+-Was ist Eprex und wann wird es angewendet?

Bei Eprex (Epoetin alfa) handelt es sich um ein gentechnologisch hergestelltes Präparat, das die Bildung der roten Blutzellen anregt.

Eprex wird auf Verschreibung des Arztes oder der Ärztin zur Korrektur von Blutarmut, verursacht durch chronisches Nierenversagen oder durch die Behandlung einer Tumorerkrankung mit bestimmten Arzneimitteln, eingesetzt.

Eprex kann von Ihrem Arzt oder Ihrer Ärztin auch zur Erhöhung der Eigenblutspende vor einer Operation verabreicht werden.

Wenn Sie an Blutarmut ohne Eisenmangel leiden und ein grosser orthopädischer Eingriff vorgesehen ist, kann Ihnen Ihr Arzt oder Ihre Ärztin Eprex zur Verminderung von Fremdbluttransfusionen verschreiben. Ihr Arzt oder Ihre Ärztin wird Ihnen gleichzeitig mit Beginn der Eprex-Behandlung ein Eisenpräparat verschreiben oder empfehlen.

Eprex darf nur auf Verschreibung des Arztes oder der Ärztin angewendet werden.

Die Fertigspritzen sind mit dem Protecs? Sicherheitssystem ausgerüstet, welches hilft, Verletzungen durch Nadelstiche nach Gebrauch der Fertigspritze zu verhindern.

+-Was sollte dazu beachtet werden?

Während der Behandlung mit Eprex muss der Blutdruck regelmässig kontrolliert werden. Bei allen Patienten muss der Spiegel des roten Blutfarbstoffes (sog. Hämoglobin) vom Arzt/der Ärztin engmaschig überwacht werden, da das Risiko für thromboembolische und kardiovaskuläre Ereignisse mit tödlichem Ausgang erhöht sein kann, wenn für die Behandlung ein Hämoglobinspiegel angestrebt wird, der über dem Zielbereich für die Indikation liegt. Das Risiko kann bei Patienten, mit chronischer Nierenstörung, welche auf die Behandlung mit Eprex nicht gut ansprechen, höher sein.

+-Wann darf Eprex nicht angewendet werden?

Bei bekannter Überempfindlichkeit gegenüber dem Wirkstoff von Eprex (gentechnologisch hergestelltes menschliches Erythropoietin) oder einem anderen Inhaltsstoff sowie bei unkontrolliertem Bluthochdruck darf Eprex nicht angewendet werden.

Patienten, welche aufgrund einer Behandlung mit einem Erythropoietin eine schwere Blutarmut entwickelt haben (Erythroblastopenie), dürfen weder Eprex noch ein anderes Erythropoietin erhalten.

Bei Patienten, bei denen eine grössere Operation geplant ist und die nicht an einem Eigenblutspendeprogramm teilnehmen, darf Eprex nicht eingesetzt werden im Falle von: schwerer Durchblutungsstörung des Herzens (koronare Herzkrankheit), der Arterien der Beine bzw. der Arme (periphere arterielle Verschlusskrankheit), der Halsgefässe (vaskuläre Erkrankung der Karotiden) oder des Gehirns (zerebrovaskuläre Erkrankung) sowie bei vor kurzem eingetretenem Herzinfarkt oder Schlaganfall (zerebrovaskuläres Ereignis). Eprex darf nicht bei Patienten mit einer Vorgeschichte von thrombo-embolischen Ereignissen (Blutgefässverschluss durch Gerinnsel) eingesetzt werden.

Patienten, die im Zusammenhang mit chirurgischen Eingriffen keine angemessene Behandlung zur Vorbeugung einer Blutpfropfbildung (Thrombose-Prophylaxe) erhalten können, dürfen nicht mit Eprex behandelt werden.

+-Wann ist bei der Anwendung von Eprex Vorsicht geboten?

Die Anwendung bei Kindern ist nicht untersucht worden.

Bei allen Patienten, die mit Eprex behandelt werden, muss der Blutdruck engmaschig überwacht und gegebenenfalls behandelt werden. Eprex sollte bei unbehandeltem, unzureichend behandeltem oder schlecht kontrollierbarem Bluthochdruck nur mit Vorsicht angewendet werden. Eine zusätzliche oder verstärkte blutdrucksenkende Therapie kann notwendig sein. Es ist besonders auf das Auftreten von ungewohnten Kopfschmerzen oder auf eine Häufung von Kopfschmerzen als mögliches Warnsignal zu achten. Ist der Blutdruck nicht kontrollierbar, muss die Eprex Behandlung abgebrochen werden.

Bei Patienten mit chronischem Nierenleiden kann es vorkommen, dass bei zu rascher Vermehrung der roten Blutkörperchen der Blutdruck ansteigt. Während der Korrektur der Blutarmut kann es bei diesen Patienten zu gesteigertem Appetit kommen. Falls Ihr Appetit zunimmt, informieren Sie Ihren Arzt oder Ihre Ärztin, evtl. muss Ihre Diät oder Dialyse geändert werden.

Eprex sollte bei Patienten mit Epilepsie, epileptischen Anfällen in der Vorgeschichte oder bei Erkrankungen, die mit einer Veranlagung für epileptische Anfälle in Verbindung stehen, wie ZNS-Infektionen und Hirnmetastasen nur mit Vorsicht angewendet werden.

Eprex sollte bei Patienten mit chronischer Leberfunktionsstörung nur mit Vorsicht angewendet werden. Die Sicherheit von Eprex wurde bei Patienten mit Leberfunktionsstörungen nicht untersucht. Aufgrund des verminderten Stoffwechsels können Patienten mit Leberfunktionsstörungen eine verstärkte Bildung roter Blutkörperchen unter Eprex aufweisen.

In seltenen Fällen wurden von Patienten Monate bis Jahre nach Beginn der Behandlung über eine ungenügende Bildung von roten Blutkörperchen berichtet, was zu einer Verstärkung der Blutarmut führte. Wenn Sie sich plötzlich sehr müde oder schwindlig fühlen oder an Kurzatmigkeit leiden, sollten Sie unverzüglich Ihren Arzt oder Ihre Ärztin informieren. Ihr Arzt oder Ihre Ärztin kann entscheiden, ob Eprex bei Ihnen nicht richtig wirkt und er wird die Therapie gegebenenfalls beenden.

Bei einigen Patientinnen mit Nierenschädigung setzte die Menstruation nach einer Therapie mit Epoetin alfa wieder ein, was eine Schwangerschaft möglich macht. Besprechen Sie in einem solchen Fall mit Ihrem Arzt oder Ihrer Ärztin die Notwendigkeit einer Kontrazeption.

Sehr selten wurde bei Patienten mit chronischer Nierenschädigung unter Epoetin alfa Behandlung eine Verschlimmerung der bestehenden Porphyrie (Störung der Bildung von Blutfarbstoff) beobachtet. Die Anwendung von Epoetin alfa sollte bei diesen Patienten nur mit Vorsicht erfolgen.

Wenn Sie eine Hämodialyse erhalten, können sich an folgenden Orten Blutpfropfen bilden:

  • in Ihrem Dialyse Shunt. Dies ist wahrscheinlicher, wenn Sie einen tiefen Blutdruck haben oder wenn sich Komplikationen mit Ihrer Fistula ergeben.
  • im Hämodialyse-System. Ihr Arzt/resp. Ihre Ärztin könnte entscheiden, dass Ihre Heparin-Dosis während der Dialyse erhöht wird.

Erythropoese-stimulierende Produkte sind Wachstumsfaktoren, welche in erster Linie die Produktion roter Blutzellen stimulieren. Erythropoetin-Rezeptoren können auf der Oberfläche verschiedenster Tumorzellen gebildet werden. Wie bei allen Wachstumsfaktoren gibt es Bedenken, dass erythropoese-stimulierende Produkte das Tumorwachstum stimulieren könnten. Bitte diskutieren Sie dies mit Ihrem Arzt/Ihrer Ärztin.

Aufgrund des erhöhten Risikos für Bluthochdruck während der Anfangsphase der Epoetin alfa Behandlung sollten Patienten mit chronischer Nierenschädigung vorsichtig sein beim Ausführen von potentiell gefährlichen Aktivitäten, wie Autofahren oder Bedienen von Maschinen, bis sie auf die optimale Erhaltungsdosis von Epoetin alfa eingestellt sind.

Informieren Sie Ihren Arzt, Apotheker bzw. Ihre Ärztin, Apothekerin, wenn Sie an andern Krankheiten (v.a. Bluthochdruck, Krampfanfällen, chronischer Leberfunktionsstörung oder Gefässverschlusskrankheiten) leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen.

+-Darf Eprex während einer Schwangerschaft oder in der Stillzeit angewendet werden?

Es ist nicht bekannt, ob Eprex bei Verabreichung während der Schwangerschaft oder Stillzeit schädigende Wirkung auf das ungeborene Kind oder den Säugling hat. Teilen Sie Ihrem Arzt oder Ihrer Ärztin mit, wenn Sie schwanger sind, es werden möchten oder stillen.

+-Wie verwenden Sie Eprex?

Je nach Empfehlung des Arztes oder der Ärztin wird Eprex einmal oder mehrmals pro Woche verabreicht. Die individuelle Dosierung, die Dauer der Therapie und der Verabreichungsweg (Gabe direkt in die Blutbahn oder unter die Haut) wird vom behandelnden Arzt resp. der behandelnden Ärztin vorgeschrieben.

Bei Patienten mit chronischem Nierenleiden sollte die Verabreichung von Eprex vorzugsweise in die Vene (intravenös) oder über einen intravenösen Zugang erfolgen. Ist dies nicht möglich, kann Ihr Arzt oder Ihre Ärztin entscheiden, dass Eprex unter die Haut (subkutan) gespritzt werden soll.

Wenn Sie sich Eprex selber unter die Haut spritzen, ändern Sie nicht von sich aus die vorgeschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt, Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

Anleitung zur subkutanen Selbstinjektion für Patienten:

Versuchen Sie nicht, die Injektion selbst zu machen, bevor sie durch Ihren Arzt/Ihre Ärztin oder eine Pflegeperson darin geschult wurden.

Falls Eprex unter die Haut (subkutan) injiziert wird, beträgt das injizierte Volumen üblicherweise nicht mehr als einen Milliliter (1 ml) in einer einzelnen Injektion.

Eprex wird alleine, ohne Beimischung anderer zur Injektion bestimmter Flüssigkeiten, verabreicht.

Schütteln Sie die Eprex Spritzen nicht. Längeres heftiges Schütteln kann das Produkt beschädigen. Wenn es heftig geschüttelt wurde, verwenden Sie das Produkt nicht.

Durchführung der Selbstinjektion (gilt bei der Verwendung von Fertigspritzen):

Die Fertigspritzen sind mit dem Nadelschutz Protecs? versehen, um dafür zu sorgen, dass Nadelstichverletzungen nach der Anwendung verhindert werden. Dies wird auf der Packung angegeben.

Die Abbildung zeigt wie die Fertigspritze aussieht.

  • Nehmen Sie die Fertigspritze 30 Minuten vor dem Gebrauch aus dem Kühlschrank, damit sie Raumtemperatur annehmen kann. Entfernen Sie während der Zeit bis zum Erreichen der Raumtemperatur nicht die Nadelkappe von der Spritze.
  • Prüfen Sie, dass Sie die verschriebene Dosierungsstärke haben, das Arzneimittel nicht abgelaufen ist, die Packung unversehrt ist und weder Schwebepartikel in der Lösung noch Farbveränderungen vorhanden sind.
  • Stellen Sie das Material (Fertigspritze, Alkoholtupfer, Pflaster) bereit.
  • Wählen Sie eine Stelle für die Injektion: Bauch (jedoch nicht in der Nähe des Bauchnabels) oder Oberschenkel. Wechseln Sie die Injektionsstelle täglich.
  • Waschen Sie Ihre Hände. Desinfizieren Sie die Injektionsstelle mit einem antiseptischen Tupfer.
  • Fassen Sie die Fertigspritze am Spritzenkörper mit der bedeckten Nadel aufwärts zeigend an.
  • Fassen Sie die Spritze nicht am Kolbenkopf, am Kolben, an den Nadelschutzflügeln oder der Nadelhülle an.
  • Ziehen Sie niemals am Kolben.
  • Entfernen Sie die Nadelkappe erst von der Fertigspritze wenn Sie bereit sind Eprex zu injizieren.
  • Nehmen Sie die Nadelkappe von der Spritze ab indem Sie den Spritzenkörper halten und die Nadelkappe vorsichtig abziehen ohne die Spritze zu verdrehen; Drücken Sie nicht am Kolben, berühren Sie nicht die Nadel oder schütteln Sie nicht die Spritze.
  • Berühren Sie nicht die Klammern zur Aktivierung der Nadelhülle (siehe Sternchen * in der Abbildung), damit die Nadel nicht vorzeitig vom Nadelschutz umhüllt wird.
  • Bilden Sie mit Zeigefinger und Daumen eine Hautfalte. Drücken Sie nicht zu fest.
  • Stechen Sie die Nadel in die Hautfalte komplett ein gemäss Anleitung des Pflegepersonals.
  • Spritzeninhalt injizieren: Kolben mit dem Daumen langsam vollständig runterdrücken, bis die gesamte Flüssigkeit injiziert ist. Drücken Sie langsam und gleichmässig, wobei Sie die Hautfalte weiter gedrückt halten. Das Protecs? Sicherheitssystem wird erst aktiviert, wenn der gesamte Spritzeninhalt injiziert ist. Sie hören wahrscheinlich einen Klick, wenn der Nadelschutz Protecs? aktiviert wurde.
  • Wenn der Kolben vollständig hineingedrückt ist, ziehen Sie Spritze heraus und drücken einen Tupfer auf die Einstichstelle, bis es nicht mehr blutet.
  • Nehmen Sie Ihren Daumen langsam von dem Kolben. Sobald der Daumen vom Kolben genommen wird, wird das Sicherheitssystem aktiviert und die Nadel wird vollständig von der Schutzkappe umschlossen.
  • Entsorgen Sie die gebrauchte Spritze in einem verschliessbaren Glas- oder Plastikbehälter.
+-Welche Nebenwirkungen kann Eprex haben?

Die häufigste unerwünschte Wirkung während der Behandlung mit Eprex ist ein dosisabhängiger Anstieg des Blutdruckes oder eine Verschlimmerung eines bereits bestehenden Bluthochdruckes. Der Blutdruck sollte, speziell zu Beginn der Behandlung, überwacht werden. Die häufigsten Nebenwirkungen, die in den klinischen Studien mit Eprex berichtet wurden sind: Durchfall, Übelkeit, Erbrechen, Fieber und Kopfschmerzen. Grippe-ähnliche Symptome können insbesondere zu Beginn der Behandlung auftreten.

Eine erhöhte Häufigkeit thromboembolischer Ereignisse (Gefässverschlüsse) wurde bei Patienten unter der Behandlung mit erythropoese-stimulierenden Produkten beobachtet.

Überempfindlichkeitsreaktionen, einschliesslich Fälle von Hautausschlag (einschliesslich Urtikaria), anaphylaktische Reaktionen und Angioödem wurden berichtet.

Bluthochdruck Krisen mit krankhafter Veränderung des Gehirns (Enzephalopathie) und Krampfanfällen, die eine sofortige ärztliche Versorgung und intensivmedizinische Behandlung erforderten, traten unter Behandlung mit Epoetin-alfa auch bei Patienten mit zuvor normalem oder niedrigem Blutdruck auf. Als mögliches Warnsignal sollte besonders auf plötzlich auftretende stechende migräneartige Kopfschmerzen geachtet werden.

Bei Patienten mit chronischem Nierenleiden und unter Hämodialyse wurde über Verschluss von Gefässzugängen (Fisteln) berichtet.

Bei Patienten mit chronischem Nierenleiden wurde in seltenen Fällen Monate bis Jahre nach der Behandlung mit Eprex oder anderen Erythropoietinen über eine ungenügende Bildung von roten Blutkörperchen im Knochenmark berichtet (Erythroblastopenie). Dies kann zu einer schweren Blutarmut führen. Der Grund für diese Blutarmut bei Patienten, welche Erythropoietin-Produkte verwenden, ist nicht bekannt. Die Berichte zur Erythroblastopenie betrafen Patienten mit chronischem Nierenleiden, bei welchen das Erythropoietin unter die Haut gespritzt wurde. Eprex soll in dieser Patientengruppe vorzugsweise intravenös verabreicht werden. Obwohl dies eine seltene Komplikation ist, müsste in diesem Fall die Therapie mit Erythropoietin gestoppt und die Anämie gegebenenfalls mit Bluttransfusionen behandelt werden.

Unabhängig von der Behandlung mit Eprex kann es bei chirurgischen Patienten mit Herz-Kreislauferkrankungen nach wiederholten Blutspenden zu Gefässverschlüssen und Gefässveränderungen (thrombotische und vaskuläre Ereignisse) kommen.

Wenn solche Reaktionen auftreten, soll die Behandlung sofort unterbrochen und der Arzt, Apotheker bzw. die Ärztin oder Apothekerin informiert werden. Weitere Nebenwirkungen, die in den klinischen Studien mit Eprex berichtet wurden sind: Husten, Kongestion der Atemwege, Gelenkschmerzen, Muskelschmerzen, Schmerzen in den Extremitäten, Schüttelfrost, Reaktionen an der Applikationsstelle, Fieber und periphere Wasseransammlungen (Ödeme).

Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Ihre Apothekerin informieren.

+-Was ist ferner zu beachten?

Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «Exp.» bezeichneten Datum verwendet werden.

Im Spital müssen die Fertigspritzen ungeöffnet im Kühlschrank bei einer Temperatur von 2?8 °C aufbewahrt und vor Licht geschützt gelagert werden. Wenn Sie Eprex zuhause benutzen, müssen Sie unbedingt darauf achten, dass Sie es in der Originalverpackung im Kühlschrank, aber nicht im Gefrierfach aufbewahren.

Eprex darf unter keinen Umständen eingefroren werden.

Nehmen Sie die Fertigspritze 30 Minuten vor Gebrauch aus dem Kühlschrank und lassen sie stehen, damit die Lösung Raumtemperatur annehmen kann. Eprex Fertigspritzen, welche für die unmittelbare Verwendung bestimmt sind, können einmalig für maximal 7 Tage bei Raumtemperatur (nicht über 25 °C!) aufbewahrt werden.

Eprex darf nicht geschüttelt und muss lichtgeschützt gelagert werden.

Verwenden Sie Eprex nicht mehr, wenn diese Lagerhinweise nicht eingehalten wurden.

Das Arzneimittel ausser Reichweite von Kindern aufbewahren.

+-Was ist in Eprex enthalten?

Eprex enthält als Wirkstoff rekombinantes humanes Erythropoietin in steriler Zubereitung zur intravenösen oder subkutanen Anwendung. Die Fertigspritzen enthalten als Hilfsstoffe zur Stabilisation Polysorbat 80 und Glycin und als weitere Hilfsstoffe: Natriumchlorid, Natriumphosphat, Natriumdiphosphat und Wasser für Injektionszwecke.
