+-Was ist Clopixol und wann wird es angewendet?

Clopixol enthält den Wirkstoff Zuclopenthixol und gehört zur Präparategruppe der so genannten Neuroleptika. Clopixol wird zur Behandlung von psychischen Krankheiten angewendet, bei denen das Denken, Fühlen und / oder Handeln beeinträchtigt ist. Typische Zeichen sind Verwirrungszustände, Halluzinationen (Hören oder Sehen von Dingen, die nicht wirklich da sind) und Wahnvorstellungen. Die Patienten bzw. Patientinnen können ausserdem ängstlich, angespannt oder aggressiv sein. Clopixol wirkt beruhigend und löst Angstzustände sowie innere Spannungsgefühle. Clopixol darf nur auf Verschreibung des Arztes oder der Ärztin verwendet werden.

+-Wann darf Clopixol nicht angewendet werden?

Bei einer Überempfindlichkeit gegen den Wirkstoff oder einen Hilfsstoff von Clopixol, oder einen verwandten Wirkstoff, bei Alkohol-, Schlafmittel- und Opiatvergiftungen, darf Clopixol nicht verwendet werden.

+-Wann ist bei der Einnahme von Clopixol Vorsicht geboten?

Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fähigkeit Werkzeuge oder Maschinen zu bedienen und die Fahrtüchtigkeit beeinträchtigen!

Clopixol kann die Konzentrationsfähigkeit herabsetzen und die Wirkung von Alkohol verstärken, daher ist ein Verzicht auf Alkohol während der Behandlung angezeigt.

Wenn es während der Behandlung zu Müdigkeit, Appetitlosigkeit, Übelkeit, Juckreiz, einem Hautausschlag oder Gelbverfärbung der Haut kommt, müssen Sie sofort den Arzt bzw. die Ärztin benachrichtigen.

Diese Zeichen können die Folge einer Leberentzündung sein, die gelegentlich durch Clopixol hervorgerufen wird (so genannte medikamentöse Hepatitis). Sie erfordern eine Abklärung durch den Arzt bzw. die Ärztin.

Bei Patientinnen und Patienten mit Leber- oder Nierenfunktionsstörungen, mit Herz-Kreislauferkrankungen (v. a. zu tiefem Blutdruck oder Störungen der Herzschlagfolge), mit Myasthenia gravis (krankhafte Muskelschwäche mit gesteigerter Ermüdbarkeit der Willkürmuskulatur), mit Parkinson-Krankheit, mit Epilepsie oder organischen Hirnerkrankungen, mit Risikofaktoren für einen Schlaganfall (z.B. Rauchen, Bluthochdruck), mit Diabetes (die antidiabetische Therapie muss gegebenenfalls angepasst werden), mit zu tiefen Kalium- oder Magnesium-Werten im Blut oder einer genetischen Veranlagung für zu tiefe Werte soll Clopixol mit Vorsicht verwendet werden.

Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie oder jemand in Ihrer Familie an Blutgerinnsel leiden.

Clopixol kann die Wirkung anderer Arzneimittel, die auf das Nervensystem dämpfend wirken (z.B. Schlafmittel, Beruhigungsmittel, starke Schmerzmittel, Mittel gegen Depression), verstärken. Vermeiden Sie den gleichzeitigen Gebrauch von anderen Neuroleptika. Die Wirkung von Arzneimitteln gegen zu hohen oder zu tiefen Blutdruck kann verändert, die Wirkung von Arzneimitteln gegen Parkinson-Krankheit vermindert werden. In diesen Fällen entscheidet der Arzt bzw. die Ärztin, welche Arzneimittel zusammen mit Clopixol eingenommen werden dürfen.

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie

  • an anderen Krankheiten leiden,
  • Allergien haben oder
  • andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden!

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie eines der folgenden Arzneimittel nehmen:

  • Trizyklische Antidepressiva
  • Lithium
  • Blutdrucksenkende Arzneimittel
  • Zentraldämpfende Arzneimittel und Barbiturate (Arzneimittel, welche Sie schläfrig machen)
  • Antiepileptika
  • Levodopa und ähnliche Arzneimittel zur Behandlung von Parkinson
  • Metoclopramid gegen Magen-Darm-Beschwerden
  • Einige Antibiotika
  • Arzneimittel, welche das Wasser- oder Salz-Gleichgewicht beeinflussen (zu tiefe Kalium- oder Magnesium-Werte im Blut)
  • Arzneimittel, welche die Clopixol-Konzentration im Blut erhöhen können
  • Adrenergika (Arzneimittel, die ähnliche Wirkungen wie Adrenalin haben)
  • Antihistaminika (Behandlung von allergischen Erkrankungen / Reaktionen, Juckreiz)

Folgende Arzneimittel sollten nicht gleichzeitig mit Clopixol eingenommen werden:

  • Arzneimittel, die die Herzfrequenz verändern
  • Andere Neuroleptika

Es besteht die Möglichkeit, dass die Sexualität bzw. Fruchtbarkeit durch eine Behandlung mit Clopixol beeinträchtigt wird (siehe «Welche Nebenwirkungen kann Clopixol haben?»). Bitte holen Sie ärztlichen Rat ein.

+-Darf Clopixol während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Teilen Sie Ihrem Arzt bzw. Ihrer Ärztin mit, wenn Sie schwanger sind, vermuten schwanger zu sein, eine Schwangerschaft planen oder stillen.

Bei Anwendung von Clopixol während des letzten Drittels der Schwangerschaft können bei Neugeborenen Zittern, Muskelsteifigkeit, Schwäche, Schläfrigkeit, Erregung, Schwierigkeiten beim Atmen oder Probleme beim Füttern auftreten.

Bitte informieren Sie Ihren Arzt bzw. Ihre Ärztin unverzüglich im Falle einer Schwangerschaft, um das weitere Vorgehen zu besprechen. Die Behandlung soll keinesfalls aus eigenem Ermessen abgesetzt werden. Ein plötzliches Absetzen von Clopixol kann schwerwiegende Folgen haben.

Bitte informieren Sie auch unverzüglich Ihren Frauenarzt oder Geburtshelfer bzw. Ihre Frauenärztin oder Geburtshelferin über eine Einnahme während der Schwangerschaft, insbesondere wenn beschriebene Auffälligkeiten bei Ihrem Kind nach der Geburt auftreten.

Clopixol soll während der Schwangerschaft nicht eingenommen werden, es sei denn, Ihr Arzt bzw. Ihre Ärztin verordnet es ausdrücklich.

In der Stillzeit sollte Clopixol nicht eingenommen werden. Bei einer notwendigen Therapie sollte daher nicht gestillt werden.

+-Wie verwenden Sie Clopixol?

Clopixol ist ein hochwirksames Arzneimittel. Der Arzt bzw. die Ärztin stimmt die Dosierung für jeden Patienten persönlich ab und beurteilt, wie lange die Behandlung dauern soll. Die Anordnung des Arztes bzw. der Ärztin muss streng beachtet werden. Die Dosierung von Clopixol hängt von der Art und der Schwere des Leidens ab. Am Anfang der Therapie wird die optimale Dosis durch eine stufenweise Erhöhung der Dosis (alle 2-3 Tage) ausfindig gemacht. In der Regel wird Clopixol in mehreren Gaben pro Tag eingenommen.

Clopixol Tropfen (20 mg/ml) sollten nur mit Wasser, Orangensaft oder Apfelsaft gemischt werden.

In akuten Fällen kann Ihr Arzt bzw. Ihre Ärztin die Behandlung mit Clopixol Acutard (Injektionslösung) einleiten. Für die langfristige Therapie kann Ihr Arzt bzw. Ihre Ärztin auf Clopixol Depot (Injektionslösung, jede 2.-4. Woche verabreicht) übergehen.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin.

Clopixol soll nicht in der Behandlung von Patientinnen bzw. Patienten unter 18 Jahren eingesetzt werden.

+-Welche Nebenwirkungen kann Clopixol haben?

Falls Sie eines der folgenden Symptome feststellen, sollten Sie sofort Ihren Arzt bzw. Ihre Ärztin kontaktieren oder das Spital aufsuchen.

Gelegentlich:

  • Ungewöhnliche Bewegungen von Mund und Zunge; dies könnte ein frühes Anzeichen einer Bewegungsstörung (Spätdyskinesie) sein.

Sehr selten:

  • Hohes Fieber, ungewöhnliche Muskelsteifheit und Bewusstseinsstörungen, wenn sie mit Schwitzen und schnellem Herzschlag auftreten; dies könnten Anzeichen eines seltenen, schwerwiegenden Krankheitsbildes (Malignes Neuroleptisches Syndrom) sein.
  • Eine Gelbverfärbung der Haut und des Auges könnte bedeuten, dass Ihre Leber betroffen ist und ein Anzeichen einer Gelbsucht sein.

Die folgenden Nebenwirkungen sind zu Beginn der Behandlung am stärksten und klingen meistens mit fortdauernder Behandlung ab:

Sehr häufig:

  • Schläfrigkeit, Unfähigkeit still zu sitzen oder bewegungslos zu bleiben, unwillkürliche Bewegungen, langsame oder verminderte Bewegungen
  • Mundtrockenheit

Häufig:

  • Herzrasen, schneller, starker oder unregelmässiger Herzschlag
  • Zittern, wiederholte Drehbewegungen der Gliedmassen, wiederholte Bewegungen oder anormale Körperhaltung aufgrund anhaltender Muskelspannung, verstärkte Muskelsteifheit, Schwindel, Kopfschmerzen, kribbelndes, stechendes oder taubes Gefühl der Haut, Beeinträchtigung der Aufmerksamkeit, Erinnerungsverlust, abnormaler Gang
  • Schwierigkeiten beim Fokussieren des Blickes, Sehstörungen
  • Schwindelgefühle
  • Verstopfte Nase, erschwerte oder schmerzhafte Atmung
  • Vermehrter Speichelfluss, Verstopfung, Erbrechen, Verdauungsprobleme oder Beschwerden im Oberbauch, Durchfall
  • Schwierigkeiten beim Wasserlassen, Harnverhalten, erhöhtes Harnvolumen
  • Verstärktes Schwitzen, Juckreiz
  • Muskelschmerzen
  • Verstärkter Appetit, Gewichtszunahme
  • Müdigkeit, Schwäche, Unbehaglichkeit, Schmerzen
  • Schlaflosigkeit, Depression, Angstzustände, Nervosität, abnormale Träume, Erregung, verminderte Libido

Gelegentlich:

  • Überaktive Reflexe, ruckartige Bewegungen, Parkinsonismus, Ohnmacht, Schwierigkeiten bei der Koordination von Bewegungen, Sprechstörungen, verringerte Muskelspannung, Krämpfe, Migräne
  • Verdrehen der Augen, Pupillenerweiterung
  • Überempfindlichkeit gegenüber gewissen Tonfrequenzen oder Alltagsgeräuschen, Tinnitus (Ohrgeräusche)
  • Bauchschmerzen, Übelkeit, Blähungen
  • Hautausschlag, Empfindlichkeit der Haut auf Licht, Pigmentierungsstörungen, fettige, glänzende und gelbliche Haut aufgrund einer erhöhten Talgproduktion, Ekzem oder Hautentzündungen, Unterhautblutungen mit roten oder violetten Hautverfärbungen
  • Muskelsteifheit, Schwierigkeiten den Mund normal zu öffnen, abnorme Kopfstellung
  • Verminderter Appetit, Gewichtsabnahme
  • Tiefer Blutdruck, Blutdruckabfall bei Lagewechsel (z.B. schnelles Aufstehen), Wallungen
  • Durst, tiefe Körpertemperatur, Fieber
  • Veränderungen der Leber
  • Sexuelle Störungen (verzögerte Ejakulation, Erektionsstörungen, Orgasmusschwierigkeiten bei Frauen, trockene Vagina)
  • Ausgeprägte Gleichgültigkeit gegenüber der Umgebung, Albträume, verstärkte Libido, Verwirrungszustände

Selten:

  • Blutbildveränderungen (verminderte Zahl der weissen Blutkörperchen und Blutplättchen)
  • Erhöhte Blutspiegel des Hormons Prolaktin
  • Hoher Blutzuckerwert, beeinträchtigte Zuckerverträglichkeit, erhöhte Blutfettwerte
  • Überempfindlichkeit, akute allergische Reaktionen
  • Vergrösserung der männlichen Brustdrüsen, Milchfluss, Menstruationsstörungen, andauernde, schmerzhafte Erektion ohne sexuelle Erregung oder Verlangen
  • Langsamer Herzschlag und Veränderung im EKG
  • Unregelmässiger Herzschlag

In seltenen Fällen könnte eine Herzrhythmusstörung zu einem plötzlichen Tod führen.

Blutgerinnsel in den Venen, insbesondere in den Beinen (die Symptome umfassen Schwellung, Schmerzen und Rötung des Beines), welche durch die Blutbahnen in die Lungen wandern, können Brustschmerzen und Atemschwierigkeiten auslösen. Wenn Sie eines dieser Symptome bemerken, ziehen Sie umgehend medizinischen Rat bei.

Es wurde bei älteren Personen mit Demenz, die Neuroleptika einnehmen im Vergleich zu solchen, die keine Neuroleptika erhalten, ein kleiner Anstieg von Todesfällen verzeichnet.

Bei Anwendung von Clopixol während des letzten Drittels der Schwangerschaft können bei Neugeborenen Zittern, Muskelsteifigkeit, Schwäche, Schläfrigkeit, Erregung, Schwierigkeiten beim Atmen oder Probleme beim Füttern auftreten.

Falls die Nebenwirkungen stärker werden oder wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

+-Was ist ferner zu beachten?

Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden. Bitte bringen Sie das nicht verwendete Arzneimittel oder das Arzneimittel, dessen Verfalldatum abgelaufen ist, Ihrer Apotheke zur Entsorgung zurück.

Die Filmdragées bei Raumtemperatur (15-25 °C) lagern.

Die Tropfen im Kühlschrank (2-8ºC) lagern. Clopixol Tropfen sind nach dem erstmaligen Öffnen der Flasche 6 Wochen haltbar, wenn sie unterhalb von 25 °C aufbewahrt werden. Um den Inhalt vor Licht zu schützen, sollte die Flasche in der Originalverpackung gelagert werden.

Clopixol Tropfen enthalten 14% Vol Alkohol.

Ausser Reichweite von Kindern lagern.

+-Was ist in Clopixol enthalten?

1 Filmdragée enthält 2 mg, 10 mg bzw. 25 mg Zuclopenthixol als Zuclopenthixoldihydrochlorid. Hilfsstoffe: Kartoffelstärke, Laktose, mikrokristalline Cellulose, Copovidon, Glycerin, Talk, Rizinusöl, Magnesiumstearat. Filmüberzug: Hypromellose, Macrogol, Color: Titandioxid (E171), Eisenoxid (E172).

Aussehen der Dragées:

Die Filmdragées sind rund, bikonvex und von unterschiedlicher Farbe: 2 mg blass rot/10 mg hell rotbraun/25 mg rotbraun.

1 ml der Tropfen enthält 20 mg Zuclopenthixol als Zuclopenthixoldihydrochlorid, Alkohol (14.2 Vol % - 120 mg/ml), gereinigtes Wasser. 20 Tropfen = 1 ml.

Aussehen der Tropfen:

Klare, fast farblose bis gelbliche, geruchlose  Lösung mit einem leicht bitteren Geschmack.
