    +-Was ist Actos und wann wird es angewendet?

    Actos Tabletten enthalten den Wirkstoff Pioglitazon, der die Insulinwirkung unterstützt und dadurch Ihren Blutzuckerspiegel kontrolliert, wenn Sie an Diabetes mellitus (Zuckerkrankheit) vom Typ 2 erkrankt sind. Diese Diabetesform tritt gewöhnlich erst im Erwachsenenalter auf und muss durch eine Ernährungsumstellung, körperliche Aktivität und manchmal medikamentös behandelt werden.

    Actos wird nur auf Verschreibung des Arztes oder der Ärztin zur Behandlung von Diabetes vom Typ 2 (Altersdiabetes oder nicht-insulinpflichtiger Diabetes) eingesetzt, wenn der Blutzuckerspiegel nicht durch eine Ernährungsumstellung und ausreichende körperliche Aktivität eingestellt werden kann und Metformin (ein anderes blutzuckersenkendes Arzneimittel) nicht geeignet ist. Ihr Arzt bzw. Ihre Ärztin wird regelmässig alle 3 bis 6 Monate überprüfen, ob Actos wirkt und ob Actos die für Sie geeignete Behandlung darstellt.

    Actos kann in Verbindung mit einer Diät allein oder zusammen mit anderen blutzuckersenkenden Arzneimitteln wie orale Antidiabetika (z.B. Chlorpropamid, Glibenclamid, Gliclazid, Glibornurid, Glimepirid, Glipizid, Metformin) oder Insulin angewendet werden.

    +-Was sollte dazu beachtet werden?

    Zur Behandlung von Diabetes vom Typ 2 ist auch eine Ernährungskontrolle notwendig. Ihr Arzt oder Ihre Ärztin wird Ihnen raten, Ihre Kalorienzufuhr einzuschränken, abzunehmen und sich körperlich zu betätigen. Die Befolgung dieser Anweisungen ist wichtig, um sicherzustellen, dass die Wirksamkeit von Actos erhalten bleibt.

    Wie bei allen anderen Arzneimitteln zur Behandlung des Diabetes sind unter der Therapie mit Actos regelmässige Kontrollen des Blut- und Urinzuckerspiegels notwendig. Den Konsum von Alkohol sollten Sie während einer Diabetes-Behandlung meiden.

    +-Wann darf Actos nicht eingenommen werden?
    • Wenn Sie bekanntermassen gegen den Wirkstoff Pioglitazon, andere ähnliche Substanzen (so genannte Thiazolidindione) oder einen der sonstigen Bestandteile der Tabletten allergisch sind.
    • Wenn Sie an Diabetes vom Typ 1 leiden (juveniler oder insulinabhängiger Diabetes; diese Diabetesform tritt in der Regel bereits in der Jugend auf und muss mit Insulininjektionen behandelt werden).
    • Wenn Sie an einer schweren Herzkrankheit leiden (Herzschwäche).
    • Bei Zuckerkrankheit mit Stoffwechselentgleisungen (diabetische Ketoazidose).
    • Wenn Sie an einer mittelschweren bis schweren Lebererkrankung leiden.
    • Wenn Sie schwanger sind oder stillen.
    • Bei Kindern und Jugendlichen.
    • Wenn Sie Blasenkrebs haben oder jemals gehabt haben
    • Wenn Sie Blut im Urin haben und Ihr Arzt bzw. Ihre Ärztin das nicht untersucht hat
    +-Wann ist bei der Einnahme von Actos Vorsicht geboten?

    Wenn Actos zusammen mit anderen oralen Antidiabetika oder Insulin verabreicht wird, kann eine Hypoglykämie (Unterzuckerung, übermässige Senkung des Blutzuckerspiegels) entstehen.

    Bitte beachten Sie Anzeichen von unerwünschten Blutzuckerveränderungen:

    • Zu hoher Blutzucker: starkes Durstgefühl, Mundtrockenheit, häufiges Wasserlassen und trockene Haut. Informieren Sie Ihren Arzt oder Ihre Ärztin, damit er/sie mit Ihnen Ihre Diät besprechen oder die Tabletteneinnahme korrigieren kann.
    • Zu tiefer Blutzucker: u.a. Heisshunger, Übelkeit, Erbrechen, Schwitzen, Zittern, Unruhe, Reizbarkeit, Kopfschmerzen, Schlafstörungen, Verwirrtheit, Schwindel, Herzklopfen. Diese Zustände lassen sich häufig rasch durch Einnahme von Zucker (z.B. Würfelzucker: 2-4 Stück, süsse Fruchtsäfte oder gezuckerten Tee) beheben. Beachten Sie, dass künstliche Süssstoffe unwirksam sind.

    Wird die Einnahme von Zucker unterlassen, kann es infolge extremer Blutzuckersenkungen zu Sprech- und Sehstörungen, Lähmungserscheinungen oder Empfindungsstörungen und Bewusstlosigkeit kommen.
    Besprechen Sie die Beobachtungen mit Ihrem Arzt oder Ihrer Ärztin.
    Wenn die Anzeichen nicht verschwinden, informieren Sie unverzüglich Ihren Arzt oder Ihre Ärztin.

    Ein nicht richtig eingestellter Blutzuckerspiegel kann Ihre Aufmerksamkeit und Reaktionsfähigkeit vermindern. Sie sollten Vorsichtsmassnahmen ergreifen, um eine Hypoglykämie (Unterzuckerung) beim Lenken eines Fahrzeugs oder beim Bedienen von Maschinen zu vermeiden. Das ist besonders dann wichtig, wenn bei Ihnen die Anzeichen einer Hypoglykämie nur schwach ausgeprägt sind oder fehlen oder wenn bei Ihnen häufig Hypoglykämien auftreten. In diesen Fällen sollten Sie in Betracht ziehen, das Lenken eines Fahrzeugs oder das Bedienen von Maschinen zu unterlassen.

    Teilen Sie Ihrem Arzt oder Ihrer Ärztin mit, wenn bei Ihnen die Leberfunktion eingeschränkt ist. Bei Patienten mit mittelschweren und schweren Lebererkrankungen soll Actos nicht angewendet werden.

    Informieren Sie Ihren Arzt oder Ihre Ärztin, wenn Sie an einer diabetischen Augenerkrankung leiden.

    Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie an einer Herzerkrankung (Herzschwäche) leiden, insbesondere wenn Sie älter als 75 Jahre sind. Actos kann zu einer Flüssigkeitsansammlung im Körper mit Gewichtszunahme führen, was die Herzerkrankung verschlimmern kann. Wenn Sie an einer leichteren Herzschwäche leiden, wird Ihr Arzt oder Ihre Ärztin entscheiden, ob Sie Actos einnehmen dürfen.

    Unter der Behandlung mit Actos kann es zu einer Gewichtszunahme kommen. Eine regelmässige Kontrolle des Körpergewichtes ist deshalb wichtig.

    In klinischen Langzeitstudien beobachtete man bei Frauen, die Pioglitazon erhielten, eine geringfügig höhere Anzahl von Knochenbrüchen als unter einer Vergleichsbehandlung, nicht jedoch bei Männern. Ihr Arzt bzw. Ihre Ärztin wird dies bei der Behandlung Ihres Diabetes berücksichtigen.

    Actos wird mit einer geringen Erhöhung des Risikos von Blasenkrebs in Verbindung gebracht. Die Fortsetzung der Behandlung mit Actos soll nach zwei Jahren überprüft werden und eine Umstellung auf eine alternative Behandlung des Diabetes mit Ihrem Arzt besprochen werden. Die Behandlung soll nur über zwei Jahre hinaus fortgesetzt werden, wenn der Nutzen das Risiko eindeutig überwiegt.

    Bei Frauen, bei denen keine monatliche Blutung auftritt, die aber auch nicht in den Wechseljahren sind, kann der Eisprung und damit die Monatsblutung bei Einnahme von Actos wieder einsetzen. Wenn dies bei Ihnen der Fall ist, konsultieren Sie dann bitte Ihren Arzt bzw. Ihre Ärztin, da Sie wieder schwanger werden können.

    Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie

    • an anderen Krankheiten leiden,
    • Allergien haben oder
    • andere Arzneimittel (auch selbst gekaufte) einnehmen!

    Dies betrifft insbesondere Arzneimittel zur Behandlung des Diabetes (Zuckerkrankheit) wie z.B. Chlorpropamid, Glibenclamid, Gliclazid, Glibornurid, Glimepirid, Glipizid oder Insulin.

    +-Darf Actos während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

    Arzneimittel wie Actos dürfen in der Schwangerschaft nicht angewendet werden. Während der Schwangerschaft wird der Diabetes mit Insulin behandelt.

    Informieren Sie Ihren Arzt oder Ihre Ärztin, wenn Sie schwanger sind oder eine Schwangerschaft planen, damit die Diabetesbehandlung auf Insulin umgestellt werden kann.

    Während der Stillzeit darf Actos nicht eingenommen werden.

    +-Wie verwenden Sie Actos?

    Ihr Arzt oder Ihre Ärztin bestimmt die Dosierung. In der Regel beträgt die Dosis 1 Tablette Actos 15 mg oder 1 Tablette Actos 30 mg einmal täglich.

    Die Einnahme erfolgt morgens mit etwas Flüssigkeit mit oder ohne Mahlzeit.

    Falls notwendig, kann Ihr Arzt oder Ihre Ärztin die Dosis auf 1 Tablette Actos 45 mg einmal täglich erhöhen.

    Ihr Arzt bzw. Ihre Ärztin kann Ihnen Actos auch in Kombination mit anderen blutzuckersenkenden Arzneimitteln zum Einnehmen oder mit Insulin verschreiben.

    Wenn Sie einmal eine Dosis vergessen haben, sollten Sie die nächste Dosis einfach wie gewohnt einnehmen. Nehmen Sie nicht die doppelte Dosis ein, um die vergessene Tablette nachzuholen.

    Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

    +-Welche Nebenwirkungen kann Actos haben?

    Folgende Nebenwirkungen können bei der Anwendung von Actos, entweder alleine oder zusammen mit anderen Arzneimitteln gegen Diabetes eingenommen, auftreten:

    Häufig: Herzinsuffizienz ist häufig bei Patienten unter Behandlung mit Actos in Kombination mit Insulin aufgetreten. Symptome sind ungewöhnliche Kurzatmigkeit oder rasche Gewichtszunahme oder lokale Schwellungen (Ödeme). Wenn eines dieser Symptome bei Ihnen auftritt, insbesondere wenn Sie über 65 Jahre alt sind, konsultieren Sie sofort einen Arzt bzw. eine Ärztin. Infektionen der oberen Atemwege, Nasennebenhöhlenentzündung, Halsentzündung, Gewichtszunahme, Unterzuckerung, Kopfschmerzen, Kribbeln oder Taubheitsgefühl, Müdigkeit, Schlaflosigkeit, Sehstörungen, Schwellungen bedingt durch Flüssigkeitsansammlungen (Ödeme), Magen-Darm-Entzündung, Verdauungsstörungen, Zahnprobleme, Gelenk-, Muskel- oder Rückenschmerzen, Atemnot und Blut im Urin.

    Gelegentlich: Blasenkrebs ist gelegentlich bei Patienten unter Behandlung mit Actos aufgetreten. Anzeichen und Symptome umfassen Blut im Urin, Schmerzen beim Harnlassen oder plötzlicher Harndrang. Wenn Sie eines dieser Anzeichen oder Symptome beobachten, sprechen Sie sobald wie möglich mit Ihrem Arzt bzw. Ihrer Ärztin. Appetitzunahme.

    Selten: Herzschwäche.

    Sehr selten: Leberfunktionsstörung.

    Sehr selten kann verschwommenes Sehen oder Abnahme des Sehvermögens, vor allem auch nachts, aufgrund einer Schwellung oder Flüssigkeitsansammlung auftreten. Wenn diese Symptome oder andere Sehstörungen bei Ihnen zum ersten Mal auftreten oder wenn diese stärker werden, sollten Sie so schnell wie möglich Ihren Arzt oder Ihre Ärztin informieren.

    Wenn Actos zusammen mit anderen Arzneimitteln gegen Diabetes eingenommen wird, kann es häufiger zu einer Unterzuckerung kommen als mit Actos allein (vor allem bei Kombination mit Insulin).
