Nach operativen Eingriffen oder bei Bettlägerigkeit besteht die Gefahr der Bildung von Blutgerinnseln (Thrombosen) insbesondere der Beine. Diese Blutgerinnsel können über den Kreislauf in die Lungen gelangen, wo sie Blutgefässe verschliessen (Lungenembolien). Dies äussert sich unter anderem in Brustschmerzen und Atemnot. Eine Lungenembolie ist lebensbedrohend. Clexane wird auf Verschreibung des Arztes bzw. der Ärztin zur Vorbeugung gegen die Entstehung von Thrombosen angewendet. Enoxaparin, der Wirkstoff von Clexane, ist ein niedermolekulares Heparin, welches den Vorgang der Blutgerinnung bremst und so der Bildung von Blutgerinnseln entgegenwirkt. Das Arzneimittel wird mittels Spritze und Nadel unter die Haut verabreicht.
Während des Spitalaufenthaltes haben Sie regelmässig Injektionen zur Vorbeugung gegen Thrombosen verabreicht erhalten. Die Fortsetzung dieser medikamentösen Behandlung ist auch nach Entlassung aus dem Spital noch während einer gewissen Zeitdauer notwendig.
Clexane darf nicht angewendet werden bei einer bekannten oder vermuteten Überempfindlichkeit (Allergie) gegen Enoxaparin, bei Blutabnormalitäten sowie bei gewissen Krankheitszuständen, deren Abwesenheit der Arzt vor der ersten Verabreichung von Clexane geprüft hat, wie gewissen entzündlichen Herzkrankheiten und akutem Magen-Darm-Geschwür.
Vorsicht ist geboten bei gewissen Lebererkrankungen und gewissen Netzhauterkrankungen.
Halten Sie sich strikte an die Anweisungen Ihres Arztes bzw. Ihrer Ärztin. Falsche Dosierung oder gleichzeitige Einnahme von anderen Medikamenten könnte in seltenen Fällen ungünstige Auswirkungen auf die Blutgerinnung haben.
Informieren Sie deshalb Ihren Arzt bzw. Ihrer Ärztin, wenn Sie andere Medikamente anwenden. Bei Anwendung von Clexane in Kombination mit gewissen anderen Medikamenten (Schmerzmittel, Entzündungshemmer und Medikamente gegen die Blutgerinnung) ist Vorsicht geboten. Falls die Kombination von Clexane mit einem dieser Medikamente erforderlich ist, wird Sie Ihr Arzt sorgfältig überwachen.

Clexane darf ausschliesslich auf ausdrückliche Verschreibung Ihres Arztes angewendet werden, da bisher nur begrenzte Erfahrungen mit einer Anwendung von Enoxaparin an Schwangeren vorliegen.
Unter der Geburt ist die peridurale Anästhesie bei Frauen, die mit Antikoagulanzien behandelt werden, kontraindiziert.
Clexane darf zudem nicht im Fall einer bevorstehenden Fehlgeburt angewendet werden.
Zur Anwendung von Enoxaparin zur Thromboseprophylaxe bei Schwangeren mit künstlichen Herzklappen liegen nur unzureichende Erkenntnisse vor. In einer klinischen Studie an Schwangeren mit künstlichen Herzklappen, die zweimal täglich 1 mg Enoxaparin-Natrium pro kg Körpergewicht zur Prävention thromboembolischer Ereignisse erhalten hatten, traten bei zwei von acht Frauen Thromben auf, die zur Blockade der Herzklappe und zum Tod von Mutter und Kind führten. Daher sollte der Arzt vor der Verabreichung von Clexane bei schwangeren Frauen mit künstlicher Herzklappe das Risiko-Nutzen-Verhältnis sorgfältig abwägen.
Es liegen keine ausreichenden Untersuchungen zur Frage eines eventuellen U?berganges von Enoxaparin in die Muttermilch vor, jedoch erscheint ein gerinnungshemmender Effekt auf den Sa?ugling unwahrscheinlich.
Clexane muss strikte gemäss den Anweisungen des Arztes angewendet werden. Ihr Arzt bzw. Ihre Ärztin setzt die Dosierung und die Behandlungsdauer fest. Nur bei genauer Einhaltung dieser Vorschriften kann der grösstmögliche Nutzen von Clexane erreicht werden. Brechen Sie die Therapie nicht vorzeitig ab, selbst wenn Sie sich besser fühlen, denn oft klingen die Krankheitszeichen vor der vollständigen Heilung ab. Eine ungenügende Anwendungsdauer oder ein zu frühes Beenden der Behandlung kann das Risiko der Bildung einer Thrombose erhöhen.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ärztin oder Apothekerin.
In der Regel wird Clexane als einmal tägliche Injektion von 40 mg Enoxaparin subkutan (unter die Haut) verabreicht. Die Injektion sollte stets zur gleichen Tageszeit erfolgen. Die Behandlung wird nach Spitalentlassung über eine Zeitdauer von ca. 3 Wochen fortgesetzt.
Über die Verabreichung der Injektion werden Sie vom Spitalpersonal genauestens instruiert.
Zur Vermeidung versehentlicher Nadelstiche nach der Injektion sind die Fertigspritzen mit einem automatischen Sicherheitssystem ausgestattet.
Als Injektionsort eignet sich die Haut der vorderen seitlichen Bauchwand sowie die Haut an der Aussenseite des Oberschenkels. Zur besseren Verträglichkeit der Injektionen wird empfohlen, von einer Injektion zur nächsten abwechselnd die rechte und die linke Seite zu verwenden.
Desinfizieren Sie die Einstichstelle mittels eines Tupfers, der mit einem Desinfektionsmittel getränkt ist.
Abb. 1: Nehmen Sie die Spritze aus der Verpackung und ziehen Sie die Verschlusskappe ab. Vermeiden Sie jeglichen Kontakt mit der Nadel, damit diese steril bleibt. Falls sich an der Nadelspitze ein Tropfen gebildet hat, darf dieser nicht abgestreift werden. Schütteln Sie ihn ab, indem Sie mit nach unten gerichteter Nadel leicht gegen die Spritze klopfen. Lassen Sie die Luftblase in der Spritze. Es kommt vor, dass sich die Luftblase nicht am Spritzenkolben befindet. In diesem Fall klopfen Sie mit nach unten gerichteter Nadel leicht gegen die Spritze, damit die Blase bis zum Spritzenkolben aufsteigt.
Wird (auf Anweisung des Arztes) nur eine Teilmenge verabreicht, bestimmen Sie mithilfe der Masseinteilung die erforderliche Menge und lassen Sie mit nach unten gerichteter Nadel vorsichtig die überschüssige Flüssigkeit ab.
Abb. 2: Bilden Sie an der desinfizierten Körperstelle mit dem Daumen und Zeigefinger eine Hautfalte. Halten Sie mit einer Hand die Hautfalte gut fest. Mit der anderen Hand stechen Sie die Spritzenadel senkrecht in die Haut und führen Sie sie in der ganzen Länge ein. Die Nadellänge ist so berechnet, dass die Injektionsflüssigkeit genau in die gewünschte subkutane Hautregion verabreicht wird. Drücken Sie langsam auf den Spritzenkolben und injizieren Sie die gesamte Flüssigkeitsmenge.
Abb. 3: Das Sicherheitssystem aktiviert sich während der Injektion, wenn der Spritzenkolben den Anschlag erreicht und schützt auf diese Weise die verunreinigte Nadel, ohne den Patienten zu stören. Um das Sicherheitssystem auszulösen, ist es notwendig, den Spritzenkolben voll durchzudrücken.
Hinweis: Das Sicherheitssystem kann erst aktiviert werden, wenn die Spritze vollkommen entleert ist.
Ziehen Sie danach die Nadel vorsichtig wieder heraus und lassen Sie erst dann die Hautfalte wieder los. Reiben oder massieren Sie die Injektionsstelle nicht.
Folgende Nebenwirkungen können bei der Anwendung von Clexane auftreten:
An der Injektionsstelle kann sich nach der Verabreichung ein kleiner Bluterguss («blauer Fleck», Hämatom) bilden; dies ist bei der Injektion solcher Wirkstoffe nichts aussergewöhnliches und völlig harmlos.
Selten können Blutungszwischenfälle auftreten, in der Regel nur bei Vorhandensein von Verletzungen oder bei gleichzeitiger Verabreichung gewisser anderer Medikamente.
Über das Auftreten von allergischen Reaktionen (Hautausschlag lokal oder über den ganzen Körper, Gesichtsschwellung, Anaphylaktischer Schock) wurde berichtet.
Es wurden auch häufig Kopfschmerzen, Leberschäden, ein Osteoporose Risiko bei längerer Behandlung und Beschleunigung von Haarausfall beobachtet.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Haltbarkeit: Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden.
Aufbewahrung/Lagerung: Bewahren Sie Clexane in der geschlossenen Originalpackung ausserhalb der Reichweite von Kindern und bei Raumtemperatur (15-25 C) auf. Nicht einfrieren.
Nach Beendigung der Behandlung das Arzneimittel mit dem restlichen Inhalt Ihrer Abgabestelle (Arzt/Apotheker) zum fachgerechten Entsorgen bringen.
