Acicutan enthält eine dem Vitamin A verwandte Substanz und hat ähnliche Wirkungen. Das Vitamin A selbst wird dem menschlichen Körper mit der Nahrung zugeführt und spielt eine wesentliche Rolle bei der Bildung, der Erneuerung und dem Schutz gewisser Körperzellen, vor allem in den oberen Haut- und Schleimhautschichten.
Acicutan kann Störungen bei der Erneuerung von Hautzellen, wie sie bei Schuppenflechte und anderen schweren Verhornungsstörungen der Haut auftreten, beheben.
Ihr Arzt oder Ihre Ärztin wird Ihnen Acicutan verabreichen, wenn Sie an schwerer und ausgedehnter Psoriasis oder anderen schweren Verhornungsstörungen leiden, die auf anderweitige Behandlungen nicht befriedigend ansprechen. Die Wirksamkeit von Acicutan hängt von der Art der vorliegenden Verhornungsstörung ab.
Acicutan darf nur auf Verschreibung des Arztes oder der Ärztin eingenommen werden. Die Verabreichung von Acicutan und die Überwachung der Behandlung gehören in die Hand eines Arztes bzw. einer Ärztin für Hautkrankheiten. Desgleichen erfordern die speziell für Acicutan geltenden Vorsichtsmassnahmen eine ständige ärztliche Kontrolle.
Wenn Sie schwanger sind, dürfen Sie Acicutan unter keinen Umständen einnehmen. Dies gilt auch für Frauen, die schwanger werden könnten, wenn nicht vier Wochen vor, während und zwei Jahre lang nach der Behandlung eine strenge Kontrazeption durchgeführt wird. Lesen Sie unbedingt die rot gedruckten Anweisungen (siehe Rotdruck) genau durch!
Acicutan soll bei schweren Krankheiten der Leber oder der Nieren nicht eingenommen werden, auch nicht bei erhöhten Blutfettwerten oder bei Überempfindlichkeit gegen das Präparat.
Acicutan wird nach Gesichtspunkten verordnet, die von Patient zu Patient bzw. von Patientin zu Patientin anders sein können.
Es wird Ihnen persönlich verschrieben, und Sie dürfen das Arzneimittel und die für Sie geltenden Vorschriften unter keinen Umständen jemand anderem weitergeben.
Eine gewisse Anzahl Laboruntersuchungen sind während der Behandlung unerlässlich, wofür man Ihnen eine oder mehrere Blutproben entnehmen wird. Akzeptieren Sie die Ratschläge Ihres Arztes oder Ihrer Ärztin und befolgen Sie diese peinlich genau.
Informieren Sie Ihren Arzt oder Ihre Ärztin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden.
Die gleichzeitige Einnahme von Acicutan und Vitamin-A-haltigen Arzneimitteln oder Stärkungsmitteln sowie Alkohol ist zu vermeiden.
Während der Behandlung mit Acicutan sollten Sie auf Sonnenbäder verzichten.
Während und ein Jahr nach Behandlung mit Acicutan darf kein Blut gespendet werden.
Während der Behandlung mit Acicutan dürfen Frauen im gebärfähigen Alter keinen Alkohol zu sich nehmen. Alkohol sollte bis 2 Monate nach Absetzen der Therapie mit Acicutan gemieden werden.
In Patientinnen und Patienten, die mit Arzneimitteln behandelt wurden, welche Ähnlichkeit zu Vitamin A und Acicutan haben, wurden sehr selten Stimmungsänderungen, aggressive Gefühle und suizidale Gedanken beobachtet. Da auch andere Faktoren zu diesen Symptomen beigetragen haben können, ist nicht sicher, ob diese in Zusammenhang mit der Acicutan Behandlung stehen. Falls bei Ihnen diese Anzeichen auftreten, sollten Sie sofort die Einnahme von Acicutan beenden, Ihren Arzt bzw. Ihre Ärztin aufsuchen und ihn bzw. sie darüber informieren.
Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fähigkeit Werkzeuge oder Maschinen zu bedienen und die Fahrtüchtigkeit beeinträchtigen:
Unter der Behandlung mit Acicutan ist eine Beeinträchtigung des Dämmerungssehens beobachtet worden. Sind Sie deshalb vorsichtig, wenn Sie nachts im Strassenverkehr sind oder ein Fahrzeug lenken.
? Acicutan ist fruchtschädigend, das heisst es kann Schädigungen des ungeborenen Kindes bewirken und infolgedessen zur Geburt eines missgebildeten Kindes führen.
? Wenn Sie schwanger sind, dürfen Sie Acicutan unter keinen Umständen einnehmen.
? Nach Abschluss einer Behandlung mit Acicutan muss eine Schwangerschaft unbedingt für weitere zwei Jahre verhütet werden.
? In besonderen Fällen kann der Arzt oder die Ärztin Acicutan Frauen verschreiben, die schwanger werden könnten. Falls dies auf Sie zutrifft, ist es absolut unerlässlich, die ärztlichen Vorschriften genauestens einzuhalten. Folgende Punkte sind dann äusserst wichtig:
1. Vor Beginn der Behandlung muss absolut sichergestellt werden, dass Sie nicht schwanger sind.
2. Die Massnahmen zur Empfängnisverhütung, die Ihnen der Arzt oder die Ärztin verordnet hat, müssen Sie strikt einhalten, und zwar nicht nur vier Wochen vor und während der Behandlung mit Acicutan, sondern noch mindestens zwei Jahre nach ihrer Beendigung, da das Präparat eine gewisse Zeit im Körper gespeichert wird. Hat Ihnen Ihr Arzt oder Ihre Ärztin zur Empfängnisverhütung eine Antibabypille verschrieben, hat er bzw. sie Ihnen mit Absicht keine sogenannte «Minipille» empfohlen, da deren Wirksamkeit durch Acicutan abgeschwächt würde.
3. Falls Sie trotz strikter Empfängnisverhütung während der Behandlung mit Acicutan oder innerhalb von zwei Jahren nach deren Beendigung schwanger werden oder den Verdacht haben, schwanger zu sein, müssen Sie sofort Ihren Arzt oder Ihre Ärztin benachrichtigen. Er bzw. sie wird die unmittelbar erforderlichen Massnahmen ergreifen.
Stillende Mütter dürfen Acicutan nicht einnehmen.
Ihr Hautarzt bzw. Ihre Hautärztin ist am besten in der Lage, Ihnen zusätzliche Information über Acicutan zu geben. Wenn Sie unsicher sind, wenden Sie sich an ihn oder sie.
Ihr Arzt oder Ihre Ärztin legt die Anzahl Kapseln fest, die Sie täglich einnehmen müssen. Befolgen Sie die ärztlichen Angaben peinlich genau, und ändern Sie die Dosis nie, ohne ihn oder sie vorher zu Rate zu ziehen. Schlucken Sie die Kapseln unzerkaut einmal täglich während einer Mahlzeit oder mit einem Glas Milch. Die vom Arzt oder der Ärztin verordnete Dosis wird zwischen 25 und 50 mg pro Tag liegen; gelegentlich wird es nötig sein, dass er bzw. sie vorübergehend bis 75 mg pro Tag verordnet.
Die meisten Nebenwirkungen treten vor allem am Anfang der Behandlung auf. Im Allgemeinen verschwinden sie wieder, wenn die Therapie beendet ist. Der Arzt bzw. die Ärztin wird Sie beraten, wie Sie die unerwünschten Begleiterscheinungen mildern können.
Folgende Nebenwirkungen können bei der Einnahme von Acicutan auftreten: Nasen-, Mund- und Lippentrockenheit, Durst, Austrocknen der Schleimhäute, Geschmacksstörungen, Verdünnung der Haut vor allem auf Handflächen und Fusssohlen, Abschuppung, Hautrötung, Juckreiz, Sonnenempfindlichkeit, Veränderung der Haarstruktur, Haarausfall, Nagelbrüchigkeit, Nagelfalzentzündung, Augenbeschwerden mit evtl. Unverträglichkeit von Kontaktlinsen, Verschwommensehen, vermindertes Nachtsehen, Kopfschmerzen, Muskel-, Gelenks- und Knochenschmerzen, Magen-Darm-Störungen und Flüssigkeitsansammlungen an Händen und Füssen.
Benachrichtigen Sie Ihren Arzt oder Ihre Ärztin baldmöglichst, falls Sie an ungewöhnlich starken oder länger dauernden Kopfschmerzen, an Sehstörungen, Übelkeit, Durchfall oder irgendwelchen anderen Problemen leiden; diese Erscheinungen machen unter Umständen eine Anpassung Ihrer Behandlung erforderlich. Die Entscheidung, eine Behandlung abzubrechen, sollte nur vom Arzt oder der Ärztin gefällt werden.
In der Regel sind mehrere Wochen nötig, bis ein zufriedenstellendes Behandlungsresultat eintritt. Bei einzelnen Patienten bzw. Patientinnen muss manchmal die Behandlung mit niedrigen Dosen einige Monate oder sogar mehrere Jahre lang fortgeführt werden.
Acicutan wirkt auf die Symptome der Hautkrankheit, beseitigt aber nicht die Ursache, die bislang noch unbekannt ist. Daher kann bei einem Rückfall eine erneute Behandlung nötig werden, falls der Arzt oder die Ärztin dies als nützlich erachtet.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Das Arzneimittel soll nicht über 30 °C gelagert werden. Es ist in seiner Originalpackung vor Licht und Feuchtigkeit geschützt und für Kinder unerreichbar aufzubewahren. Es darf nur bis zu dem auf dem Behälter mit EXP bezeichneten Datum verwendet werden.
Weitere Auskünfte erteilt Ihnen Ihr Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin. Diese Personen verfügen über die ausführliche Fachinformation.
