sein. Der Arzt bzw. die Ärztin wird dann eine alternative Therapie in Erwägung ziehen.

Patientinnen und Patienten, die auf Azofarbstoffe, Acetylsalicylsäure sowie Rheuma- und Schmerzmittel (Prostaglandin-Hemmer) überempfindlich reagieren, sollen die Tabletten Glimepirid Actavis zu 2 mg nicht anwenden.

Patienten mit Galaktose- oder Laktose-Unverträglichkeit oder Malabsorption von Glukose-Galaktose dürfen Glimepirid Actavis nicht nehmen.

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbst gekaufte!) einnehmen oder äusserlich anwenden.

+-Darf Glimepirid Actavis während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Glimepirid Actavis darf während der Schwangerschaft und Stillzeit nicht eingenommen werden, da eine schädigende Wirkung auf den Fötus oder den Säugling (v.a. Unterzuckerung infolge Übertritt von Glimepirid in die Muttermilch) nicht ausgeschlossen werden kann. Diabetes in der Schwangerschaft und Stillzeit ist besser mit Insulin zu behandeln.

Bei Kinderwunsch ist frühzeitig ein Gespräch mit dem behandelnden Arzt bzw. der Ärztin aufzunehmen. Wenn Sie während der Behandlung mit Glimepirid Actavis schwanger werden sollten, müssen Sie sofort Ihren Arzt bzw. Ihre Ärztin verständigen. Er bzw. sie wird Sie zur weiteren Behandlung Ihrer Zuckerkrankheit beraten.

+-Wie verwenden Sie Glimepirid Actavis?

Nehmen Sie die Glimepirid Actavis Tabletten jeweils morgens unmittelbar vor dem Frühstück unzerkaut mit ausreichend Flüssigkeit ein. Falls Sie nicht frühstücken, nehmen Sie die Tabletten erst vor der nächsten Hauptmahlzeit ein. Wenn Sie Glimepirid Actavis eingenommen haben, dürfen Sie die anschliessende Mahlzeit nicht auslassen. Ihr Arzt bzw. Ihre Ärztin wird Sie entsprechend beraten.

Zu Beginn der Behandlung wird Ihr Arzt bzw. Ihre Ärztin Ihnen eine kleinere Dosis verordnen. Diese wird, wenn nötig, in ein- bis zweiwöchigen Abständen erhöht, bis Ihr Blutzuckerspiegel optimal eingestellt ist.

Im Laufe der Behandlung kann die notwendige Glimepirid Actavis-Dosierung abnehmen. Deshalb müssen von Ihrem Arzt bzw. Ihrer Ärztin regelmässige Stoffwechselkontrollen durchgeführt werden.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin.

Die Einhaltung der von Ihrem Arzt bzw Ihrer Ärztin vorgeschriebenen Einnahme von Glimepirid Actavis ist wichtig. Einnahmefehler dürfen Sie nicht mit nachträglichen oder zusätzlichen Einnahmen korrigieren. Informieren Sie unverzüglich Ihren Arzt bzw. Ihre Ärztin, wenn Sie Glimepirid Actavis falsch eingenommen haben.

+-Welche Nebenwirkungen kann Glimepirid Actavis haben?

Folgende Nebenwirkungen können bei der Einnahme von Glimepirid Actavis auftreten:

Häufig ist das Vorkommen einer Unterzuckerung (Hypoglykämie). Die hierbei möglicherweise auftretenden Symptome sind im Abschnitt «Wann ist bei der Anwendung von Glimepirid Actavis Vorsicht geboten?» beschrieben.

Gelegentlich können Übelkeit, Erbrechen, Durchfall, Druck- oder Völlegefühl auftreten.

Gelegentlich treten auch Überempfindlichkeitsreaktionen der Haut, wie Hautausschläge, Juckreiz oder Überempfindlichkeit gegenüber UV-Licht auf. In Einzelfällen können sich aus leichteren Überempfindlichkeitsreaktionen auch schwerwiegende Symptome entwickeln, wie Atemnot und schockartige Reaktionen.

Veränderungen des Blutbildes (z.B. Verminderung der Zahl der Blutplättchen verbunden mit einem Hautausschlag; und Verringerung der weissen und der roten Blutkörperchen) sind selten, können jedoch schwerwiegend sein. In Einzelfällen ist eine Entzündung der Blutgefässe berichtet worden. Sehr selten sind Fälle von Veränderungen der Leberfunktion (z.B. Gelbsucht, Leberentzündung, Gallenstauung) berichtet worden, die sehr schwerwiegend und gefährlich sein können.

Vor allem am Anfang der Behandlung können vorübergehende Sehstörungen durch die Änderungen des Blutzuckerspiegels auftreten.

Wenn Sie oben genannte Symptome beobachten oder Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

Glimepirid Actavis ist bei Raumtemperatur (15?25 °C) in der Originalpackung und ausserhalb der Reichweite von Kindern aufzubewahren.

Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP:» bezeichneten Datum verwendet werden.
