Euthyrox enthält das synthetische Schilddrüsenhormon Levothyroxin, das mit dem von der Schilddrüse gebildeten Hormon völlig identisch ist. Euthyrox wird bei verschiedenen Formen einer Schilddrüsenunterfunktion zur Ergänzung des fehlenden Hormons angewendet. Bei Schilddrüsenüberfunktion kann Euthyrox gemeinsam mit einem Mittel zur Hemmung der Schilddrüsenfunktion (Thyreostatikum) eingenommen werden, um eine stabilere Stoffwechsellage zu gewährleisten. Ausserdem dient Euthyrox zur Unterdrückung eines erneuten Tumorwachstums und zur Ergänzung fehlender Schilddrüsenhormone vor allem nach operativer Entfernung von bösartigen Tumoren (Malignomen) der Schilddrüse. Euthyrox wird auch angewendet zum Testen der Schilddrüsenfunktion, sowie zur Verhütung einer erneuten Kropfbildung nach einer Kropfoperation oder zur Behandlung eines gutartigen Kropfes.
Die bei Unterfunktion der Schilddrüse oft bestehenden Krankheitszeichen wie Antriebsarmut, Leistungsabfall, Müdigkeit, Kälteempfindlichkeit, Veränderungen von Haut und Haaren oder Verstopfung werden durch die Behandlung mit Euthyrox beseitigt.
Das Arzneimittel darf nur auf Verschreibung des Arztes oder der Ärztin angewendet werden.
Bei Überfunktion der Schilddrüse. Ausnahme: als Begleittherapie bei Einnahme eines Mittels zur Hemmung einer Schilddrüsenüberfunktion (nach Erreichen der normalen Stoffwechsellage).
Im Falle einer bekannten Überempfindlichkeit auf Levothyroxin, Laktose oder auf einen anderen Hilfsstoff darf Euthyrox nicht eingenommen werden.
Euthyrox darf ebenfalls nicht angewendet werden bei akutem Herzinfarkt, Schmerzen und Engegefühl in der Brust (Angina pectoris), Herzschwäche mit gesteigertem Herzschlag, akuter Entzündung des Herzmuskels oder der Herzwand, sowie bei unbehandelten Funktionsstörungen der Nebennierenrinde oder der Hypophyse. Bei Schilddrüsenüberfunktion während der Schwangerschaft darf Euthyrox nicht zusammen mit einem Mittel zur Hemmung der Schilddrüsenüberfunktion (Thyreostatikum) eingenommen werden.
Da nicht erwiesen ist, dass Schilddrüsenhormone bei übergewichtigen Patienten mit einer normalen Schilddrüsenfunktion eine gewichtsvermindernde Wirkung haben, und höhere Dosen schwere Nebenwirkungen verursachen können, darf Euthyrox nicht zur Behandlung von Übergewicht angewendet werden.
Informieren Sie unbedingt Ihren Arzt oder Ihre Ärztin, falls Sie an anderen Krankheiten leiden. Da Euthyrox Laktose enthält, ist Vorsicht geboten, falls Sie auf Laktose überempfindlich sind.
Mittel gegen epileptische Anfälle (z.B. Carbamazepin, Phenytoin, Barbiturate), Antibiotika (z.B. Rifampicin), Arzneimittel zur Behandlung von psychischen Erkrankungen (wie Lithiumpräparate, Johanniskraut), Arzneimittel gegen Herzrhythmusstörungen, Schmerzmittel, wassertreibende Arzneimittel, Blutverdünner, Arzneimittel gegen HIV-Infektionen (vom Typ Proteasehemmer), Arzneimittel zur Behandlung von Krebserkrankungen (vom Typ Tyrosinkinasehemmer).
Eine gleichzeitige Einnahme von Orlistat (einem Arzneimittel zur Gewichtsreduktion) kann die Wirkung von Euthyrox verringern. Gegebenenfalls muss Ihr Arzt/Ihre Ärztin die Dosierung von Euthyrox anpassen.
Colestyramin oder Colestipol gegen erhöhte Cholesterinspiegel (Lipidsenker) können die Aufnahme von Euthyrox aus dem Darm hemmen und sollten daher 4-5 Stunden nach Euthyrox eingenommen werden.
Aluminium- und calciumhaltige Arzneimittel gegen Magenübersäuerung oder Sodbrennen, Eisenpräparate zum Einnehmen können die Aufnahme von Euthyrox aus dem Darm hemmen und sollten deshalb mindestens 2 Stunden nach Euthyrox eingenommen werden.
Wenn Sie erstmals Östrogene (?Pille? oder Hormonersatztherapie nach der Menopause) einnehmen oder das Arzneimittel wechseln, informieren Sie bitte Ihren Arzt resp. Ihre Ärztin, da die Dosierung von Euthyrox gegebenenfalls angepasst werden muss.
Arzneimittel, welche die Leistungsfähigkeit des Herzmuskels verbessern (Herzglykoside), Kreislaufanregende Arzneimittel, Arzneimittel gegen Depressionen vom Typ trizyklische Antidepressiva.
Blutzuckersenkende Arzneimittel (Antidiabetika): Der Blutzuckerspiegel ist regelmässig zu kontrollieren. Unter Umständen wird Ihr Arzt resp. Ihre Ärztin die Dosis des blutzuckersenkenden Arzneimittels anpassen.
Blutverdünner: Eine regelmässige Kontrolle der Blutgerinnung ist erforderlich.
Sojaprodukte können die Aufnahme von Euthyrox verringern. Sprechen Sie mit Ihrem Arzt resp. Ihrer Ärztin falls Sie regelmässig Sojaprodukte konsumieren.
Informieren Sie Ihren Arzt, Apotheker bzw. Ihre Ärztin, Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen.
Während der Schwangerschaft und Stillzeit sollte die Einnahme von Euthyrox nicht unterbrochen werden. Jedoch wird Ihr Arzt oder Ihre Ärztin entscheiden, ob und in welcher Dosierung Sie während einer Schwangerschaft oder während der Stillzeit Euthyrox einnehmen müssen. Informieren Sie Ihren Arzt, Ihre Ärztin, wenn Sie eine Schwangerschaft planen, resp. schwanger sind oder stillen.
Die gesamte Tagesdosis wird morgens nüchtern, mindestens ½ Stunde vor dem Frühstück, unzerkaut mit etwas Flüssigkeit eingenommen. Falls nötig können die Tabletten geteilt werden.
Die erforderliche tägliche Dosis ist individuell unterschiedlich und wird durch den Arzt oder die Ärztin anhand labordiagnostischer und klinischer Untersuchungen festgelegt.
Die übliche Anfangsdosis für Erwachsene beträgt 25-50 µg täglich. Erhöhung der Tagesdosis in Abständen von 2-4 Wochen gemäss Anweisung des Arztes bzw. der Ärztin, um jeweils 25-50 µg bis zur Normalisierung des Stoffwechsels. Die Erhaltungsdosis beträgt in der Regel 100-200 µg pro Tag.
Bei älteren Patienten, Patienten mit Herzkrankheiten, mit seit langem bestehender oder schwerer Schilddrüsenunterfunktion oder solchen mit anderen Stoffwechselstörungen ist die Anfangsdosis niedriger.
Bei Kindern wird die Dosis entsprechend dem Körpergewicht festgelegt.
Säuglinge erhalten die gesamte Tagesdosis mindestens ½ Stunde vor der ersten Tagesmahlzeit. Hierzu lässt man die Tabletten in etwas Wasser zerfallen und verabreicht die entstandene milchige Mischung mit etwas zusätzlicher Flüssigkeit sofort.
Halten Sie sich strikte an die vom Arzt bzw. von der Ärztin verordnete Dosis. Haben Sie versehentlich an einem Tag die Einnahme von Euthyrox vergessen, so nehmen Sie am folgenden Tag nur Ihre normale Dosis ein. Eine erfolgreiche Behandlung setzt die regelmässige Einnahme des Arzneimittels voraus. Falls trotz mehrwöchiger Tabletteneinnahme die bei einem Mangel an Schilddrüsenhormonen auftretenden Krankheitszeichen weiter bestehen, sollte der Arzt oder die Ärztin aufgesucht werden, der/die gegebenenfalls über eine Erhöhung der Dosis entscheiden wird.
Bei Unterbrechung oder vorzeitiger Beendigung der Behandlung kann es zum Wiederauftreten von Krankheitssymptomen kommen, deren Art von der jeweiligen Grunderkrankung abhängig ist.
Die Behandlung mit Euthyrox erfolgt bei Schilddrüsenunterfunktion und nach Operation eines bösartigen Tumors der Schilddrüse meist lebenslang, bei gutartigem Kropf und zur Verhütung eines erneuten Kropfwachstums mehrere Monate bis Jahre. Bei Behandlung der Schilddrüsenüberfunktion wird Euthyrox gleichlang wie das Arzneimittel zur Hemmung der Schilddrüsenfunktion eingenommen. Schilddrüsenfunktionstest: Einnahme während 14-28 Tagen bis zur Durchführung des Szintigramms.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Als Folge der stoffwechselsteigernden Wirkung von Euthyrox können gelegentlich, vor allem bei zu hoher Dosierung oder bei zu schneller Steigerung der Dosis zu Beginn der Behandlung, folgende Nebenwirkungen auftreten: übermässig rascher Herzschlag (Tachykardie), Herzrhythmusstörungen, Schmerzen im Brustkorb (Angina pectoris), innere Unruhe, vermehrtes Schwitzen, Durchfall, Gewichtsabnahme, Zittern, Kopfschmerzen, Schlaflosigkeit, psychische Störungen mit Verlust des Realitätsbezuges. In diesen Fällen sollten Sie Rücksprache mit dem Arzt oder der Ärztin nehmen. Unter Umständen wird Ihre Therapie geändert. Es wurde über allergische Reaktionen wie Gesichts- oder Halsschwellung (Angioödem) berichtet.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt, Apotheker bzw. Ihre Ärztin, Apothekerin informieren.
Euthyrox soll ausserhalb der Reichweite von Kindern, nicht über 25 °C und zum Schutz vor Licht in der Originalverpackung aufbewahrt werden.
Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden.
