    +-Was ist Atorvastatin-Mepha und wann wird es angewendet?

    Atorvastatin-Mepha ist ein Arzneimittel zur Behandlung von Störungen des Fettstoffwechsels bei ungenügender Wirkung anderer Massnahmen (z.B. Umstellung der Ernährung, körperliches Training, Gewichtsabnahme) bei Patientinnen und Patienten mit

    • erhöhten Blutcholesterinwerten, die nicht auf eine andere Erkrankung zurückzuführen sind,
    • erhöhten Cholesterin- und Triglyceridwerten im Blut,
    • erblich bedingten erhöhten Blutcholesterinwerten.

    Atorvastatin-Mepha kann auch zur Prävention von Herz-Kreislauf-Ereignissen (z.B. Herzinfarkte, Angina pectoris, Hirnschläge) angewendet werden.

    Atorvastatin-Mepha ist nur auf Verschreibung des Arztes oder der Ärztin zu verwenden.

    +-Was sollte dazu beachtet werden?

    Vor der Behandlung mit Atorvastatin-Mepha soll die Umstellung auf eine cholesterinarme Diät erfolgen. Diese Diät sollten Sie auch während der Therapie mit Atorvastatin-Mepha beibehalten.

    +-Wann darf Atorvastatin-Mepha nicht angewendet werden?

    Sie dürfen Atorvastatin-Mepha nicht einnehmen, wenn Sie

    • gegen einen der Inhaltsstoffe allergisch sind,
    • eine bestehende Lebererkrankung haben oder bei Ihnen unklare und dauerhafte Erhöhungen der Leberfunktionswerte (Serumtransaminasen) gemessen wurden,
    • schwanger sind oder schwanger werden möchten,
    • stillen.

    Therapieerfahrungen bei Kindern liegen nur ungenügend vor. Deshalb sollte das Arzneimittel bei Kindern nicht angewendet werden, ausser auf ausdrückliche Verordnung des Arztes bzw. der Ärztin.

    Es liegen ausreichende Therapieerfahrungen an Erwachsenen im Alter von 70 oder mehr Jahren vor. Basierend auf diesen Untersuchungen ist eine Dosisanpassung bei älteren Patientinnen und Patienten nicht erforderlich.

    +-Wann ist bei der Einnahme von Atorvastatin-Mepha Vorsicht geboten?

    Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fahrtüchtigkeit und Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigen.

    Informieren Sie Ihren Arzt bzw. Ihre Ärztin bevor Sie die Behandlung beginnen, wenn:

    • Sie an einer Nierenkrankheit leiden,
    • Sie an einer Unterfunktion der Schilddrüse leiden,
    • bei Ihnen persönlich oder in Ihrer Familie eine erbliche Muskelkrankheit vorgekommen ist,
    • bei Ihnen selber bereits eine Muskelschädigung in Verbindung mit einem Statin oder Fibrat aufgetreten ist,
    • Sie grosse Mengen Alkohol trinken und/oder an einer Leberkrankheit gelitten haben,

    Leberfunktionsprüfungen sollten vor Therapiebeginn und anschliessend in regelmässigen Abständen durch Ihren Arzt bzw. Ihre Ärztin durchgeführt werden. Sollte ein Anstieg der Leberfunktionswerte auf mehr als das 3fache des Normalwertes über längere Zeit bestehen bleiben, wird eine Verringerung der Dosis oder ein Abbrechen der Behandlung mit Atorvastatin-Mepha empfohlen.

    Wenn Sie an einer Blutzuckererkrankung (Diabetes) leiden oder bei Ihnen das Risiko besteht, eine solche zu entwickeln, wird Ihr Arzt bzw. Ihre Ärztin Sie während der Behandlung mit Atorvastatin-Mepha daraufhin überwachen.

    Grapefruitsaft enthält eine oder mehrere Komponenten, die den Stoffwechsel einiger Arzneimittel einschliesslich Atorvastatin-Mepha verändern. Wenn Sie Atorvastatin-Mepha einnehmen, sollten Sie grössere Mengen Grapefruitsaft (mehr als 1 Liter täglich) vermeiden; bei normalen Mengen (1 Glas zu 250 ml täglich) ist es unwahrscheinlich, dass daraus Probleme entstehen können.

    Atorvastatin-Mepha kann die Skelettmuskulatur beeinflussen. Beim Auftreten von Muskelschmerzen oder -empfindlichkeit, Muskelkrämpfen oder Muskelschwäche, vor allem in Kombination mit Fieber oder Unwohlsein, konsultieren Sie bitte sofort Ihren Arzt bzw. Ihre Ärztin.

    Weil die Einnahme von Atorvastatin-Mepha mit jedem der nachfolgend genannten Arzneimitteln das Risiko von Muskelbeschwerden erhöhen kann (siehe «Welche Nebenwirkungen kann Atorvastatin-Mepha haben?»), ist es besonders wichtig, Ihren Arzt bzw. Ihre Ärztin darüber zu informieren, wenn Sie folgende Arzneimittel einnehmen:

    • Ciclosporin (Sandimmun®),
    • Hepatitis-C-Proteasehemmer, z.B. Telaprevir (Incivo®) oder Boceprevir (Victrelis®),
    • Colchicinhaltige Arzneimittel oder Zubereitungen,
    • Arzneimittel gegen Pilze, z.B. Itraconazol (Sporanox®), Ketoconazol (Nizoral®) oder Voriconazol (Vfend®),
    • Fibratderivate, z.B. Gemfibrozil (Gevilon®), Bezafibrat (Cedur®) oder Fenofibrat (Lipanthyl®),
    • Antibiotika, z.B. Erythromycin (Erythrocin®), Clarithromycin (Klacid®), Rifampicin oder Fusidinsäure (Fucidin®),
    • HIV-Proteasehemmer, z.B. Indinavir (Crixivan®), Nelfinavir (Viracept®), Ritonavir (Norvir®), Lopinavir/Ritonavir (Kaletra®), Saquinavir (Invirase®), Tipranavir (Aptivus®), Darunavir (Prezista®) oder Fosamprenavir (Telzir®).

    Es ist auch wichtig, Ihren Arzt bzw. Ihre Ärztin darüber zu informieren, wenn Sie Antikoagulantien (Mittel zur Blutverdünnung), Arzneimittel bei Herz-Kreislauf-Erkrankungen wie Diltiazem (Dilzem®), Digoxin (Digoxin Sandoz®) oder Präparate mit Amlodipin, Arzneimittel zur Behandlung von HIV-Infektionen wie Efavirenz (Stocrin®), Antazida (Mittel gegen Magenübersäuerung) oder orale Kontrazeptiva (Verhütungsmittel) einnehmen.

    Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie

    • an anderen Krankheiten leiden,
    • Allergien haben oder
    • andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden (bei Externa)!
    +-Darf Atorvastatin-Mepha während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

    Wenn Sie schwanger sind oder stillen, dürfen Sie Atorvastatin-Mepha nicht einnehmen. Frauen im gebärfähigen Alter können Atorvastatin-Mepha nur dann anwenden, wenn wirksame Massnahmen zur Schwangerschaftsverhütung sichergestellt sind. Wenn es trotzdem zu einer Schwangerschaft kommt, muss die Behandlung mit Atorvastatin-Mepha sofort unterbrochen und der Arzt bzw. die Ärztin darüber informiert werden.

    +-Wie verwenden Sie Atorvastatin-Mepha?

    Die übliche Anfangsdosis beträgt einmal täglich 10 mg Atorvastatin-Mepha. Ihr Arzt bzw. Ihre Ärztin wird die weitere Dosierung so festlegen, dass bei Ihnen ein normaler Blutcholesterinwert erzielt wird. Eine entsprechende Dosisanpassung wird individuell in Abständen von 4 oder mehr Wochen durchgeführt. Die maximale Tagesdosis beträgt einmal täglich 80 mg Atorvastatin-Mepha.

    Der therapeutische Erfolg zeigt sich innerhalb von 2 Wochen, und die grösste Wirkung wird in der Regel schon nach 4 Wochen erreicht. Die Wirkung bleibt unter Dauertherapie erhalten.

    Atorvastatin-Mepha ist zur Langzeitanwendung geeignet.

    Atorvastatin-Mepha kann zu jeder Tageszeit unabhängig von den Mahlzeiten mit etwas Flüssigkeit eingenommen werden. Eine einmal gewählte Tageszeit soll beibehalten werden.

    Die Lactab zu 20 mg, 40 mg und 80 mg verfügen über eine Bruchrille und können in Hälften geteilt werden.

    Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

    Was tun, wenn Sie zu viel eingenommen haben?

    Informieren Sie sofort Ihren Arzt bzw. Ihre Ärztin.

    Was tun, wenn Sie eine Dosis vergessen haben?

    Versuchen Sie Atorvastatin-Mepha so einzunehmen, wie es Ihnen verschrieben wurde. Falls Sie eine Dosis vergessen haben, holen Sie die Einnahme nicht mit einer Extradosis nach, sondern fahren Sie am folgenden Tag mit der normalen Einnahme weiter.

    +-Welche Nebenwirkungen kann Atorvastatin-Mepha haben?

    Häufig wurde berichtet über Entzündung des Nasenrachenraumes, Schmerzen im Rachen- und Kehlraum, Nasenbluten, Gelenk- oder Muskelschmerzen, Muskelkrämpfe, geschwollene Gelenke, Gliederschmerzen, Kopfschmerzen, zu hoher Blutzuckerspiegel, Durchfall, Verdauungsstörungen, Verstopfung, Übelkeit, Blähungen und allergische Reaktionen.

    Gelegentlich wurde berichtet über Bauchschmerzen, Haarausfall, Hautausschlag, Juckreiz, Rücken- und Nackenschmerzen, Muskelschwäche, Sensibilitätsstörungen (Kribbeln, Schmerzen, Taubheit in Händen und Füssen), Schwindel, Schlaflosigkeit, Müdigkeit, Schwächezustände, Unwohlsein, Erbrechen, Appetitlosigkeit, Geschmacksstörungen, Aufstossen, Schmerzen im Brustkorb, Ohrensausen, Alpträume, zu tiefer Blutzuckerspiegel, Gewichtszunahme, Wasseransammlung in den Extremitäten (z.B. Schwellung der Fussgelenke), Sehstörungen und Fieber.

    Selten wurde berichtet über Sehnenscheidenentzündung, Gelbsucht, Flüssigkeitsansammlung v.a. im Gesicht und Impotenz.

    Sehr selten wurde berichtet über Gedächtnisstörung, Geschmacksverlust, Überempfindlichkeitsreaktionen wie z.B. der Haut (rote Flecken, Papeln) und Sehnenriss.

    Wenn solche Symptome bei Ihnen auftreten, müssen Sie sofort Ihren Arzt bzw. Ihre Ärztin aufsuchen und die Einnahme von Atorvastatin-Mepha abbrechen.

    Ihr Arzt bzw. Ihre Ärztin wird in periodischen Abständen Ihr Blut untersuchen, da bei gewissen Patientinnen und Patienten die Leberfunktionswerte änderten oder Muskelschmerzen vorkamen. Da Muskelprobleme in seltenen Fällen schwerwiegend sein können, sollten Sie unverzüglich Ihren Arzt bzw. Ihre Ärztin kontaktieren, wenn Sie Muskelschmerzen, Muskelempfindlichkeit, Muskelkrämpfe oder Muskelschwäche verspüren.

    Wenn Sie diese oder andere Nebenwirkungen, die hier nicht beschrieben sind, an sich beobachten, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren. Diese verfügen über die ausführliche Fachinformation.

    +-Was ist ferner zu beachten?

    Während der Behandlung mit Atorvastatin-Mepha ist auf eine übermässige Einnahme von alkoholischen Getränken möglichst zu verzichten.

    Atorvastatin-Mepha ist für Kinder unerreichbar aufzubewahren.

    Nicht über 30 °C und in der Originalverpackung lagern.

    Das Arzneimittel darf nur bis zu dem auf der Packung mit «EXP» bezeichneten Datum verwendet werden.
