+-Was ist Bilol comp. und wann wird es angewendet?

Bilol comp. ist eine Kombination von zwei Wirkstoffen, die sich gegenseitig ergänzen. Bilol comp. senkt den erhöhten Blutdruck (Hypertonie).

Bisoprolol senkt den erhöhten Blutdruck und vermindert bei einer eventuell bestehenden koronaren Herzkrankheit (Angina pectoris) die Häufigkeit und Schwere der Schmerzanfälle. Gleichzeitig werden vom Nervensystem kommende Reize gedämpft, wodurch Überreaktionen des Herzens, wie Pulsjagen und Herzklopfen, vermieden werden.

Hydrochlorothiazid regt die Harnausscheidung an, wodurch unter anderem der Natriumchlorid- und Wassergehalt im Körper vermindert und der Blutdruck gesenkt werden.

Bilol comp. darf nur auf Verschreibung des Arztes oder der Ärztin angewendet werden.

+-Was sollte dazu beachtet werden?

Sie können den Behandlungserfolg in vielen Fällen deutlich unterstützen, wenn Sie Ihre Bewegungs- und Essgewohnheiten nach den Empfehlungen Ihres Arztes ausrichten.

Das Auftreten von Nebenwirkungen (z.B. stark verlangsamter Herzschlag, zu niedriger Blutdruck oder andere Krankheitszeichen wie zunehmende Atemnot, die auf eine Verschlechterung der Herzschwäche hinweisen) kann zum Abbruch der Behandlung mit Bilol comp. führen.

Während der Therapie mit Bilol comp. sollte eine regelmässige Kontrolle des Salz- und Wasserhaushaltes (insbesondere Kalium, Natrium und Kalzium), sowie der Blutfette erfolgen. Patienten, die zuckerkrank sind, sollten regelmässig, speziell zu Behandlungsbeginn, ihren Blutzucker kontrollieren, weil die Wirkung der blutzuckersenkenden Arzneimittel durch Bilol comp. verstärkt werden kann. Bei Patienten, die an Gicht leiden, muss regelmässig eine Kontrolle der Harnsäure erfolgen.

+-Wann darf Bilol comp. nicht eingenommen werden?

Bilol comp. darf nicht eingenommen werden bei bestimmten Herzfunktionsstörungen (z.B. AV-Block 2. oder 3. Grades ohne Herzschrittmacher, Sinusknoten-Syndrom, sinuatrialer Block), bei akuter Herzmuskelschwäche, bei plötzlicher Verschlechterung einer Herzmuskelschwäche, bei stark verlangsamtem und unregelmässigem Herzschlag vor Therapiebeginn (Puls unter 50 Schlägen/Minute), bei schweren Formen von peripheren Durchblutungsstörungen (z.B. der Beine und Hände) infolge Arterienverengung, bei Schockzuständen (mit tiefem Blutdruck und Kreislaufversagen einhergehend), bei unbehandelten Tumoren des Nebennierenmarks (Phäochromozytom), bei gewissen Störungen des Stoffwechsels (metabolische Azidose), bei schweren Störungen des Salz- und Wasserhaushalts (z.B. therapieresistenter Kaliummangel), bei schweren Leber- und Nierenfunktionsstörungen, bei schwerem Asthma, bei Gicht und bei schwangerschaftsbedingtem Bluthochdruck.

Bei bekannter Überempfindlichkeit auf Bisoprolol, Hydrochlorothiazid, einen Hilfsstoff von Bilol comp., andere Thiazide oder Sulfonamide soll Bilol comp. nicht angewendet werden.

+-Wann ist bei der Einnahme von Bilol comp. Vorsicht geboten?

Bilol comp. darf nur mit besonderer Vorsicht eingenommen werden bei gleichzeitiger Herzmuskelschwäche (Herzinsuffizienz), gewissen Störungen der Herzfunktion (z.B. AV-Block 1. Grades), gewissen Durchblutungsstörungen des Herzens (z.B. Prinzmetal-Angina), Leberfunktionsstörungen, peripheren, arteriellen Durchblutungsstörungen, Schuppenflechte (Psoriasis) sowie bei strengem Fasten. Bei gleichzeitiger Desensibilisierungstherapie wegen Allergie/Heuschnupfen ist Vorsicht geboten.
Beim Auftreten von akut verminderter Sehschärfe oder Augenschmerz (innerhalb von Stunden bis Wochen nach Beginn der Therapie) kontaktieren Sie bitte umgehend Ihren Arzt bzw. Ihre Ärztin. Er bzw. sie wird Ihnen gegebenenfalls empfehlen die Behandlung abzusetzen.
Beim Auftreten von Hautreaktionen nach Sonnenlichtbestrahlung wird empfohlen, die exponierten Hautstellen entsprechend zu schützen. In schweren Fällen sollte die Therapie abgebrochen werden.
Bei bestimmten Tumoren des Nebennierenmarks (Phäochromozytom) darf Bilol comp. erst nach vorheriger Gabe von Alpha-Rezeptoren-Blockern angewendet werden.
Bei schweren Atemwegserkrankungen (mit Verengung der Atemwege, welche Symptome wie Atemnot verursachen können, z.B. COPD).
Bei Asthma kann eine Dosisanpassung von bronchienerweiternden Arzneimitteln notwendig werden.
Bei Diabetikern mit stark schwankenden Blutzuckerwerten, bei der Kombination mit oralen Antidiabetika oder Insulin ist Vorsicht geboten. Bilol comp. kann die Symptome eines stark erniedrigten Blutzuckerspiegels (Hypoglykämie), unter anderem den erhöhten Herzschlag, verschleiern oder vermindern.
Die Symptome einer Schilddrüsenüberfunktion (Thyreotoxikose) können durch Bilol comp. verdeckt werden.
Eine Langzeitanwendung von Bilol comp. kann zu einer Kalzium-Anreicherung (Hyperkalzämie) im Blut führen.

Wechselwirkungen mit anderen Arzneimitteln

Zahlreiche andere Arzneimittel und Alkohol können bei gleichzeitiger Einnahme mit Bilol comp. die blutdrucksenkende Wirkung verstärken oder abschwächen bzw. unerwünschte Wirkungen auslösen oder verstärken. Bitte informieren Sie daher Ihren Arzt, Ihre Ärztin über alle Arzneimittel, die Sie einnehmen.

Die gleichzeitige Anwendung von Narkosemitteln kann eine verstärkte Blutdrucksenkung zur Folge haben. Bilol comp. kann Wechselwirkungen mit anderen Arzneimitteln eingehen. Der Narkosearzt muss deshalb vor einer Narkose über die Behandlung mit Bilol comp. informiert werden.
Bei gleichzeitiger Anwendung von Lithium kann Bilol comp. die Ausscheidung von Lithium verringern und so die unerwünschten Wirkungen von Lithium auf Herz und Nerven verstärken.
Bei gleichzeitiger Anwendung von gewissen anderen blutdrucksenkenden Arzneimitteln, wie z.B. zentral wirksame blutdrucksenkende Arzneimittel, Kalzium-Antagonisten oder ACE-Hemmer, wurde eine verstärkte Blutdrucksenkung beobachtet.
Bei gleichzeitiger Einnahme von Bilol comp. und Mitteln gegen Herzrhythmusstörungen (Antiarrhythmika) kann die dämpfende Wirkung auf das Herz verstärkt werden. Dies ist insbesondere bei Verapamil und Diltiazem zu beachten. Ein gleichzeitiger Kaliummangel begünstigt das Auftreten von Herzrhythmusstörungen.
Die Kombination mit zu Kaliumverlusten führenden Arzneimitteln, wie gewisse Antibiotika (z.B. Amphotericin B), Abführmittel oder Kortison, verstärkt das Risiko eines Kaliummangels und begünstigt das Auftreten von Herzrhythmusstörungen. Kortison kann die blutdrucksenkende Wirkung von Bilol comp. vermindern.
Bei gleichzeitiger Therapie mit Digitalispräparaten begünstigt ein Kaliummangel das Auftreten von Digitalis-bedingten Nebenwirkungen wie z.B. Herzrhythmusstörungen.
Die blutdrucksenkende Wirkung von Bilol comp. kann durch die gleichzeitige Gabe von bestimmten Schmerz- und Rheumamitteln vermindert werden.
Bestimmte andere blutdrucksenkende Arzneimittel, Schlaf- und Beruhigungsmittel, Arzneimittel zur Behandlung einer Depression, Alkohol sowie Augentropfen zur lokalen Behandlung des grünen Stars können zu verstärktem Blutdruckabfall führen.
Bei Patienten mit hohen Harnsäurewerten kann das Risiko eines Gichtanfalls erhöht werden. Die Wirkung von Gichtmitteln kann durch Bilol comp. abgeschwächt werden.
Bei Kombination von Bilol comp. und Mefloquin-haltigen Malariamitteln ist das Risiko für eine Pulsverlangsamung erhöht.
Eine gleichzeitige Einnahme von gewissen Lipidsenkern (z.B. Colestyramin, Colestipol) kann die Wirkung von Bilol comp. abschwächen.
Durch individuell auftretende unterschiedliche Reaktionen auf die Blutdrucksenkung kann die Fähigkeit zur aktiven Teilnahme am Strassenverkehr oder zum Bedienen von Maschinen beeinträchtigt werden. Dies gilt in verstärktem Masse bei Behandlungsbeginn, bei einem Wechsel des Arzneimittels sowie im Zusammenwirken mit Alkohol.

Informieren Sie Ihren Arzt, Apotheker bzw. Ihre Ärztin, Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen.

+-Darf Bilol comp. während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Bilol comp. sollte während der Schwangerschaft nicht angewendet werden, es sei denn, es ist klar notwendig. Stillende Mütter sollten auf die Anwendung von Bilol comp. verzichten.

+-Wie verwenden Sie Bilol comp.?

Allgemeine Dosierungshinweise

Die Behandlung mit Bilol comp. wird vom Arzt bzw. der Ärztin individuell dem Patienten angepasst. Die Filmtabletten sollten morgens nüchtern oder zum Frühstück unzerkaut mit etwas Flüssigkeit eingenommen werden. Es ist auf eine ausreichende Flüssigkeitsaufnahme zu achten. Die übliche Dosierung beträgt 1 Filmtablette Bilol comp. 5/12,5 resp. 1 Filmtablette Bilol comp. 10/25 pro Tag.

Spezielle Dosierungshinweise

Die Anwendung und Sicherheit von Bilol comp. bei Kindern und Jugendlichen ist bisher nicht geprüft worden. Deshalb wird Bilol comp. für die Behandlung von Kindern und Jugendlichen nicht empfohlen. Für ältere Patienten ist keine Dosisanpassung notwendig. Für Patienten mit leichten bis mittelschweren Nierenfunktionsstörungen ist die niedrigere Dosierung Bilol comp. 5/12,5 zu bevorzugen.

Vorgehen bei vergessenen Filmtabletten

Falls Sie die Einnahme von Bilol comp. vergessen haben, so dürfen Sie das nicht durch die Einnahme der doppelten Menge Filmtabletten kompensieren. Sie führen die Therapie wie gewohnt und von Ihrem Arzt oder Ihrer Ärztin verordnet fort.

Beendigung der Behandlung

Bitte unterbrechen oder beenden Sie die Behandlung mit Bilol comp. nicht ohne vorherige Absprache mit Ihrem Arzt bzw. Ihrer Ärztin. Besonders bei Patienten mit gewissen Durchblutungsstörungen des Herzens (koronare Herzkrankheit, Angina pectoris) sollte die Behandlung nicht plötzlich, sondern schrittweise beendet werden, um eine mögliche Verschlechterung des Zustandes durch das Absetzen von Bilol comp. zu vermeiden.

Vorgehen bei Überdosierung

Bei einer Überdosierung benachrichtigen Sie bitte Ihren Arzt bzw. Ihre Ärztin. Diese können entsprechend der Schwere der Überdosierung über die gegebenenfalls erforderlichen Massnahmen entscheiden.

Die häufigsten Zeichen einer Überdosierung mit Bilol comp. sind verlangsamter Herzschlag (Bradykardie), plötzlich auftretende Atemprobleme (Bronchialverkrampfung), starker Blutdruckabfall, plötzlich auftretende Herzprobleme (akute Herzmuskelschwäche) und bei Diabetikern Unterzuckerung (Hypoglykämie). Ihr Arzt oder Ihre Ärztin wird Sie über die weitere Behandlung genau orientieren.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

+-Welche Nebenwirkungen kann Bilol comp. haben?

Häufig:

Erhöhung der Blutfettwerte, der Blutharnsäurewerte sowie der Blutzuckerwerte, Störungen des Wasser- und Elektrolythaushaltes (z.B. Verminderung der Kalium- und Natriumwerte im Blut), Erschöpfungszustände (Fatigue), Schwindel, Kopfschmerzen und Schwitzen. Diese Erscheinungen treten insbesondere zu Behandlungsbeginn auf, sind meist leichter Natur und verschwinden im weiteren Verlauf der Behandlung.

Kribbeln und Kältegefühl in den Extremitäten, Übelkeit, Erbrechen, Durchfall, Verstopfung, Bauchschmerzen.

Gelegentlich:

Allgemeine Schwäche (Asthenie), Schlafstörungen, Depression, Magen-Darm-Beschwerden, Appetitverlust, Muskelschwäche, Muskelkrämpfe, verlangsamter Herzschlag (Bradykardie), verstärkter Blutdruckabfall, erhöhter Atemwiderstand bei Patienten mit Asthma und chronischen Atemwegserkrankungen, Entzündungen der Bauchspeicheldrüse.

Selten:

Gesteigerte Traumaktivität, Halluzinationen, Überempfindlichkeitsreaktionen der Haut wie Jucken, Rötung, Ausschlag oder Sonnenallergie, reduzierter Tränenfluss (bei Kontaktlinsenträgern zu beachten), Sehstörungen, Kreislaufkollaps, allergischer Schnupfen, Gelbsucht, Hepatitis, Potenzstörungen, Hörstörungen, Blutbildveränderungen (Leukopenie, Thrombozytopenie).

Sehr selten:

Reduzierung der Säure im Blut (metabolische Alkalose), Mundtrockenheit, Schmerzen in der Brust, Bindehautentzündung des Auges (Konjunktivitis), kreisrunder Haarausfall, Auslösung bzw. Verschlechterung einer Schuppenflechte (Psoriasis), Immunkrankheiten der Haut (kutaner Lupus erythematodes), Verminderung der weissen Blutkörperchen (Agranulozytose), Entzündung der Gefässe (Vaskulitis).

Häufigkeit unbekannt:

Lungenfibrose (interstitielle Lungenerkrankung).

In der Originalverpackung, bei Raumtemperatur (15-25 °C) und ausser Reichweite von Kindern lagern.
