Dostinex enthält den Wirkstoff Cabergolin, einen Mutterkornabkömmling. Cabergolin hemmt die Ausschüttung des Hormons Prolaktin aus der Hirnanhangsdrüse. Prolaktin regt die Milchbildung der Brustdrüse an. Überhöhte Prolaktin-Blutspiegel führen zu Störungen der Fruchtbarkeit, wie Ausbleiben oder Verminderung der Monatsblutungen und Ausbleiben des Eisprunges sowie zu unerwünschtem Milchfluss (Lactation). Dostinex wird angewendet:
Dostinex darf nur auf Verschreibung des Arztes bzw. der Ärztin angewendet werden.
Wenn Sie überempfindlich auf Mutterkornabkömmlinge oder auf einen der Hilfsstoffe von Dostinex reagieren, dürfen Sie Dostinex nicht einnehmen.
Wenn bei Ihnen aus der Vorgeschichte eine fibrotische Erkrankung (Bindegewebsvermehrung in einem Organ) bekannt ist oder Sie aktuell an einer krankhaften Bindegewebsvermehrung (sogenannte Fibrose) leiden, bei Ihnen eine Herzklappenerkrankung vorliegt oder wenn Sie an einer schweren Lebererkrankung leiden, dürfen Sie Dostinex nicht einnehmen.
Kinder und Jugendliche dürfen Dostinex nicht einnehmen.
Dostinex sollte mit Vorsicht von Patientinnen und Patienten eingenommen werden, die an folgenden Krankheiten leiden: Herzkreislauferkrankungen, Raynaud-Krankheit (Erkrankung der Blutgefässe mit Gefässkrämpfen), niedrigem Blutdruck, Erkrankungen der Hirngefässe, Leberkrankheiten, Nierenfunktionsstörungen, Magendarmgeschwüre oder Magendarmblutungen. Ebenso ist Vorsicht geboten bei Patientinnen und Patienten, die früher an psychischen Krankheiten litten.
Bei der Anwendung von Dostinex zum Abstillen soll während der ersten Tage im Wochenbett der Blutdruck regelmässig kontrolliert werden. Bei Frauen mit einer sogenannten Präeklampsie (Erkrankung in der Schwangerschaft mit Wasser in den Beinen, Eiweissverlust durch den Harn und Bluthochdruck) und bei Frauen mit Bluthochdruck nach der Geburt sollte Dostinex nicht angewendet werden.
Informieren Sie Ihren Arzt, wenn Sie oder Ihre Familie bzw. Betreuungspersonen bemerken, dass Sie für Sie ungewöhnliche Verhaltensweisen entwickeln, wie zum Beispiel Spielsucht, übermässiges Essen oder Geldausgeben, ein abnorm starker Sexualtrieb oder eine Zunahme sexueller Gedanken oder Gefühle. Ihr Arzt muss möglicherweise Ihre Dosis anpassen oder das Arzneimittel absetzen.
Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fahrtüchtigkeit und die Fähigkeit Werkzeuge oder Maschinen zu bedienen beeinträchtigen. In seltenen Fällen kann während der Behandlung Schläfrigkeit oder plötzliches Einschlafen auftreten. Letzteres kann manchmal ohne Vorwarnung auftreten. Deshalb ist während der Einnahme von Dostinex beim Lenken eines Fahrzeugs oder beim Ausüben von anderen Aktivitäten (z.B. Werkzeuge oder Maschinen bedienen), bei denen eine verminderte Aufmerksamkeit Sie selbst oder andere gefährdet, Vorsicht geboten. Wenn Sie sich tagsüber sehr stark müde fühlen oder ohne erkennbare Schläfrigkeitsanzeichen plötzlich einschlafen, dürfen Sie während der Behandlung kein Fahrzeug lenken oder Werkzeuge oder Maschinen bedienen. Beim Auftreten dieser Anzeichen, sollten Sie Ihren Arzt oder Ihre Ärztin informieren.
Dostinex enthält Laktose (Milchzucker). Daher sollten Patienten und Patientinnen mit der seltenen, vererbten Galactose-Unverträglichkeit, Lactase-Mangel oder Glucose-Galactose-Malabsorption Dostinex nicht einnehmen.
Die Wirkung von blutdrucksenkenden Mitteln kann durch Dostinex verstärkt werden.
Dostinex sollte nicht gleichzeitig mit anderen Mutterkornpräparaten oder Substanzen, die die Wirkung von Dostinex abschwächen könnten, eingenommen werden. Dazu zählen gewisse Mittel gegen psychische Erkrankungen und Metoclopramid gegen Übelkeit und Störungen des Magen-Darm-Trakts. Arzneimittel wie z.B. die Antibiotika Erythromycin und Clarithromycin und die Pilzmittel Ketokonazol, Itraconazol, Voriconazol die den Abbau von Cabergolin hemmen können, dürfen nicht zusammen mit Dostinex eingenommen werden.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie
Dostinex soll grundsätzlich während der Schwangerschaft nicht eingenommen werden. Dostinex hebt eine bestehende Unfruchtbarkeit wieder auf. Da eine Schwangerschaft möglicherweise vor dem Wiederbeginn der Monatsblutung auftreten kann, ist ein Schwangerschaftstest mindestens alle 4 Wochen angezeigt. Frauen, die kein Kind wünschen oder aus medizinischen Gründen keines haben dürfen, sollen während der Behandlung mit Dostinex mechanische Verhütungsmittel (z.B. Kondome) anwenden. Wenn Sie eine Schwangerschaft wünschen, sollten Sie erst einen Monat nach dem Absetzen von Dostinex schwanger werden. Werden Sie während der Behandlung mit Dostinex schwanger, soll Dostinex nach Bestätigung der Schwangerschaft abgesetzt werden.
Dostinex soll in der Stillzeit, ausser zum Abstillen, nicht angewendet werden.
Nehmen Sie die Dostinex Tabletten jeweils mit einer Mahlzeit ein.
Dostinex soll innerhalb der ersten 24 Stunden nach der Geburt eingenommen werden. Die empfohlene Dosis beträgt 1 mg = 2 Tabletten. Sie sollten die 2 Tabletten auf einmal einnehmen.
Die Dosis beträgt ½ Tablette (0.25 mg) Dostinex alle 12 Stunden über zwei Tage, d.h. insgesamt 4x ½ Tablette.
Dostinex wird einmal oder zweimal wöchentlich eingenommen. Der Arzt bzw. die Ärztin wird Ihnen zu Beginn eine niedrige Dosis verschreiben. Je nach Bedarf wird dann schrittweise die Dosis erhöht, bis eine Normalisierung des Prolaktin-Blutspiegels erreicht ist und die damit verbundenen Beschwerden zurückgehen.
Die Anwendung von Dostinex bei Kindern, Jugendlichen und älteren Patientinnen und Patienten wurde nicht geprüft.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Folgende Nebenwirkungen können bei der Einnahme von Dostinex auftreten: Bei länger dauernder Einnahme von Dostinex kommt es bei 30% der Patienten und Patientinnen zu Übelkeit und Kopfschmerzen, bei 19% zu Benommenheit. Weniger häufig sind Schwindel, Verdauungsstörungen, Bauch- und Magenschmerzen, Erbrechen, Verstopfung, Brustschmerzen, Hitzewallungen, Depression, Halluzinationen, Verwirrtheit, Schwäche und Müdigkeit, Empfindungsstörungen in Armen und Beinen und Bewegungsstörungen.
Diese Nebenwirkungen verschwinden meistens trotz Fortsetzen der Behandlung. Da die unerwünschten Wirkungen dosisabhängig sind, sollten Sie Ihren Arzt bzw. Ihre Ärztin informieren, wenn bei Ihnen anhaltende unerwünschte Wirkungen auftreten, damit gegebenenfalls die Dosis reduziert werden kann.
Unter der Behandlung mit Dostinex können gelegentlich Blutdruckabfall, Ohnmacht, Gefäss- und Beinkrämpfe, Ödeme und Herzschmerzen (Angina pectoris) auftreten. Selten können Hautveränderungen mit Rötung, Schmerzen und Überwärmung auftreten. Sehr selten können Überempfindlichkeitsreaktionen, Haarausfall, Hautausschlag, Flüssigkeitsansammlungen zwischen Lunge und Brustwand bzw. im Herzbeutel, Entzündung des Herzbeutels, erschwerte Atmung bzw. Atemnot, Bindegewebszunahme in der Lunge und Herzklappenveränderungen vorkommen. Wenn Beschwerden wie Atemnot, anhaltender Hustenreiz oder Schmerzen in der Brust auftreten, informieren Sie unverzüglich Ihren Arzt bzw. Ihre Ärztin.
Seit der Markteinführung wurden zu dem folgende Nebenwirkungen beobachtet:
Erkrankungen des Brustfells mit Sympotomen wie z.B. Brustschmerzen und Atembeschwerden, Verdickung des Bindegewebes um die Nieren (Retroperitonealfibrose), aggressives Verhalten, Spielsucht, verstärkter Sexualtrieb, übermässiges Essen und Geldausgeben.
Informieren Sie Ihren Arzt, wenn eine dieser Verhaltensweisen bei Ihnen auftritt; er wird mit Ihnen Massnahmen zur Verhinderung oder Behebung der Symptome besprechen.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Dostinex nicht über 25 °C im Originalbehälter und ausser Reichweite von Kindern aufbewahren.
Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden.
Wenn Ihr Arzt bzw. Ihre Ärztin Ihnen sagt, dass Sie die Behandlung abbrechen sollen, sollten Sie alle nicht gebrauchten Dostinex Tabletten Ihrem Arzt bzw. Ihrer Ärztin oder Ihrer Apotheke zurückbringen.
