Actiq ist eine Arzneiform zur direkten Aufnahme des arzneilichen Wirkstoffs Fentanyl durch die Mundschleimhaut.
Der Wirkstoff Fentanyl ist ein starkes Schmerzmittel, das als Opioid bezeichnet wird. Sobald Sie Actiq in den Mund legen, löst sich das Arzneimittel auf und der Wirkstoff wird über die Mundschleimhaut aufgenommen und in Ihre Blutbahn abgegeben. Durch diese Art der Einnahme kann der Arzneistoff sehr schnell aufgenommen werden, so dass Ihre Durchbruchschmerzen schnell gelindert werden.
Auf Verschreibung des Arztes oder der Ärztin ist Actiq für die Behandlung von Durchbruchschmerzen bei Patientinnen und Patienten bestimmt, deren chronische Tumorschmerzen bereits mit Opioiden als Basistherapie behandelt werden. Durchbruchschmerzen sind Schmerzen, die plötzlich auftreten, obwohl Sie Ihre gewohnten Opioid-Schmerzmittel eingenommen haben.
Sie dürfen Actiq nur anwenden, wenn Sie gegen Ihre chronischen (andauernden) Schmerzen ein vom Arzt bzw. der Ärztin verschriebenes Opioid-Arzneimittel regelmässig jeden Tag über mindestens eine Woche angewendet haben. Wenn Sie solche Arzneimittel nicht anwenden, kann Actiq das Risiko für eine verlangsamte oder flache Atmung bis hin zum Atemstillstand erhöhen.
Verwenden Sie Actiq nicht zur Behandlung von Schmerzen durch Verletzungen, Operationen, Kopfschmerzen oder Migräne.
Actiq enthält Glucose und Sucrose (zwei Arten von Zucker). Bitte wenden Sie Actiq erst nach Rücksprache mit Ihrem Arzt bzw. Ihrer Ärztin an, wenn Ihnen bekannt ist, dass Sie unter einer Zuckerunverträglichkeit leiden, d.h. wenn Sie manche Arten von Zucker nicht vertragen bzw. verdauen können.
Jede Dosiereinheit Actiq enthält ca. 2 Gramm verwertbare Kohlenhydrate. Sie müssen dies berücksichtigen, wenn Sie Diabetes haben.
Der Glucoseanteil in Actiq kann schädlich für die Zähne sein. Achten Sie immer darauf, dass Sie Ihre Zähne regelmässig putzen.
Wenden Sie Actiq nicht an, wenn eine der oben genannten Situationen auf Sie zutrifft. Wenn Sie sich nicht sicher sind, fragen Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin vor der Anwendung von Actiq um Rat.
Während der Einnahme von Actiq sollten Sie das Opioid-Schmerzmittel, das Sie bereits gegen Ihre chronischen (andauernden) Schmerzen einnehmen, weiter anwenden.
Actiq wird für die Anwendung bei Kindern unter 16 Jahren nicht empfohlen.
Im Folgenden wird beschrieben, wann Sie Actiq nur unter bestimmten Bedingungen und nur mit besonderer Vorsicht anwenden dürfen. Fragen Sie hierzu bitte Ihren Arzt bzw. Ihre Ärztin. Dies gilt auch, wenn diese Angaben bei Ihnen früher einmal zutrafen.
Bei wiederholter Gabe von Opioiden wie Fentanyl kann es zu Toleranzentwicklung und physischer und/oder psychischer Abhängigkeit kommen.
Wenden Sie dieses Arzneimittel nicht an und informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie folgende Arzneimittel anwenden:
Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fahrtüchtigkeit und Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigen! Sie sollten deshalb auf das Führen von Fahrzeugen und das Bedienen von Maschinen verzichten.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) ausser Ihre regelmässig verordneten Opioid-Schmerzmittel einnehmen oder äusserlich anwenden bzw. vor kurzem eingenommen haben!
Es ist besonders wichtig, dass Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren, wenn Sie eines der folgenden Arzneimittel anwenden:
Actiq kann vor oder nach den Mahlzeiten angewendet werden. Wenden Sie Actiq jedoch nicht während der Mahlzeiten an.
Vor der Anwendung von Actiq können Sie etwas Wasser trinken, um Ihren Mund anzufeuchten. Während der Anwendung von Actiq dürfen Sie jedoch nichts trinken oder essen.
Sie dürfen während der Behandlung mit Actiq keinen Grapefruitsaft trinken. Grapefruitsaft kann den Abbau von Actiq in Ihrem Körper beeinflussen.
Trinken Sie keinen Alkohol während der Anwendung von Actiq. Alkohol kann das Risiko gefährlicher Nebenwirkungen erhöhen.
Sie sollten Actiq während der Schwangerschaft nicht anwenden, es sei denn, Sie haben dies mit Ihrem Arzt bzw. Ihrer Ärztin so besprochen. Sie dürfen Actiq nicht während der Geburt anwenden, da Fentanyl beim Neugeborenen Atemschwierigkeiten verursachen kann. Bei längerfristiger Anwendung während der Schwangerschaft besteht ausserdem das Risiko von Entzugserscheinungen beim Neugeborenen.
Fentanyl geht in die Muttermilch über und kann beim Säugling Nebenwirkungen hervorrufen. Sie dürfen Actiq in der Stillzeit nicht anwenden, es sei denn, Sie haben es mit Ihrem Arzt bzw. Ihrer Ärztin besprochen. Das Stillen darf frühestens 48 Stunden nach der letzten Anwendung von Actiq wieder begonnen werden.
Wenn Sie schwanger sind oder stillen, fragen Sie bitte vor der Einnahme aller Arzneimittel Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin um Rat.
Bei der ersten Anwendung von Actiq wird Ihr Arzt bzw. Ihre Ärztin mit Ihnen gemeinsam die Dosis Actiq suchen, die Ihre Durchbruchschmerzen lindern kann. Es ist sehr wichtig, dass Sie sich bei der Anwendung von Actiq genau an die Anweisungen Ihres Arztes bzw. Ihrer Ärztin halten. Ändern Sie nicht eigenmächtig die Dosierung von Actiq oder Ihren anderen Schmerzmitteln. Dosisänderungen müssen vom Arzt bzw. der Ärztin angeordnet und überprüft werden.
Bitte sprechen Sie mit Ihrem Arzt bzw. Ihrer Ärztin, wenn Sie sich bezüglich der richtigen Dosis nicht sicher sind oder wenn Sie Fragen zur Anwendung von Actiq haben.
Sobald Sie die Dosiereinheit in den Mund legen:
Die Dosiereinheit löst sich auf und der Wirkstoff wird freigesetzt. Dieser Vorgang dauert ca. 15 Minuten.
Der Wirkstoff wird über die Mundschleimhaut in die Blutbahn aufgenommen.
Durch diese Art der Anwendung kann der Wirkstoff schnell aufgenommen werden. Somit werden Ihre Durchbruchschmerzen schnell gelindert.
Bei der Anwendung von Actiq sollten Sie schnell eine gewisse Linderung spüren.
Während Sie zusammen mit Ihrem Arzt bzw. Ihrer Ärztin die richtige Dosis ermitteln, die Ihre Durchbruchschmerzen wirksam lindern kann, kann es vorkommen, dass Sie auch 30 Minuten nach Anbruch der Actiq-Dosiereinheit (15 Minuten nach Aufbrauchen der Actiq-Dosiereinheit) noch keine ausreichende Schmerzlinderung verspüren. In diesem Fall wird Ihr Arzt bzw. Ihre Ärztin Ihnen möglicherweise erlauben, für die weitere Behandlung dieser Durchbruchschmerzattacke eine zweite Actiq-Dosiereinheit der gleichen Stärke anzuwenden. Wenden Sie nur auf ausdrückliche Anordnung des Arztes bzw. der Ärztin eine zweite Dosiereinheit an. Sie dürfen niemals mehr als zwei Dosiereinheiten zur Behandlung einer einzelnen Durchbruchschmerzattacke anwenden.
Während der Dosisfindung haben Sie möglicherweise gleichzeitig mehrere Stärken der Actiq-Dosiereinheiten bei sich zu Hause. Behalten Sie immer nur die Stärken, die Sie zu Hause benötigen. Dies dient zur Vermeidung von Verwechslungen oder einer versehentlichen Überdosierung. Sprechen Sie mit Ihrem Apotheker bzw. Ihrer Apothekerin über die Entsorgung ungebrauchter Actiq-Dosiereinheiten.
Sobald zusammen mit Ihrem Arzt bzw. Ihrer Ärztin die richtige Dosierung gefunden wurde, verwenden Sie eine Dosiereinheit pro Durchbruchschmerzattacke. Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn die für Sie ermittelte Dosierung Ihre Durchbruchschmerzen für mehrere Durchbruchschmerzattacken in Folge nicht lindert. Ihr Arzt bzw. Ihre Ärztin wird dann entscheiden, ob Ihre Dosierung verändert werden muss.
Warten Sie mindestens 4 Stunden, bevor Sie eine weitere Durchbruchschmerzepisode mit Actiq behandeln.
Informieren Sie sofort Ihren Arzt bzw. Ihre Ärztin, wenn Sie Actiq mehr als drei Mal täglich anwenden. Ihr Arzt bzw. Ihre Ärztin wird Sie in diesem Fall möglicherweise zur Behandlung Ihrer chronischen (andauernden) Schmerzen auf ein anderes Arzneimittel umstellen. Wenn eine Umstellung erfolgt ist und Ihre chronischen (andauernden) Schmerzen unter Kontrolle gebracht sind, muss Ihr Arzt bzw. Ihre Ärztin Ihre Dosis Actiq unter Umständen weiter verändern. Um optimale Ergebnisse zu erhalten, informieren Sie bitte Ihren Arzt bzw. Ihre Ärztin über das Ausmass Ihrer Schmerzen und über die Wirkung von Actiq, damit die Dosis bei Bedarf angepasst werden kann.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Jede Dosiereinheit Actiq ist in einer eigenen Blisterpackung versiegelt. Öffnen Sie die Packung erst, wenn Sie bereit zur Anwendung von Actiq sind. Öffnen Sie die Packung nicht im Voraus. Halten Sie die Blisterpackung so, dass die bedruckte Seite von Ihnen weg zeigt. Ergreifen Sie das kurze Laschenende der Blisterpackung. Setzen Sie die Schere nahe beim Ende der Actiq Dosiereinheit an und schneiden Sie das Ende mit der langen Lasche knapp oberhalb der Dosiereinheit Actiq mit der Schere vollständig ab (siehe Abbildung).
Trennen Sie die bedruckte Rückseite von der Blisterpackung und ziehen Sie sie vollständig von der Blisterpackung ab. Entnehmen Sie die Actiq-Dosiereinheit der Blisterpackung und legen Sie sie sofort in den Mund.
Anwendung der Actiq-Dosiereinheit
Legen Sie Actiq zwischen Ihre Wange und Zahnfleisch. Greifen Sie die Dosiereinheit am Applikator und bewegen Sie sie kontinuierlich in Ihrem Mund hin und her. Streichen Sie dabei insbesondere an den Wangeninnenseiten entlang. Drehen Sie den Applikator häufig.
Um die optimale Schmerzlinderung zu erzielen, sollten Sie die Dosiereinheit innerhalb von 15 Minuten vollständig aufbrauchen. Wenn Sie Actiq zu schnell aufbrauchen, schlucken Sie eine zu grosse Arzneimittelmenge, so dass Ihre Durchbruchschmerzen weniger gut gelindert werden.
Sie dürfen die Dosiereinheit nicht zerbeissen, lutschen oder kauen; dies würde zu geringeren Blutspiegeln und weniger Schmerzlinderung führen als bei bestimmungsgemässer Anwendung. Wenn Sie bei jeder Durchbruchschmerzattacke die Dosiereinheit Actiq aus irgendeinem Grund nur unvollständig anwenden, informieren Sie bitte Ihren Arzt bzw. Ihre Ärztin.
Sobald die Dosis ermittelt ist, mit der Ihre Schmerzen wirksam behandelt werden können, sollten Sie nicht mehr als drei Actiq-Dosiereinheiten pro Tag anwenden. Wenn Sie denken, dass Sie mehr als drei Actiq-Dosiereinheiten pro Tag benötigen, sprechen Sie bitte sofort mit Ihrem Arzt bzw. Ihrer Ärztin.
Verwenden Sie nicht mehr als zwei Dosiereinheiten zur Behandlung einer einzelnen Durchbruchschmerzattacke.
Die häufigsten Nebenwirkungen bei einer Überdosierung sind Schläfrigkeit, Übelkeit oder Schwindel. Wenn Sie sich schwindlig fühlen, Ihnen übel wird oder Sie sich sehr schläfrig fühlen, bevor die Dosiereinheit vollständig aufgelöst ist, nehmen Sie sie aus Ihrem Mund heraus und bitten Sie eine andere Person um Hilfe.
Eine schwerwiegende Nebenwirkung von Actiq ist eine verlangsamte und/oder flache Atmung. Dies kann auftreten, wenn die Dosierung von Actiq für Sie zu hoch ist oder wenn Sie Actiq zu oft anwenden.
Falls dies eintritt, rufen Sie bitte sofort den medizinischen Notdienst.
Wenn Sie glauben, dass jemand Actiq versehentlich angewendet hat, rufen Sie bitte sofort den medizinischen Notdienst. Versuchen Sie, die Person wach zu halten (indem Sie ihren Namen rufen und sie am Arm oder an der Schulter schütteln), bis der Notdienst eintrifft.
Wenn Ihre Durchbruchschmerzen noch andauern, können Sie Actiq wie von Ihrem Arzt bzw. Ihrer Ärztin verordnet anwenden. Wenn der Durchbruchschmerz aufgehört hat, wenden Sie Actiq bitte erst wieder bei der nächsten Durchbruchschmerzattacke an.
Brechen Sie die Anwendung von Actiq nicht ohne vorherige Rücksprache mit Ihrem Arzt bzw. Ihrer Ärztin ab. Normalerweise führt der Abbruch der Behandlung mit Actiq nicht zu merklichen Nebenwirkungen. Zur Behandlung Ihrer chronischen (andauernden) Schmerzen sollten Sie auch weiterhin Ihr gewohntes Opioid-Schmerzmittel nach Anweisung Ihres Arztes bzw. Ihrer Ärztin einnehmen.
Die unter Actiq beobachteten Nebenwirkungen sind typische Nebenwirkungen von Opioiden. Häufig verschwinden diese Opioid-Wirkungen bei fortgesetzter Anwendung von Actiq oder verlieren an Intensität, während der Patient auf die richtige Dosis eingestellt wird. Opioid-Wirkungen sind zu erwarten und sollten entsprechend behandelt werden.
Die schwerwiegendsten Nebenwirkungen aller Opioide sind Einschränkung der Atmung (die zu Atemstillstand führen kann), Kreislaufinsuffizienz, niedriger Blutdruck und Schock. Bitte achten Sie auf die Symptome einer Einschränkung der Atmung und benachrichtigen Sie sofort Ihren Arzt oder Ihre Ärztin.
Wenn Sie beobachten, dass der Patient bei der Anwendung von Actiq langsam und/oder flach atmet, oder wenn Sie ihn nur schwer aufwecken können, ergreifen Sie SOFORT die folgenden Massnahmen:
Folgende Nebenwirkungen können bei der Anwendung von Actiq auftreten:
Häufigkeit nicht bekannt: schwerwiegende allergische Reaktion, die mit Atemschwierigkeiten oder Schwindelgefühlen sowie Anschwellen der Zunge, Lippen oder Hals einhergeht.
Häufig: Kraftlosigkeit, Kopfschmerzen.
Gelegentlich: Aufgetriebener Bauch, Bauchschmerzen, unfallbedingte Verletzungen, überlange Nachwirkungen, Unwohlsein.
Gelegentlich: Niedriger Blutdruck, Herzjagen, Gefässerweiterung.
Sehr häufig: Übelkeit/Erbrechen.
Häufig: Verstopfung, Verdauungsstörungen.
Gelegentlich: Gewichtsabnahme, Lippenentzündung, Schluckstörungen, Aufstossen, Blähungen, Zahnfleischentzündung, Zahnfleisch-bluten, verstärkte Speichelabsonderung, Darmverschluss, Gelbsucht, Mundgeschwüre, Entzündungen der Mundschleimhaut, Zungenerkrankungen, Karies
Unbekannte Häufigkeit: Zahnverlust, Zahnfleischrückgang.
Gelegentlich: Durst.
Häufig: Angst, Verwirrtheit, Schwindelgefühl, Schwindel, Mundtrockenheit, Schlaflosigkeit, Schläfrigkeit, Bewusstseinsverlust, Krampfanfälle, Sedierung
Gelegentlich: Koordinationsstörungen, anormale Träume, Denkstörungen, Unruhe, Erinnerungslücken, Störung der Bewegungsabläufe, Missempfindungen um den Mund, verringerte Reflexe, Wahn, Depersonalisierung, Depressionen, emotionale Labilität, Euphorie, Halluzinationen, gesteigerte Empfindung schmerzhafter und nicht schmerzhafter Reize (Hyperästhesie), Mangel an Willkür- und Reaktivbewegungen (Hypokinesie), Muskelzucken, Taubheitsgefühl, Anfälle, Koma, Sprachstörungen.
Häufig: Atemnot
Gelegentlich: Asthma, verlangsamte, flache Atmung, Entzündung der Rachenschleimhaut, Einschränkung der Atmung.
Häufigkeit nicht bekannt: Rachenschwellung.
Häufig: Juckreiz, Schwitzen.
Gelegentlich: Ausschlag, Nesselsucht.
Häufig: Sehstörungen.
Gelegentlich: Geschmacksstörungen.
Gelegentlich: Harnverhaltung.
Gelegentlich: Reaktionen an der Applikationsstelle wie Reizungen, Schmerzen und Geschwürbildung sowie Zahnfleischbluten.
Über die zu ergreifenden Gegenmassnahmen entscheidet Ihr Arzt bzw. Ihre Ärztin.
Wenn Sie sich während der Anwendung von Actiq übermässig schwindelig, schläfrig oder anderweitig krank fühlen, nehmen Sie die Dosiereinheit am Applikator aus dem Mund heraus und entsorgen Sie sie gemäss den Anweisungen in dieser Patienteninformation. Für weitere Anweisungen zur Anwendung von Actiq wenden Sie sich dann bitte an Ihren Arzt bzw. Ihre Ärztin.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Der schmerzstillende Wirkstoff von Actiq ist sehr stark wirksam und könnte lebensbedrohlich sein, wenn er versehentlich von einem Kind eingenommen wird. Actiq muss für Kinder unzugänglich aufbewahrt werden.
Das Arzneimittel darf nur bis zu dem auf der Blisterpackung und der Faltschachtel mit «EXP» bezeichneten Datum verwendet werden.
Die Lutschtablette ist normalerweise weiss, bei der Lagerung kann sie aber ein leicht gesprenkeltes Aussehen annehmen. Dies ist auf leichte Veränderungen der Aromastoffe zurückzuführen und hat keinerlei Einfluss auf die Wirksamkeit des Arzneimittels.
Nicht über 30 °C lagern.
Lassen Sie Actiq immer solange in der Blisterpackung, bis Sie bereit zur Anwendung sind. Sie dürfen Actiq nicht verwenden, wenn die Blisterpackung beschädigt oder geöffnet wurde.
Wenn Sie Actiq nicht länger anwenden, oder wenn Sie ungebrauchte Actiq Dosiereinheiten zu Hause haben, bringen Sie alle ungebrauchten Packungen zu Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin zurück.
Eine teilweise aufgebrauchte Actiq-Dosiereinheit kann genug Wirkstoff enthalten, um für ein Kind gesundheitsschädlich oder lebensbedrochlich zu sein. Auch wenn nur noch wenig oder überhaupt kein Arzneimittel mehr auf dem Applikator zurückgeblieben ist, muss der Applikator selbst ordnungsgemäss wie folgt entsorgt werden:
