+-Was ist Diamox® und wann wird es angewendet?

Der Wirkstoff von Diamox, Acetazolamid, führt zu einer verstärkten Wasserausscheidung (Diurese).

Für das Auge bedeutet dies eine Senkung des Innendrucks. Diamox wird deshalb zur Glaukombehandlung (grüner Star) eingesetzt.

Bei Atembeschwerden führt Diamox zur verbesserten Ansprechbarkeit des Atemzentrums. Das Arzneimittel wird zur Prophylaxe der Höhenkrankheit eingesetzt.

Hirnödeme (Wasseransammlungen im Hirn) und bestimmte Epilepsieformen werden durch Diamox günstig beeinflusst.

In der Bauchspeicheldrüse wird die Sekretion durch das Arzneimittel vermindert.

Diamox darf nur auf Verschreibung des Arztes oder der Ärztin angewendet werden!

+-Was sollte dazu beachtet werden?

Während einer Langzeittherapie mit Diamox muss für eine ausreichende Zufuhr von Kalium gesorgt werden. Hierzu eignen sich Diätmassnahmen (Gemüse, Obst, vor allem getrocknete Aprikosen) oder eine zusätzliche Abgabe von Kaliumpräparaten. Hierüber entscheidet der Arzt oder die Ärztin.

+-Wann darf Diamox® nicht angewendet werden?

Diamox sollte nicht eingenommen werden:

  • bei bekannter Überempfindlichkeit gegenüber Sulfonamid, Sulfonamidderivaten (inkl. Acetazolamid) oder einem Inhaltsstoff von Diamox
  • bei schweren Lebererkrankungen;
  • wenn Sie an schwere Nierenbeschwerden leiden (oder gelitten haben);
  • wenn Sie an Nebennierenrindeninsuffizienz (auch als Addison-Krankheit bekannt) leiden;
  • bei erniedrigtem Natrium- und/oder Kaliumspiegel oder erhöhtem Chloridspiegel im Blut;

Die Anwendung und Sicherheit von Diamox bei Kindern und Jugendlichen ist bisher nicht geprüft worden.

Fragen Sie Ihren Arzt bzw. Ihre Ärztin um Rat, falls Sie von irgendeiner dieser Kontraindikationen betroffen sind.

Eine Diamox-Langzeitbehandlung ist nicht angezeigt bei Patienten und Patientinnen mit chronischem, nicht stark ausgeprägtem Glaukom, deren Tränenausführgänge verstopft sind.

+-Wann ist bei der Einnahme von Diamox® Vorsicht geboten?

Diamox kann Schläfrigkeit oder Verwirrung verursachen oder die Sehkraft beeinträchtigen, und damit die Reaktionsfähigkeit, die Fahrtüchtigkeit und die Fähigkeit, Werkzeuge oder Maschinen zu bedienen beeinträchtigen! Wenn Sie nach Einnahme von Diamox solche Nebenwirkungen empfinden, dann sollten Sie weder fahren noch Maschinen bedienen.

Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie

  • an Nierenbeschwerden wie Nierensteinen leiden oder gelitten haben;
  • an Diabetes oder an Störungen des Blutzuckerspiegels leiden;
  • älter als 65 Jahren sind;
  • einen Hautausschlag oder irgendein anderes allergisches Symptom an sich bemerken.

Wenige Personen, die mit Mitteln gegen Epilepsie, wie Acetazolamid, behandelt werden, hatten Selbstverletzungen- oder Suizidgedanken; hätten Sie solche Gedanke, dann müssen Sie Ihren Arzt bzw. Ihre Ärztin sofort kontaktieren.

Bei Patienten mit Lungenerkrankungen, wie Lungenemphysem oder chronischer Bronchitis, bei denen die Atmung eingeschränkt ist, sollte Diamox nur mit Vorsicht eingesetzt werden.

Die Wirkung von Diamox wird durch Probenecid und Sulfinpyrazon verstärkt.

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie andere Arzneimittel (auch selbstgekaufte!) einnehmen oder letztlich eingenommen haben! Es betrifft insbesondere folgende Arzneimittel, die die Wirkung von Diamox beeinflussen können:

  • Mittel für Ihr Herz, wie Herzglykoside (z.B. Digoxin)
  • Mittel zur Erniedrigung des Blutdrucks
  • Mittel zur Blutverdünnung (Antikoagulanzien)
  • Barbiturate
  • Mittel gegen Diabetes, um Ihr Blutzuckerspiegel zu erniedrigen (z.B. Metformin)
  • Mittel gegen Epilepsie oder Anfälle (insbesondere Phenytoin, Primidon, Carbamazepin oder Topiramat)
  • Methotrexat
  • Aspirin (Acetylsalicylsäure) und verwandte Arzneimittel, z.B. Salicylsäure oder Cholinsalicylat für Mundkrankheiten
  • Benannte Carboanhydrasehemmer (zur Behandlung von erhöhten Augeninnendruck angewendet)
  • Amphetamine (stimulierend), Chinidin (behandelt unregelmässigen Herzschlag; in der Schweiz nicht zugelassen) oder Lithium (zur Behandlung schwerer Geistesbeschwerden)
  • Ciclosporin (wird nach Transplantationen zur Immunsuppression angewendet)

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder anderen Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden!

+-Darf Diamox® während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Es gibt keine kontrollierten Studien mit Diamox während der Schwangerschaft oder Stillzeit, deshalb sollte Diamox während der Schwangerschaft (vor allem in den ersten 3 Monaten), wenn Sie denken, schwanger zu sein oder schwanger werden möchten, nicht eingenommen werden, es sei denn, dass der Arzt oder die Ärztin dies ausdrücklich verordnet.

Acetazolamid, der Wirkstoff von Diamox, tritt in die Muttermilch über. Während der Stillzeit müssen Sie Ihren Arzt bzw. Ihre Ärztin um Rat fragen, ob Sie Diamox einnehmen dürfen.

+-Wie verwenden Sie Diamox®?

Diamox wird je nach Anwendungsgebiet in verschiedenen Dosierungen vom Arzt oder von der Ärztin verschrieben.

Erwachsene:

Übliche Dosierung, falls nicht anders vom Arzt oder von der Ärztin verordnet:

Glaukom

1. In akuten Fällen: 250-1000 mg (1-4 Tabletten) über 24 h verteilt
2. Dauermedikation: 125-250 mg (½-1 Tabl.) täglich

Atembeschwerden

500-750 mg (2-3 Tabletten) täglich

Epilepsie

250-1000 mg (1-4 Tabletten) über 24 h verteilt

Ödeme

Anfangs 250-375 mg (1-1½ Tabletten) täglich, anschliessend 250-375 mg (1-1½ Tabletten) wöchentlich

Prophylaxe der Höhenkrankheit

Ca. 500 mg (2 Tabl.) täglich (Beginn: 1 Tag vor Aufbruch)

Die Anwendung und Sicherheit von Diamox bei Kindern und Jugendlichen ist bisher nicht geprüft worden.

Falls Sie mehr Diamox als vorgesehen eingenommen haben, sollten Sie sofort ärztliche Hilfe in Anspruch nehmen, indem Sie entweder Ihren Arzt bzw. Ihre Ärztin anrufen oder zum nächstgelegenen Spital (Notfallabteilung) gehen. Nehmen Sie die übrigbleibenden Tabletten und diese Packungsbeilage mit, so wird der Arzt wissen, was Sie eingenommen haben.

Beim Vergessen einer Dosis sollten sie diese einnehmen, sobald Sie sich daran erinnern. Falls aber nur weniger als 2 Stunden bis nächster Einnahme bleiben, dann müssen Sie die vergessene Dosis auslassen. Nehmen Sie nie doppelte Dosen auf einmal.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

+-Welche Nebenwirkungen kann Diamox® haben?

Folgende Nebenwirkungen können bei der Anwendung von Diamox auftreten:

Sehr selten (weniger als 1 Person aus 10?000 betroffen):

  • Allergische Reaktionen. Beim Auftreten von schwerwiegenden allergischen Reaktionen wie plötzlichen Atembeschwerden, Schwellung der Augenlider, des Gesichtes oder der Lippen, Hautausschlag oder Jucken (insbesondere wenn der ganze Körper betroffen ist), suchen Sie sofort Ihren Arzt, bzw. Ihre Ärztin auf.
  • Hautausschläge. Wenn schwerwiegende Hautsymptome wie Bläschenbildung oder Schälen der Haut um Lippen, Augen, Mund, Nasen und Genitalien herum, grippeähnliche Symptome und Fieber, Muskel- und Gelenkschmerzen, allgemeines Unwohlsein auftreten, brechen Sie die Behandlung ab und suchen Sie sofort Ihren Arzt bzw. Ihre Ärztin auf.
  • Lichtempfindlichkeit
  • Blutbildveränderungen
  • Flüssigkeitsansammlung in Lungengeweben (Lungenödem), einschliesslich Atembeschwerden, Husten mit blutig-schaumigem Auswurf und bleiche Haut
  • Leberfunktionsstörungen, Lebernekrose (massiver Zerfall der Leberzellen)
  • Ausscheidung von Blut oder Glukose im Harn

Selten (weniger als 1 Person aus 1000 betroffen):

  • Hepatitis (Lebervergrösserung, Leberentzündung); Gelbsucht (gelbe Haut und Augen, dunkelfarbiger Harn, heller Stuhl)

Gelegentlich (weniger als 1 Person aus 100 betroffen):

  • Depression und Verwirrtheit, Erregbarkeit, Muskelschwächen und-Lähmungen, Krampfanfälle, Störungen der Koordination von Bewegungsabläufen (Ataxie)
  • Kurzsichtigkeit
  • Ohrengeräusche (Tinnitus) und Hörstörungen
  • Blutdrucksenkung
  • Krampfartige Beschwerden in Händen und Füssen
  • Wachstumsverzögerung bei Kindern
  • Vermehrter Harndrang, Nierenversagen
  • Durst, Mundtrockenheit

Häufig (weniger als 1 Person von 10 betroffen):

  • Taubheitsgefühl und Kribbeln an Haut, Fingern oder Zehen, Hitzegefühl, Leistungsabfall, Schwindel, Kopfschmerzen
  • Übelkeit, Erbrechen, Durchfall, Appetitlosigkeit oder Geschmacksstörungen, Müdigkeit, mögliche schwarze oder Teerstühle

Häufigkeit unbekannt:

  • Azidose (Übersäuerung des Blutes); Symptome sind u.a. Übelkeit, Erbrechen, Herzklopfen oder Kopfweh. Sie müssen Ihren Arzt bzw. Ihre Ärztin oder einen Spital sofort kontaktieren.
  • Rote, weiche, warme, geschwollene Gelenke (Gichtanfälle)
  • Nieren-und Harnrährenkolik
  • Schmerzen, oft im Form Anfällen am Seiten- und unteren Rücken, wegen Nierensteinen
  • Hautrötung, verminderter Sexualtrieb, Fieber

Ausserdem kann Diamox Nebenwirkungen auslösen, die Sie normalerweise nicht bemerken werden, wie Veränderungen in Elektrolyten (Salze). Ihr Blutzuckerspiegel kann verändert werden. Sprechen Sie mit Ihrem Arzt bzw. mit Ihrer Ärztin. Bei einer Langzeitbehandlung mit diesem Arzneimittel sollte Ihr Blut regelmässig kontrolliert werden.

Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

+-Was ist ferner zu beachten?

Diamox kann gewisse Labortests beeinflussen. Falls Sie einen Spital oder eine Klinik für Labortesten besuchen, müssen Sie den betroffenen Arzt informieren, dass Sie Diamox einnehmen.

Diamox ist ausserhalb der Reichweite von Kindern, bei Raumtemperatur (15-25 °C) aufzubewahren.

Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden.
